BACKGROUND: Granulocyte-monocyte colony stimulating factor (GM-CSF) is the treatment of choice for patients with life threatening neutropenias. Hypersensitivity to GM-CSF may lead to cessation of treatment. Hyposensitization (immunotherapy) of hymenoptera venom allergy has been practised for 70 years. About 20 years ago the use of ineffective whole-body extracts was abandoned, as effective therapy with preparations of bee venom and wasp venom became available. The diagnosis of hymenoptera venom allergy is based on history, skin tests and determination of venom specific IgE-antibodies in the serum. Late asthmatic reaction (LAR) and dual asthmatic reaction (DAR) were developed in guinea pigs by repetitive treatment with toluene 2,4-diisocyanate (TDI). We report a case of an 8-year-old atopic boy in whom ingestion of a chamomile-tea infusion precipitated a severe anaphylactic reaction. Calmodulin antagonists, which block intracellular calcium/calmodulin-dependent processes like exocytosis, inhibit effectively mast cell mediator release in vitro. Our experiments confirm that they are also effective in vivo. Thioridazine (5 mg/kg), a representative of the group, prevents the severe anaphylactoid shock in the rat, induced with compound 48/80, and permits a rapid and complete recovery, with special protection of respiration. Thus, calmodulin antagonists seem to be potential drugs for the management of mast cell-mediated allergic diseases.

This study extends our experience applying the criterion of a fall in serum venom-specific IgE (RAST) to insignificant levels as an indication to stop venom immunotherapy (VIT) and compares the follow-up results with the results of re-stings in patients who stopped VIT "prematurely" by self-choice. Serum IgE antibody responses were generated in mice by intranasal exposure to grass pollen extract. The most significant complications of hydatid cyst of the liver are rupture and suppuration with an incidence of 6-19%. Rupture into the biliary tract was the most frequent complication and comprised 170 out of 222 complicated hydatid cysts of the liver in the author's series. These jaundiced patients were managed both by obliteration of the residual cavity and restoration of the patency of the biliary tract which was obstructed by daughter cysts of remnants of the laminated membrane. Rupture into the bronchi requires immediate treatment due to bronchopulmonary complications which might ensue.

The effect of naloxone on a number of experimental shock models, using the anaesthetized rat, was studied with special emphasis on mean arterial blood pressure (MABP) and chance of survival. Only a slight increase in MABP was noted in haemorrhagic shock models whereas survival was not affected. Naloxone was without effect in endotoxin shock (i.p. administration of endotoxin). The effects of splenectomy on antibody synthesis and delayed hypersensitivity (DH) reactions were studied in Bacillus Calmette-Guerin (BCG)-treated guinea pigs injected intravenously with an hapten-carrier complex.
