Biological gestational age and its calendar assessment with ultrasound. Part 1: Physiological considerations.

Biological gestational age differs from calendar age, which is only its measurement. In spite of that in many countries all over the world, the rule for termination of pregnancy exclusively because its calendar duration is longer than 293 days (so-called postterm pregnancy) is widely applied. Constant increase in the blood oxytocinase level of pregnant women, just like the sonographic measurements of fetal bodies, proves the appropriate development of pregnancy with all the more accuracy, the more similar the subsequent values of the enzymatic and ultrasonographic determinations. From medical

[Umbilical flowmetry in twin pregnancy. A method for identifying discordant fetal growth?].

Umbilical artery velocimetry was performed in 33 twin gestations to evaluate the predictivity of the test (umbilical artery S/D difference between the pair > 0.4) in the screening of discordant fetal growth. Four out of 10 cases of discordant fetal growth were identified by the test and it was abnormal in 1 out of 23 concordant twins. The sensitivity was 40%, specificity 95%, positive predictive value 80% and negative predictive value of 79%. In spite of its low sensitivity the test can be useful in detecting discordant fetal growth in twin pregnancies.

Interaction between systemic circulation and brain injuries in newborns.

In several studies different pathogenetic mechanisms of metabolic and/or cardiovascular origin were shown depending on the type of hypoxic or hypoxic/hypercapnic stress and conditions of animals (normal developed or intrauterine growth retarded newborn piglets). Thus, in normal-weight piglets the prevention of compensatory hyperventilation owing to severe hypoxia (artificially ventilated animals) leads to critical, i.e. lethal acidosis. In intrauterine growth retarded piglets, a severe hypoxia cannot be vitally compensated because of the restricted circulatory centralization. Moreover, a limit

Evaluation of the developmental toxicity of chlorinated phenols utilizing Hydra attenuata and postimplantation rat embryos in culture.

Chlorinated phenols (CPs) represent a major component of hazardous oily and wood-preserving wastes that are widely distributed in chemical dumpsites throughout the United States. Pentachlorophenol (C5P) has been reported to be highly embryolethal and embryotoxic in rats. However, data pertaining to the developmental toxicities of other important CPs are limited. In this study, the toxicities of phenol, CP homologues and their isomers, selected phenyl acetates, anisoles, sodium phenates, and tetrachlorobenzoquinones (a total of 38 chemicals) were evaluated using cultures of Hydra attenuata (HA)

[Embryofetal diseases in the tropical zone].

The treatment and the prevention of acquired fetal diseases in developing countries are difficult. Nevertheless the deleterious consequences of fever, alcohol consumption, drug intake and malaria on the embryo and the foetus can be avoided by appropriate measures. As for the prevention and the diagnostic in utero of rubella and toxoplasmosis, they may be considered lately when the current priority objectives of public health are reached. Epidemiological studies on these diseases are therefore justified.

Professional responsibility in maternity care: role of medical audit.

In 1965, Baroda Medical College initiated a process of medical audit of maternal and perinatal deaths occurring at this institution, and consultation in peripheral medical facilities providing antenatal and obstetric care. By 1984 maternal and perinatal mortality had declined and clinical judgment in maternity care had improved.

Sex difference in digit and palate formation of mouse embryos at midgestation.

We investigated whether there is a difference in timing between the sexes of mouse embryos with regard to digit and palatal formation at midgestation by using a simple method of sex chromatin analysis for rodent embryos. At day 14.4 of gestation, although the mean body weight of male embryos was greater than that of female embryos, digit and palatal formation of female embryos was found to be more advanced than in males when compared according to their body weight. This was contrary to our previous finding of digit formation at the earlier stage (day 12.0 of gestation): namely, that digit deve

Measurement of prothrombin mRNA during gestation and early neonatal development.

Prothrombin levels in the fetal and newborn lamb follow a gestational age-related pattern of development. The changes in activity are reflected by similar changes in prothrombin antigen concentration. Our investigations indicate that these changes in prothrombin antigen and activity levels are paralleled by changes in the levels of prothrombin messenger RNA present in liver samples from fetuses of various gestational ages.

