BACKGROUND: Deep inferior epigastric artery perforator (DIEP) flap breast reconstruction depends on perfusion and outflow through microvascular anastomoses. Preoperative anemia is predictive of transfusion and increased complications; however, transfusion is also linked to thrombosis. Moreover, guidelines for transfusion after DIEP flaps are not established. We aimed to identify a safe hemoglobin level after DIEP flaps and identify risk factors and complications associated with transfusion.

Serum ferritin, liver iron stores, and liver histology were studied in 38 children with thalassaemia major who were being treated by regular blood transfusions. Chronic transfusion therapy (CTT) for sickle cell disease (SCD) reduces disease morbidity by suppressing the amount of circulating hemoglobin S (HbS)-containing red blood cells (RBC). The effectiveness of CTT depends on the rate of RBC clearance. This study has estimated the risk of Leishmania transmission via blood transfusion in one of the largest blood banks in Northeastern Brazil, where visceral leishmaniasis is endemic. Five hundred blood samples from donors were tested for circulating Leishmania spp. DNA by real-time PCR.
