Cataract Surgery in Eyes with Microphthalmos and/or Uveal Coloboma.

INTRODUCTION: Cataract may cause severe visual impairment in eyes with microphthalmos (MO) and uveal coloboma (UC). Despite their similarities, distinguishing between these conditions is crucial for cataract surgery. We aimed to compare complications, nucleus hardness, surgical strategies, and outcomes of cataract surgery between MO and UC. METHODS: This retrospective comparative study included 19 eyes with MO and 20 with UC. Preoperative, intraoperative, and postoperative data of eyes that underwent cataract surgery were analysed. RESULTS: MO eyes had lower high-hardness nucleus rates (p =

The adhesion behavior of the retina.

Ocular diseases and treatment related to rhegmatogenous retinal detachment (RRD) are highly correlated with retinal adhesion behavior. Therefore, this paper proposes to study the adhesion behavior of the intact retina. This can provide theoretical guidance for the treatment and research of retinal detachment (RD) related diseases. To systematically analyze this aspect, two experiments were performed on the porcine retina. The pull-off test combined with the modified JKR theory was used to study the adhesion behavior of the vitreoretinal interface, while the peeling test was used to study the a

Molecular targets in proliferative vitreoretinopathy: rethinking prevention, treatment and drug delivery after rhegmatogenous retinal detachment.

INTRODUCTION: Proliferative vitreoretinopathy (PVR) is the leading cause of surgical failure after rhegmatogenous retinal detachment repair and the principal indication for repeat vitreoretinal surgery, yet four decades of mechanistic research have not produced an effective pharmacological adjunct. This review reexamines why, and asks how prevention, treatment and drug delivery might be reframed. AREAS COVERED: This narrative expert review conceptualizes PVR as a temporally evolving, overlapping fibro-inflammatory continuum spanning inflammation and chemotaxis, retinal pigment epithelial acti…

Removal of subretinal strands without creating an intentional retinal hole: A case report.

BACKGROUND: Proliferative vitreoretinopathy (PVR) is a severe vitreoretinal disease. In cases of PVR with subretinal strands (SRS), creating an intentional retinal hole is typically necessary for removing SRS during vitrectomy. Herein, we describe a novel surgical approach for managing total retinal detachment (RD) with SRS. CASE REPORT: A 59-year-old man presented with vision loss in the right eye persisting for four years before the initial presentation. Visual acuity was limited to light perception. The right eye had a mature cataract, and ultrasound computed tomography indicated total ret

Posterior vitreous detachment rates post-uncomplicated phacoemulsification surgery: a systematic review.

PURPOSE: It is commonly accepted that phacoemulsification surgery is a risk factor for the development of posterior vitreous detachment (PVD) and may accelerate the process. This is an important consideration particularly in cases involving young patients who pre-operatively have no PVD, given the increased risk of retinal tears and detachments. METHODS: A comprehensive literature search was conducted to identify studies reporting incidence of PVD post-uncomplicated phacoemulsification surgery. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement was used for searc

Postsurgical cystoid macular edema.

Cystoid macular edema (CME) is a primary cause of reduced vision following both cataract and successful vitreoretinal surgery. The incidence of clinical CME following modern cataract surgery is 0.1-2.35%. Preexisting conditions such as diabetes mellitus and uveitis as well as intraoperative complications can raise the risk of developing CME postoperatively. The etiology of CME is not completely understood. Prolapsed or incarcerated vitreous and postoperative inflammatory processes have been proposed as causative agents. Pseudophakic CME is characterized by poor postoperative visual acuity. Flu…

The role of scleral buckle in experimental posterior penetrating eye injury.

PURPOSE: Although episcleral buckles are frequently placed as an additional procedure to vitreoretinal surgery, little is known about their independent effect after ocular trauma. The authors created a posterior penetrating ocular injury model to evaluate the isolated role of primary episcleral buckle placement. METHODS: Twenty eyes underwent surgery. The penetrating injury consisted of two 5-mm circumferential incisions placed five clock hours apart and 8 mm behind the limbus. A segmental episcleral buckle was placed over a randomly chosen injury site after wound closure. The degrees of fibr…

Idiopathic vitreomacular traction and macular hole: a comprehensive review of pathophysiology, diagnosis, and treatment.

Posterior vitreous detachment (PVD) is a common phenomenon in the aging eye. However, this may be complicated by persistent symptomatic vitreomacular adhesions that exert tractional forces on the macula (vitreomacular traction; VMT). VMT itself may be associated with epiretinal membrane formation and the development of idiopathic macular holes (IMH). Such pathologies may cause visual disturbances, including metamorphopsia, photopsia, blurred vision, and decreased visual acuity, which impact an individual's quality of life. Technologies such as optical coherence tomography allow an increasingly

The treatment of advanced retinopathy of prematurity by cryotherapy and scleral buckling surgery.

Seventeen eyes of nine extremely premature infants with severe acute proliferative retinopathy of prematurity (ROP, Grades III-V) were treated. Cryotherapy alone was used in ten eyes to ablate extensive areas of avascular retina to thereby induce involution of widespread intravitreous neovascularization. No attempt was made to directly treat the arteriovenous shunt or neovascularization itself. Scleral buckling surgery was used in combination with cryotherapy in seven additional eyes to relieve diffuse vitreous traction to intravitreous neovascularization which had caused extensive traction re

Controversies, consensuses, and guidelines on macular hole surgery by the Asia-Pacific Vitreo-retina Society (APVRS) and the Asia-Pacific Academy of Professors in Ophthalmology (AAPPO).

Macular hole surgery, primarily pars plana vitrectomy with internal limiting membrane peeling (ILM) and gas tamponade, has become the standard of care for full-thickness macular hole (FTMH). Despite the 85% to 95% anatomical closure rate, several aspects of the procedure are well accepted whereas some may remain controversial among vitreoretinal surgeons. An international panel of experts (IPE) comprising 27 experts from 10 countries/territories was established to evaluate a total of 38 consensus statements on ILM peeling extent, vital dye selection, face-down positioning requirements, tampona

Challenges and outcomes of cataract surgery after vitrectomy.

PURPOSE OF REVIEW: This review examines the challenges and outcomes of cataract surgery after pars plana vitrectomy (PPV), focusing on surgical techniques, timing, and complication management. RECENT FINDINGS: Cataract formation remains the primary complication post-PPV, affecting approximately 80-100% of patients within two years. Nuclear sclerotic cataracts are most common, occurring in 60-100% of patients over 50, followed by posterior subcapsular cataracts (4-34%), which primarily affect younger and diabetic patients. PPV disrupts the normal oxygen gradient in the vitreous, resulting in a
