Healthy male volunteers received intravenous infusions of Big endothelin (ET)-1 (1-38) or Big ET-1 (22-38). Blood samples were drawn from catheters in the brachial and pulmonary arteries and the hepatic, renal, jugular and deep forearm veins. The in vivo half-lives of circulating plasma Big ET-1 (1-38) were 6.6 +/- 0.3 min for the initial phase and 23 +/- 1.4 min for the late phase. CASE: A 20-year-old pregnant female presented with an ulna fracture after a crush and bending type injury. Anatomic reduction of her ulna was not possible due to plastic deformation of the radius, requiring a corrective osteotomy of the radius. She is now 16 months out from surgery with full painless range of motion and radiographic union.

A 78-year-old woman with a history of follicular lymphoma treated with obinutuzumab and bendamustine developed Merkel cell carcinoma (MCC) on her left forearm. A child sustained an ipsilateral supracondylar humerus (SCH) and distal both-bone forearm fractures bilaterally, in addition to facial injuries, following a fall from height. CASE: A 7-year-old boy sustained a Monteggia fracture-dislocation. Corrective osteotomy of the ulna was performed to restore proper forearm anatomy and achieve radial head reduction. Recurrent radial head instability was noted postoperatively, and magnetic resonance imaging indicated recurrent dislocation of the radial head with, notably, an entrapped brachialis tendon and annular ligament.
