Molecular phylogeny of the blowfly genus Chrysomya.

Chrysomya Robineau-Desvoidy (Diptera: Calliphoridae) is a genus of blowfly commonly observed in tropical and subtropical countries of the Old World. Species in this genus are vectors of bacteria, protozoans and helminths, cause myiasis, are predators of other carrion insects, and are important forensic indicators. Hypotheses concerning the evolution of sex determination, larval anatomy and genome size in Chrysomya have been difficult to evaluate because a robust phylogeny of the genus was lacking. Similarly, the monophyly of subgenera was uncertain. The phylogeny of Chrysomya spp. was reconstr

Validation of an automated safety surveillance system with prospective, randomized trial data.

OBJECTIVE: We sought to validate 3 methods for automated safety monitoring by evaluating clinical trials with elevated adverse events. METHODS: An automated outcomes surveillance system was used to retrospectively analyze data from 2 randomized, TIMI multicenter trials. Trial A was stopped early due to elevated 30-day mortality rates in the intervention arm. Trial B was not stopped early, but there was transient concern regarding 30-day intracranial hemorrhage rates. We compared statistical process control (SPC), logistic regression risk adjusted SPC (LR-SPC), and Bayesian updating statistic

Sequential actions of Pax3 and Pax7 drive xanthophore development in zebrafish neural crest.

The Pax3/7 gene family has a fundamental and conserved role during neural crest formation. In people, PAX3 mutation causes Waardenburg syndrome, and murine Pax3 is essential for pigment formation. However, it is unclear exactly how Pax3 functions within the neural crest. Here we show that pax3 is expressed before other pax3/7 members, including duplicated pax3b, pax7 and pax7b genes, early in zebrafish neural crest development. Knockdown of Pax3 protein by antisense morpholino oligonucleotides results in defective fate specification of xanthophores, with complete ablation in the trunk. Other p

Evolution and dispersal of St. Louis encephalitis virus in the Americas.

Using a Bayesian coalescent approach on a dataset of 73 envelope gene sequences we estimated substitution rates and dates of divergence for St. Louis encephalitis virus (SLEV) in the Americas. We found significant rate heterogeneity among lineages, such that "relaxed" molecular clock models were much better supported than a strict molecular clock. The mean substitution rate estimated for all SLEV was 4.1x10(-4)substitutions/site/year (95% HPD 2.5-5.7)-higher than previous estimates that relied on the less well-suited strict clock. Mean substitution rates for individual lineages varied from 3.7

Incorporating medical interventions into carrier probability estimation for genetic counseling.

BACKGROUND: Mendelian models for predicting who may carry an inherited deleterious mutation of known disease genes based on family history are used in a variety of clinical and research activities. People presenting for genetic counseling are increasingly reporting risk-reducing medical interventions in their family histories because, recently, a slew of prophylactic interventions have become available for certain diseases. For example, oophorectomy reduces risk of breast and ovarian cancers, and is now increasingly being offered to women with family histories of breast and ovarian cancer. Men

Comparison of Bayesian and frequentist approaches in modelling risk of preterm birth near the Sydney Tar Ponds, Nova Scotia, Canada.

BACKGROUND: This study compares the Bayesian and frequentist (non-Bayesian) approaches in the modelling of the association between the risk of preterm birth and maternal proximity to hazardous waste and pollution from the Sydney Tar Pond site in Nova Scotia, Canada. METHODS: The data includes 1604 observed cases of preterm birth out of a total population of 17559 at risk of preterm birth from 144 enumeration districts in the Cape Breton Regional Municipality. Other covariates include the distance from the Tar Pond; the rate of unemployment to population; the proportion of persons who are sepa

Predicting renal graft failure using multivariate longitudinal profiles.

Patients who have undergone renal transplantation are monitored longitudinally at irregular time intervals over 10 years or more. This yields a set of biochemical and physiological markers containing valuable information to anticipate a failure of the graft. A general linear, generalized linear, or nonlinear mixed model is used to describe the longitudinal profile of each marker. To account for the correlation between markers, the univariate mixed models are combined into a multivariate mixed model (MMM) by specifying a joint distribution for the random effects. Due to the high number of marke

Bayesian analysis for cystic fibrosis risks in prenatal and carrier screening.

