Universal prevention of depression in women postnatally: cluster randomized trial evidence in primary care.

BACKGROUND: To test whether receiving care from a health visitor (HV) trained in identification and psychological intervention methods prevents depression 6-18 months postnatally in women who are not depressed 6 weeks postnatally. METHOD: The study was a prospective cluster trial, randomized by GP practice, with follow-up for 18 months in 101 primary care teams in the Trent area of England. The participants were women scoring <12 on the postal Edinburgh Postnatal Depression Scale (EPDS) at 6 weeks postnatally (1474 intervention and 767 control women). Intervention HVs (n=89, 63 clusters) were

Fuzzy cognitive explainable AI framework integrating ResNet-50 and causal clinical concept reasoning for skin lesion detection.

Melanoma is one of the most aggressive forms of skin cancer, and its early detection from dermoscopic images remains a significant clinical challenge. Deep learning (DL) convolutional neural networks (CNNs) have achieved remarkable results in medical image analysis. However, their inherent opacity is a barrier to trustworthy clinical adoption. Clinicians need not only accurate predictions but also clear, understandable reasons behind those predictions. Although post-hoc explainability techniques such as Grad-CAM and SHapley Additive exPlanations (SHAP) are available, but there is a critical ga

Methodological advances in claims-based dementia algorithms: integrating medication and clinical data for medicare populations.

BACKGROUND: Accurate identification of Alzheimer's Disease and Related Dementias (ADRD) in Medicare data is essential for research, policy, and care planning. Existing approaches, including the Centers for Medicare & Medicaid Services (CMS) Chronic Conditions Warehouse (CCW) algorithm, rely primarily on International Classification of Diseases, Tenth Revision (ICD-10) codes and are subject to misclassification and potential bias across populations. METHODS: We evaluated whether incorporating neuropsychiatric symptom (NPS) ICD-10 codes and medication data improves prediction of algorithm-defin

The effect of integrating pain neuroscience education with stabilization exercises using a flexi-bar on functional disability, pain, and geometry of abdominal and multifidus muscles in patients with chronic non-specific low back pain.

OBJECTIVE: Chronic non-specific low back pain is a multifactorial condition encompassing both physical and psychosocial components, frequently characterized by maladaptive pain-related cognitions and fear-avoidance behaviours. Pain neuroscience education seeks to reconceptualize these cognitions and mitigate pain-related fear. This study evaluated the additional benefits of integrating pain neuroscience education with Flexi-Bar-based stabilization exercises on pain intensity, functional disability, and geometry of the abdominal and lumbar Multifidus muscles in individuals with chronic non-spec

Development and validation of a machine learning model to detect psychiatric symptoms in Huntington's disease using speech analysis.

Huntington's disease (HD) causes progressive disability through motor, psychiatric, and cognitive symptoms. Machine learning speech analysis can detect motor and cognitive symptoms of HD, but not yet psychiatric symptoms. This study investigated whether speech analyses can detect the presence of psychiatric symptoms in HD. Audio recordings of six narrative tasks (cookie-theft picture description, red-riding hood storytelling, most recent 24 hours recalling, happy, sad, or angry storytelling) were prospectively collected from subsequent genetically confirmed HD participants from the BIOHD and R

Deep Learning-Based Prediction of PET Amyloid Status Using MRI.

BACKGROUND AND PURPOSE: Identifying amyloid-beta (Aβ)-positive patients is essential for Alzheimer disease clinical trials and disease-modifying treatments but currently requires PET or CSF sampling. Previous MRI-based deep learning models using only T1-weighted (T1w) images have shown moderate performance. MATERIALS AND METHODS: Multicontrast MRI- and PET-based quantitative Aβ deposition were retrospectively obtained from 3 public data sets: ADNI, OASIS3, and A4. Aβ positivity was defined using the recommended Centiloid threshold of each data set. Two EfficientNet models were trained to pred

Deep normative modelling reveals insights into early-stage Alzheimer's disease using multi-modal neuroimaging data.

BACKGROUND: Exploring the early stages of Alzheimer's disease (AD) is crucial for timely intervention to help manage symptoms and set expectations for affected individuals and their families. However, the study of the early stages of AD involves analysing heterogeneous disease cohorts which may present challenges for some modelling techniques. This heterogeneity stems from the diverse nature of AD itself, as well as the inclusion of undiagnosed or 'at-risk' AD individuals or the presence of comorbidities which differentially affect AD biomarkers within the cohort. Normative modelling is an eme

Dorsomedial Striatal Scube1 Expression Correlates With Perseveration in a Rat Reversal-Learning Task.

Background  Obsessive-compulsive disorder (OCD) is associated with reversal-learning deficits and dysregulation within cortico-striatal circuitry. Genetic studies have nominated several OCD risk genes, but their relationship to striatal gene expression and cognitive inflexibility in vivo remains unclear. Scube1 (signal peptide, CUB, EGF-like domain-containing 1) is a secreted EGF-related glycoprotein linked to vascular/inflammatory signaling; its relevance to DMS mechanisms of behavioral flexibility is unknown. This study aimed to test whether dorsomedial striatum (DMS) expression of candidate

Synergistic acupuncture and neuromodulation for chronic insomnia: a structured narrative review with systematic search and future directions.

