The history of anxiety symptoms among 171 primary alcoholics.

This study used patient and resource person face-to-face interviews to explore the history of anxiety symptoms and syndromes in 171 primary alcoholic male veterans on an alcohol treatment program. Almost all men (98%) reported at least one symptom of anxiety during drinking or withdrawal, including 80% who related problems with palpitations and/or shortness of breath. In addition, seven men (4%) described at least one episode of panic lasting from 1 to 4 or more hours, although only two of these individuals experienced three or more panic attacks in a 3-week period in the context of heavy drin

Adaptation of self-image in burn-disfigured children.

Massive burn injury alters both the physical appearance and the functional ability of the child. The changed body necessitates concomitant alteration of the internal self-image, including feelings about the internal self or self-esteem. Six children of primary school age are being followed up in a prospective, longitudinal study to track their adaptation to burn disfigurement. Results of the first 5 1/2 years reveal that, initially, all children displayed pervasive developmental regression, accompanied by phobias, nightmares, and various other symptoms. Two years later, one half of the childre

Long-term psychological distress among chemically exposed firefighters.

Long-term psychological effects of exposure to a dangerous chemical were evaluated in a sample of firefighters who fought a large polyvinyl chloride (PVC) fire. When compared with nonexposed firefighter controls (n = 22), the exposed firefighters (n = 64) had significantly higher levels of demoralization and specific emotional distress 22 months after the incident. Longitudinal analyses of a subsample of exposed firefighter subjects (n = 55) surveyed 5 to 6 weeks after the fire and again 22 months later revealed that there was no reduction in symptomatology over time. Some psychological distre

Sounds of silence--coping with hearing loss and loneliness.

The vast majority of people will suffer presbycusis during the aging process. This hearing loss has potential social consequences, which place these individuals at risk for loneliness. Nurses need to encourage thorough audiological examination and follow through on recommendations to improve hearing. The use of hearing aids needs to be encouraged. A suggested model of interaction among variables influencing loneliness identifies areas amenable to nursing intervention. Frequency of significant interaction and motivation to engage in interactions represent two important variables that influence

Individual psychotherapy with victims of incest.

In treating victims of incest, therapists need to be aware of the gamut of symptoms, personality traits, interpersonal difficulties, and clinical syndromes that occur with special frequency in this population. A list of these features is provided. Vignettes with typical therapist-patient dialogue are offered, demonstrating techniques for handling the initial consultation and opening phase of treatment. During later phases of treatment the focus will usually shift to the discussion of the pertinent symptoms (such as suicidal feelings, substance abuse, dissociative phenomena, sexual disturbances

[Neurotic and neurosis-like disorders in the school maladaptation of children with retardation of mental development].

Sixty-six schoolchildren aged 6 to 11 attending community schools suffered from various types of psychophysical infantilism and encephalopathic forms (in the cerebrasthenia and psycho-organic syndrome background) of mental retardation. The decompensation of the residual organic cerebral insufficiency with enhancement of neurosis-like and cerebrasthenic disorders resulting from increased mental load was the first stage of these children disadaptation to the school demands. At the 2nd stage, as the self-esteem lowered and negative attitude of other schoolchildren arose, the neurotic disorders em

Emotional fainting and the psychophysiologic response to blood and injury: autonomic mechanisms and coping strategies.

This experiment was designed to examine the psychophysiologic response pattern associated with emotional fainting. A survey was carried out among students, from which 30 volunteers who reported that they felt faint at the sight of blood or injury were recruited, together with 26 nonfainters. Blood pressure, heart rate, and respiration rate were monitored while subjects viewed a film depicting open-heart surgery and a neutral control film. Parasympathetic cardiac control was indexed by respiratory sinus arrhythmia (RSA), and coping was assessed by questionnaire. Responders (n = 17, group FPOS)

Intimacy motivation and subjective mental health in a nationwide sample.

Over 1,200 adults in a representative nationwide sample were administered the Thematic Apperception Test (TAT) and an unstructured interview which produced 25 measures of subjective mental health and ultimately six factor scores: unhappiness, lack of gratification, strain, feelings of vulnerability, lack of self-confidence, and uncertainty. The TATs were scored for intimacy motivation (McAdams, 1980)--a recurrent preference or readiness for experiences of warm, close, and communicative interaction with others. Controlling for age, education, and their interaction, high intimacy motivation in w

Appraisal, coping, health status, and psychological symptoms.

