BACKGROUND: The sharp increase in fungal infections, insufficient diagnostic and treatment capabilities for fungal infections, poor prognosis of patients with fungal infections as well as the increasing drug resistance of fungi are serious clinical problems. It is necessary to explore the implementation and evaluation methods of antifungal stewardship (AFS) to promote the standardized use of antifungal drugs.
METHODS: The AFS programme was implemented at a tertiary first-class hospital in China using a plan-do-check-act (PDCA) quality management tool. A baseline investigation was carried out

PURPOSE: The aim of this study was to elucidate the mechanisms underlying the formation and maintenance of drug resistance in cancer cells. Previously, we demonstrated that prolonged treatment of estrogen-dependent MCF-7 breast cancer cells with exosomes derived from estrogen-resistant MCF-7/T cells leads to a partial loss of estrogen sensitivity in MCF-7 cells. Moreover, repeated transfection with one of the exosomal microRNAs-microRNA-181a-2-induced an irreversible decrease in hormonal sensitivity in the recipient cells. In the present work, to further investigate the possible mechanism of m

Antibiotics save countless lives each year and have dramatically improved human health outcomes since their introduction in the 20th century. Unfortunately, bacteria are now developing resistance to antibiotics at an alarming rate, with many new strains of "superbugs" showing simultaneous resistance to multiple classes of antibiotics. To mitigate the global burden of antimicrobial resistance, we must develop new antibiotics that are broadly effective, safe, and highly stable to enable global access. In this manuscript, we report the development of polyacrylamide-based copolymers as a class of

Antimicrobials are a type of agent widely used to prevent various microbial infections in humans and animals. Antimicrobial resistance is a major cause of clinical antimicrobial therapy failure, and it has become a major public health concern around the world. Increasing the development of multiple antimicrobials has become available for humans and animals with no appropriate guidance. As a result, inappropriate use of antimicrobials has significantly produced antimicrobial resistance. However, an increasing number of infections such as sepsis are untreatable due to this antimicrobial resistan

BACKGROUND: Antimicrobial resistance (AMR) poses a significant threat to global health, exacerbated by the protective mechanisms of biofilms formed by drug-resistant bacteria. Extracellular polymeric substances (EPS) produced by bacteria in biofilms serve as a formidable shield, impeding the efficacy of antimicrobial agents. Here, we investigated the potential of vitamin C (sodium ascorbate) to disrupt biofilm formation in drug-resistant bacteria isolated from diabetic foot ulcer (DFU) patients and studied the antimicrobial and antibiofilm activity of vitamin C on these bacteria.
RESULTS: Out

BACKGROUND: Emergence and spread of drug resistance to every anti-malarial used to date, creates an urgent need for development of sensitive, specific and field-deployable molecular tools for detection and surveillance of validated drug resistance markers. Such tools would allow early detection of mutations in resistance loci. The aim of this study was to compare common population signatures and drug resistance marker frequencies between two populations with different levels of malaria endemicity and history of anti-malarial drug use: Tanzania and Sénégal. This was accomplished by implementing

Chemotherapy remains a cornerstone of cancer treatment, yet therapeutic resistance continues to limit durable clinical responses. MicroRNAs (miRNAs) are key post-transcriptional regulators of gene expression that orchestrate diverse cellular processes implicated in drug sensitivity and resistance. However, the contribution of miRNA regulatory networks to chemotherapy response has not been systematically characterized using clinical patient cohorts. Here we present DrugMiR, an integrative multi-omics modeling framework that integrates drug response-associated miRNAs with experimentally validate

Background: Stromal remodeling in the tumor microenvironment contributes to multiple myeloma (MM) progression and drug resistance, but the extracellular mediators that drive these changes remain incompletely defined. Extracellular enolase-1 (ENO1), including membrane-associated and secreted forms, has been implicated in tumor progression; however, its role in cancer-associated fibroblast (CAF)-associated stromal reprogramming in MM is unclear. Methods: The effects of extracellular ENO1 on stromal activation and tumor-supportive functions were examined in MM using MM-bone marrow stromal cell (B

PURPOSE OF REVIEW: Germline or somatic biallelic inactivation of BRCA1, BRCA2 and PALB2 genes define a clinically and biologically distinct subset of breast cancers characterized by homologous recombination deficiency (HRD) and sensitivity to PARP inhibitors. Despite regulatory approvals of olaparib and talazoparib for breast cancers in patients harboring germline BRCA1 and BRCA2 pathogenic/likely pathogenic variants, therapeutic progress in this setting has been limited by therapeutic resistance and treatment-related toxicity. This review studied emerging therapeutic strategies aimed at exten