Quantitative aspects of perinatal lung growth.

Weight, DNA, protein, hydroxyproline and disaturated phosphatidylcholine (DSPC) content were investigated in lungs of 97 normally formed infants over an age range from 22 to 75 postconceptional weeks, including 25 cases of hyaline membrane disease (HMD) and 13 small-for-dates infants (SFD). Lung weight and lung DNA relative to body weight were markedly lower in infants who died at 37-41 weeks than in those who died at shorter gestations or in early infancy. Total lung DSPC and DSPC concentration had a narrow peak at 36-41 weeks. The DSPC concentration per milligram of lung DNA in the first few

Mode of delivery and neonatal outcomes with early preterm severe preeclampsia: does fetal growth restriction matter?

OBJECTIVE: Severe preeclampsia diagnosed at or prior to 34 weeks is an indication for preterm delivery. Many patients with severe preeclampsia develop fetal growth restriction as a result of the placental dysfunction associated with both conditions. The ideal mode of delivery in cases of preterm severe preeclampsia with fetal growth restriction remains controversial, with providers often proceeding directly to cesarean delivery rather than attempting a trial of labor due to theoretic concerns about the harms of labor in the face of placental dysfunction. There are limited data supporting this

Decreased baseline variability on fetal heart rate pattern in a fetus with heterotaxy syndrome.

In a fetus with suspected heterotaxy syndrome, a decreased/absent baseline variability of fetal heart rate pattern developed at gestational week 36(+5) and continued for 5 days until birth at gestational week 37(+2), while repeat biophysical profile scorings with ultrasound were consistently unremarkable. This neonate weighing 2404 g with Apgar scores of 7 (1-min) and 8 (5-min) and umbilical arterial cord blood pH of 7.28 with base deficit of 3.9 mmol/L, showed a heart rate of 120 b.p.m. for 3 h after birth, but subsequently developed sinus bradycardia (84 b.p.m.) unresponsive to crying. Prenatal diagnosis of human parvovirus B19 in nonimmune hydrops fetalis by polymerase chain reaction.

OBJECTIVE: Nonimmune hydrops fetalis is a potentially lethal condition reflecting the clinical manifestation of several pathologic processes. Recently maternal infection by human parvovirus B19 has been reported to result in nonimmune fetal hydrops. We sought to develop a rapid and sensitive test to detect the presence of this agent in utero. STUDY DESIGN: Using a cloned isolate of the virus, we developed an assay based on enzymatic amplification of a segment of the human parvovirus B19 genome that allows direct detection of this agent in samples of fetal blood and amniotic fluid. RESULTS: T

Can maternal dietary components or supplements be the answer to mitigate heat-related effects to mothers and offspring in mammalian species?

Extreme heat events are rising in frequency and duration and can have a significant impact on fetal growth and pregnancy outcomes. This review provides strong evidence that links heat stress during pregnancy and/or lactation in mammals will impair the maternal gut, immune response, hormone balance, placenta function and morphology, and alter fetal growth and birth outcomes. In addition, there are also effects to lactation and postnatally, including a change in milk production and composition, and alterations to offspring's gut, immune response, muscle, and body composition, independent of diet

Placental ferroptosis and impaired fetal growth in symptomatic and asymptomatic SARS-CoV-2 infections.

SARS-CoV-2 infection disrupts iron homeostasis in organs such as the lung. Emerging evidence suggests similar dysregulation in the placenta may impair function and fetal growth. Ferroptosis, an iron-dependent form of cell death, is driven by the accumulation of labile ferrous iron, catalyzing the peroxidation of polyunsaturated fatty acids in the absence of sufficient iron storage or antioxidant defenses. This pathway has been implicated in SARS-CoV-2-related tissue damage in other organs and may contribute to placental dysfunction during pregnancy. However, the influence of infection timing,
