Endoscopic retrograde pancreatography (ERP) and endoscopic ultrasound (EUS) are superior to cross-sectional imaging procedures for detection of low-grade pancreatitis, but detection of duct alterations is more reliable by middle and high-grade chronic pancreatitis than by low-grade. In addition to assessment of alterations in the pancreatic duct, EUS also allows detection of parenchymatous alterations. Because of the risk of post-ERP pancreatitis, ERP has been mostly eliminated from diagnostic procedures. In contrast, endoscopic retrograde cholangiopancreatography (ERCP) allows an unrivalled a

BACKGROUND: Traumatic optic neuropathy (TON) is an important cause of severe visual loss following blunt or penetrating head trauma. Following the initial insult optic nerve swelling within the optic nerve canal or compression by bone fragments are thought to result in secondary retinal ganglion cell loss. Optic nerve decompression with steroids or surgical interventions or both have therefore been advocated to improve visual prognosis in TON.
OBJECTIVES: To examine the effects and safety of surgical interventions in the management of TON.
SEARCH METHODS: We searched CENTRAL (which contains

Thoracic spine metastases are a common manifestation of advanced malignancies and may in some cases require interdisciplinary management. Due to the complex anatomy of the thoracic region, particularly the proximity to mediastinal organs, major vessels, and the spinal cord, precise preoperative planning and surgical execution are essential. Thoracic surgical expertise is crucial for mobilising intrathoracic structures, securing vascular control, and establishing a safe anterior surgical corridor, especially in diseases involving thoracic organs and the spine. Neurosurgery focuses on decompress

Although placebo-controlled trials are considered the gold standard for evaluating the efficacy of healthcare interventions, they can be perceived to be controversial and challenging to conduct for surgical treatments. The SUcceSS trial is the first placebo-controlled trial of lumbar decompression surgery for symptomatic lumbar canal stenosis. The SUcceSS trial has experienced common issues affecting the implementation of randomised placebo-controlled surgery trials, accentuated by the COVID-19 pandemic. Using the SUcceSS trial as an example, we discuss key challenges and mitigation strategies

BACKGROUND: Multilevel artificial cervical disc replacement and anterior hybrid surgery have been introduced as reliable treatments for multilevel cervical degenerative disc disease. Surgical techniques are important for resolving patients' symptoms and maintaining the normal functioning of cervical implants. However, the use of inappropriate surgical strategies could lead to complications such as implant migration and neurological deficit. In this paper, we summarize our surgical strategies used in multilevel cervical disc replacement and hybrid surgery into five major notes.

BACKGROUND: Spinal epidural lipomatosis (SEL) is a rare disorder characterized by pathological accumulation of adipose tissue in the spinal epidural space, leading to signs and symptoms of lumbar stenosis. SEL is increasingly linked to obesity, and treatment requires surgical decompression if conservative measures fail. The role of modern pharmacological weight-loss agents in reversing SEL has not been previously described.
OBSERVATIONS: A 35-year-old man with severe, symptomatic lumbar SEL and class II obesity (BMI 37.8 kg/m) presented with progressive neurogenic claudication and functional

The primary goal of surgery with spinal meningioma is complete safe tumor removal and decompression of the spinal cord. For the surgical removal of spinal meningioma, internal debulking before dissection of the tumor capsule is essential. Intraoperative ultrasonography to localize the tumor is recommended by some authors, but we use indocyanine green (ICG) videography to visualize the localization of tumor before dural incision. ICG videography allows safe and complete delineation of intradural tumors before dural opening. This technique is quick, cost-effective, and simple to use, especially

Laminectomy and facetectomy are surgical techniques used for decompression of the cervical spinal stenosis. Recent in vitro and finite element studies have shown significant cervical spinal instability after performing these surgical techniques. However, the influence of degenerated cervical disk on the biomechanical responses of the cervical spine after these surgical techniques remains unknown. Therefore, a three-dimensional nonlinear finite element model of the human cervical spine (C2-C7) was created. Two types of disk degeneration grades were simulated. For each grade of disk degeneration

OBJECTIVE Surgical decompression is extremely effective in relieving pain and symptoms due to lumbar spinal stenosis (LSS). Decompression with interlaminar stabilization (D+ILS) is as effective as decompression with posterolateral fusion for stenosis, as shown in a major US FDA pivotal trial. This study reports a multicenter, randomized controlled trial in which D+ILS was compared with decompression alone (DA) for treatment of moderate to severe LSS. METHODS Under approved institutional ethics review, 230 patients (1:1 ratio) randomized to either DA or D+ILS (coflex, Paradigm Spine) were treat