Chronic and recalcitrant dermatophytosis has become an increasing therapeutic challenge, particularly in India, where widespread antifungal misuse and environmental factors contribute to persistent infections. This study investigated the clinical patterns, antifungal susceptibility, and molecular mechanisms underlying terbinafine resistance in patients with tinea corporis and tinea cruris. A total of 105 clinically diagnosed and KOH-positive patients were enrolled. The majority were male (60%) with a mean age of 34 years and an average disease duration of 13 months. Most cases involved multipl

Acquisition of drug resistance, particularly to immunomodulatory drugs (IMiDs) in multiple myeloma (MM), is an important challenge in patient treatment. IMiDs are the canonical molecular glue that bind to cereblon (CRBN) and redirect its E3 ubiquitin ligase activity to neo-substrates, such as IKZF1/IFZF3. Genetic changes in the CRBN gene have been associated with IMiD resistance, including the exon 10-spliced CRBN transcripts that increase in incidence in parallel with IMiD-refractory states. Herein, we designed a new probe against CRBN splice isoform to detect exon 10 deletion and validated t

The therapeutic challenges caused by multidrug-resistant (MDR) and extensively drug-resistant (XDR) bacteria have necessitated the development of alternative treatment strategies. Phage-antibiotic synergy (PAS) has recently emerged to replace or possibly supplement antibiotics. We isolated the lytic phage NTNC80A from hospital sewage in Chandigarh, India, and it belongs to the class Caudoviricetes of viruses. We tested multiple strategies to investigate the synergistic effects of phage and antibiotic combinations against highly drug-resistant uropathogenic Escherichia coli isolates. The maximu

AIM: As a major efflux pump system in Gram-negative bacteria, AcrAB-TolC plays a key role in the transport of multiple drug substrates and is considered a potential target for the development of novel antimicrobials. Our previous study found that TolC inactivation compromised the resistance to different antimicrobials in porcine extraintestinal pathogenic Escherichia coli (ExPEC) strain PPECC042 (WT). This study was designed to investigate the functional substitution of TolC by other outer membrane proteins (OMPs) with similar β-barrel structures in pumping out different antimicrobials.

BACKGROUND: Antimicrobial resistance is continuously increasing. Adding tazobactam to ceftolozane improves the latter's activity spectrum against resistant strains. We aimed to determine the susceptibility of recently collected bacterial isolates to ceftolozane/tazobactam (C/T) and other antibiotics.
METHODS: This was a prospective cohort study conducted between March 2017 and March 2018. The in-vitro activities of C/T and 14 other antibiotics were assessed against 192 gram-negative bacterial (GNB) isolates (P. aeruginosa, K. pneumonia, E. coli, and other Enterobacterales) prospectively colle

One-third of the world's population carries Mycobacterium tuberculosis (Mtb), the infectious agent that causes tuberculosis (TB), and every 17 s someone dies of TB. After infection, Mtb can live dormant for decades in a granuloma structure arising from the host immune response, and cholesterol is important for this persistence of Mtb. Current treatments require long-duration drug regimens with many associated toxicities, which are compounded by the high doses required. We phenotypically screened 35 6-azasteroid analogues against Mtb and found that, at low micromolar concentrations, a subset of

INTRODUCTION: When used in combination, antiretroviral drugs are highly effective for suppressing HIV replication. Nevertheless, treatment failure commonly occurs and is generally associated with viral drug resistance. The choice of an alternative regimen may be guided by a drug-resistance test. However, interpretation of resistance from genotypic data poses a major challenge.
METHODS: As an alternative to current interpretation systems, we have developed artificial neural network (ANN) models to predict virological response to combination therapy from HIV genotype and other clinical informat

INTRODUCTION: Aldo-keto reductase 1C3 (AKR1C3) is a drug target for the treatment of various androgen‑dependent malignancies, including castration-resistant prostate cancer as well as hematological cancers. The enzyme plays a key role in the conversion of androgen precursors into potent androgen receptor ligands and the conversion of prostaglandins from pro-differential to pro-proliferative, thereby facilitating the progression of these malignancies. Additionally, AKR1C3 plays a role in chemotherapy resistance, reducing therapeutics to inactive forms and stabilizing the expression of mutant an

Colorectal cancer (CRC), which arises from colonic and rectal epithelial tissues, is one of the most common and life-threatening gastrointestinal malignancies worldwide, and its initiation and progression are closely associated with genetic mutations and dysregulated oncogenic signaling pathways. Despite significant advancements in conventional chemotherapy and targeted therapeutic strategies, the clinical management of CRC remains challenged by high metastatic rates, intrinsic or acquired drug resistance, and poor prognosis in patients with advanced-stage disease, underscoring the urgent need