In this study we examined the relation between personality factors (mastery and interpersonal trust), primary appraisal (the stakes a person has in a stressful encounter), secondary appraisal (options for coping), eight forms of problem- and emotion-focused coping, and somatic health status and psychological symptoms in a sample of 150 community-residing adults. Appraisal and coping processes should be characterized by a moderate degree of stability across stressful encounters for them to have an effect on somatic health status and psychological symptoms. These processes were assessed in five

Genetic counselling--a contemporary approach.

With improvements in living standards in many parts of the world and advances in medicine and surgery, infectious diseases and nutritional deficiencies are becoming less common causes of morbidity and mortality. As these environmental diseases have declined, others have been uncovered which are largely, or even entirely, genetically determined.

Panic disorder and alcohol misuse.

The literature on the relationship of panic disorder to alcoholism is reviewed. Four cases of panic disorder are presented, illustrating instances in which the diagnosis of panic disorder was delayed or obscured by alcoholism. Professionals in alcoholism treatment should therefore be better able to recognize panic disorder. Also, since tricyclic antidepressants are very effective in the treatment of panic disorder, the opposition in the alcoholism field to the use of any medication is criticized.

Personality correlates and degree of impairment in male adolescents with progressive and nonprogressive physical disorders.

Investigated the personality profiles (MMPI) of 44 adolescents with progressive (Duchenne Muscular Dystrophy) and nonprogressive (mixed orthopedic) physical impairment to determine the relationship among personality correlates, disability type, functional level of impairment, and rapidity of decline in those with progressive disease. Both groups of males evidenced many similarities to severe physical limitations regardless of diagnosis or progression of their physical status. Common personality characteristics of increasing social inhibition, passiveness, and depression were identified as a re

Depression secondary to cancer.

Depression is the most frequent psychiatric complication experienced by cancer patients. Recent surveys indicate that 17--25% of patients hospitalized with neoplastic disease suffer from depression severe enough to warrant psychiatric intervention. The disorder tends to be reactive in nature and occurs mot frequently among the severely ill. Despite an increased risk of suicide, self-destruction remains a rare occurrence in cancer victims. The most important etiologic factors are associated with the disease itself but additional factors have to do with its treatment. Those related to the illnes

A comparative study of psychosexual adjustment in men with testicular cancer and acute leukemia.

Twenty-five men with testicular germ cell tumors were compared by developmental history and past and present psychologic adjustment to 25 men with acute leukemia. The mean age was 30 years for the cancer group and 25 years for the leukemia group. Current and Past Psychopathology Scales (18 scales of prior and 8 of present adjustment) were rated during a semistructured interview. The following differences were found in developmental history: Onset of puberty was 12.4 years for leukemics and 15.1 years for the cancer group (P less than 0.001); cryptorchidism was found in 20% of cancer patients a

[The psychology of pacemaker patients].

Examinations of 50 patients with pacemakers showed in relation to 50 control persons that the former reacted on their specific situation with emotional and behaviour disturbances. In relation to the control group there were signs of neurotic developments marked by hysterical symptoms, hypochondriac fears, and depressive reactions. Simultaneously there existed obvious tendences to minimize the problem of the implantation. Furthermore increased grievances of multiple bodily complaints were observed. In contrast to this the intellectual capacities of the patients provided with a pacemaker general

Culture change and mental illness.

A study of the relationship of culture change to the frequency of mental illness in the outpatient clinic in the city of Cairo indicates that rapid industrialisation tends to produce significantly higher rates of patients who were judged to have serious symptoms of depression especially among village migrants who moved from their villages to work in the city's factories. A suggestive explanation is that the higher rates of depression are a factor of the social disorganisation associated with moving from the village to the city.

Scapegoating and the unpopular nurse.

This paper considers the theories of scapegoating and their application to nursing. It reviews the literature on scapegoating drawing particular attention to those studies which have emphasised on the instrumental uses of scapegoating in organisational contexts. The paper draws on data from a qualitative study of the identification and management of the 'problem' nurse conducted in three healthcare Trusts. It describes a number of illustrative cases in which different types of scapegoating occurred. The paper argues that scapegoating is an ethical challenge which nurses must face.

Adolescent homosexuality and concerns regarding disclosure.

Development of sexual identity in middle childhood and early adolescence is a natural process. However, it is more stressful for homosexual adolescents. Society continues to stigmatize and marginalize homosexuality. To avoid rejection and hostility, homosexual adolescents are pressured to hide their sexual identities. This fact compounds the anticipated normal developmental concerns of adolescence, and can create unique problems for the homosexual adolescents. Homosexuality can place them at risk for social stigmatization, isolation, depression, suicide, abuse, and rejection by their families