PURPOSE: Risk assessment is an essential component of genetic counseling and testing, and Bayesian analysis plays a central role in complex risk calculations. We previously developed generalizable Bayesian methods to calculate the autosomal recessive disease risk of a fetus when one or no mutation is detected, and another, independent risk factor is present. Our methods are particularly useful for calculating the CF disease risk for a fetus with echogenic bowel. In genetics practice, however, there are other scenarios for which our previous methods are inadequate. METHODS AND RESULTS: We prov

Rethinking a universal framework in the psychiatric symptom-disorder relationship.

Research on disparities in health and services should include a critical examination of how social, cultural, and contextual factors affect the connection between symptoms and psychiatric disorders. Systematic differences in this connection by these factors will mean that clinicians using a universalist approach to disorders may make more diagnostic errors for certain patients (e.g., ethnic minorities, the poor). Based on the literature on normative clinical decision-making, this study tests for whether the effect of specific psychiatric disorders on key symptoms is the same for certain groups

Epidemiology and burden of facial fractures in China, 1990-2021: findings from the Global burden of disease study 2021.

OBJECTIVE: Facial fractures can be disabling injuries, leading to long-term functional, aesthetic, and psychological impairments. Comprehensive assessment of their disease burden in China is essential for informed health policy and prevention strategies. METHODS: Data regarding facial fractures in China from 1990 to 2021 were extracted from the Global Burden of Disease study (GBD) 2021 database. Incidence, prevalence, and years lived with disability (YLDs) were estimated using DisMod-MR 2.1, a Bayesian meta-regression modeling tool. Estimates were presented as numbers and rates, with comparis

Bayesian Spatial Analysis of Trends and Disparities in Telehealth Use During the COVID-19 Pandemic: Retrospective Observational Study.

BACKGROUND: Telehealth has emerged as an essential health care tool, particularly during the COVID-19 pandemic, when in-person medical services were significantly restricted. While telehealth adoption surged during the pandemic, disparities in its access and use have been observed, especially among vulnerable populations. Understanding these trends and identifying barriers is crucial for promoting equitable health care delivery. OBJECTIVE: This study aims to assess disparities in telehealth use across Virginia, focusing on demographic, socioeconomic, and geographic factors influencing access.

Explainable machine learning with bayesian hyper-optimization for predicting cognitive impairment from longitudinal nursing home data.

The monitoring of daily life in nursing home residents generates diverse and heterogeneous sources of information. Artificial Intelligence (AI) is increasingly used to predict a wide range of outcomes in both research and clinical practice, including mortality and cognitive impairment (CI). A key challenge is determining which information sources (IS) provide the most accurate predictions. In this work, we present an integrative AI-based framework that combines harmonized temporal modeling, Bayesian hyperparameter optimization, XGBoost, and explainable AI (SHAP) to predict CI in nursing home r

Bayesian reanalysis of early remdesivir for the treatment of COVID-19 in outpatients with high risk of progression to severe disease.

BACKGROUND: Though Bayesian methods are flexible, intuitive, and readily incorporated into clinical decision-making, with particular utility when prior information is available, they remain underutilized in the analysis of clinical trials. METHODS: In PINETREE, a Phase 3 randomized controlled trial (RCT) of remdesivir (RDV) for the treatment of outpatients with COVID-19 at high risk of severe disease, the primary outcome of COVID-19-related hospitalization or all-cause death was reanalyzed using a range of reference and data-driven priors. Posterior probability distributions were used to calc

Uric acid levels mediate the association between four dietary indices and kidney stones in US adults: A cross-sectional study of NHANES 2007-2018.

BACKGROUND: Diet and uric acid are closely linked to the formation of kidney stones. However, the specific dietary indices associated with kidney stone risk and the mediating role of uric acid remain unclear. This study investigates the association between four dietary indices and kidney stone risk while exploring the mediating role of uric acid. METHODS: Data were obtained from the National Health and Nutrition Examination Survey (NHANES). Four dietary indices were calculated: the Mediterranean Diet (MED), Healthy Eating Index-2020 (HEI-2020), Alternate Healthy Eating Index (AHEI), and Dieta