Insomnia represents a significant global public health issue, affecting approximately 10% of adults with chronic symptoms and up to 20% with intermittent episodes. Closely associated with chronic diseases such as depression, anxiety, and cardiovascular conditions, insomnia markedly impairs patients'quality of life and imposes substantial economic burdens. Current treatments include pharmacological and non-pharmacological approaches. Pharmacotherapy primarily involves benzodiazepines, effective in short-term symptom relief but associated with long-term risks such as dependency, tolerance, and c

Exercise4Psychosis: A randomised control trial assessing the effect of moderate-to-vigorous exercise on inflammatory biomarkers and negative symptom profiles in men with first-episode psychosis.

INTRODUCTION: First-Episode Psychosis (FEP) is a devastating mental health condition that commonly emerges during early adulthood, and is characterised by a disconnect in perceptions of reality. Current evidence suggests that inflammation and perturbed immune responses are involved in the pathology of FEP and may be associated specifically with negative symptoms. Exercise training is a potent anti-inflammatory stimulus that can reduce persistent inflammation, and can improve mood profiles in general populations. Therefore, exercise may represent a novel adjunct therapy for FEP. The aim of this

Towards real-world wearable sleepiness detection: Electrodermal activity data during speech can identify sleep deprivation.

Accurate assessment of sleepiness is pivotal in managing the fatigue-associated risks stemming from sleep deprivation. Speech signals are easy to obtain, allowing detection of sleepiness anywhere. Previous machine learning (ML) studies using speech have not been successful in achieving reliable estimation of perceived sleepiness levels, which results in inaccurate sleepiness determination. In this paper, we propose that these challenges primarily stem from the inherent complexities of speech signals with inaccurate labels of sleepiness. Because the physical effects of sleepiness become pronoun

Agitation and aggression in people living with dementia and mild cognitive impairment in shared-housing arrangements - validation of a German version of the Cohen-Mansfield Agitation Inventory-Short Form (CMAI-SF).

BACKGROUND: The Cohen-Mansfield Agitation Inventory-Short Form (CMAI-SF) is a 14-item scale for assessing agitation and aggression, derived from the original 29-item CMAI, and completed by a proxy. Because the CMAI-SF has not yet been validated in German language, the aim of this study is to explore its construct validity. METHODS: Baseline data from a cluster-randomized trial to evaluate a non-pharmacological complex intervention for people living with dementia (PlwD) and mild cognitive impairment (MCI) were analyzed. The study sample consisted of 97 shared-housing arrangements (SHAs) in Ger

Decomposing MRI phenotypic heterogeneity in epilepsy: a step towards personalized classification.

In drug-resistant temporal lobe epilepsy, precise predictions of drug response, surgical outcome and cognitive dysfunction at an individual level remain challenging. A possible explanation may lie in the dominant 'one-size-fits-all' group-level analytical approaches that does not allow parsing interindividual variations along the disease spectrum. Conversely, analysing inter-patient heterogeneity is increasingly recognized as a step towards person-centred care. Here, we used unsupervised machine learning to estimate latent relations (or disease factors) from 3 T multimodal MRI features [cortic

Development and Evaluation of a Digital Health Intervention for Substance Use Reduction in Young Refugees With Problematic Use of Alcohol and/or Cannabis-Study Protocol for a Single-Armed Feasibility Trial.

Background: Refugee populations are at substantial risk of developing substance use disorder (SUD) and other mental health disorders. At the same time, refugee populations face numerous barriers to accessing mental health services. Digital interventions can address some of these issues, as emerging evidence indicates that digital interventions offer an effective, low-cost alternative with high accessibility and similar efficacy as standard SUD prevention programs. As an add-on to traditional services, they further present a viable approach to the lack of personnel available for foreign languag

Machine learning-based ability to classify psychosis and early stages of disease through parenting and attachment-related variables is associated with social cognition.

BACKGROUND: Recent views posited that negative parenting and attachment insecurity can be considered as general environmental factors of vulnerability for psychosis, specifically for individuals diagnosed with psychosis (PSY). Furthermore, evidence highlighted a tight relationship between attachment style and social cognition abilities, a key PSY behavioral phenotype. The aim of this study is to generate a machine learning algorithm based on the perceived quality of parenting and attachment style-related features to discriminate between PSY and healthy controls (HC) and to investigate its abil

A Biomarker for Alzheimer's Disease Based on Patterns of Regional Brain Atrophy.

Introduction: It has been shown that Alzheimer's disease (AD) is accompanied by marked structural brain changes that can be detected several years before clinical diagnosis via structural magnetic resonance (MR) imaging. In this study, we developed a structural MR-based biomarker for in vivo detection of AD using a supervised machine learning approach. Based on an individual's pattern of brain atrophy a continuous AD score is assigned which measures the similarity with brain atrophy patterns seen in clinical cases of AD. Methods: The underlying statistical model was trained with MR scans of pa

Evidence, theory and context: using intervention mapping in the development of a community-based self-management program for chronic low back pain in a rural African primary care setting - the good back program.

BACKGROUND: Rural Nigeria has one of the greatest burdens of low back pain but there are no effective evidence-based interventions to manage it in this population. This paper presents the application of the intervention mapping (IM) approach in the development of a complex behavior change intervention - The Good Back program, aimed at reducing non-specific chronic low back pain (CLBP) disability in rural Nigeria. METHODS: The first four steps of IM were applied. A critical review of the literature, 2 qualitative studies and a population-based cross-sectional study in rural Nigeria helped to i
