Revision total hip arthroplasty for aseptic failure. A review of 276 cases.

The results of 276 cemented revision total hip arthroplasties performed for aseptic failure between 1977 and 1986 have been reviewed. The mean time between revision and review was 75 months (range 30 to 144). Of 220 cases available for review, 159 were assessed clinically and radiographically, and 32 by postal questionnaire. Eighteen hips required further revision, 12 for loosening, two for sepsis, two for persistent pain, and one each for fracture and recurrent dislocation. A further six hips were radiologically and symptomatically loose. Pain was mild or absent in 83%. Over half were able to

Evaluating the Outcomes of Surgical Hip Dislocation for Femoral Head Trauma: A Systematic Review.

Femoral head fractures are complex injuries often associated with high-energy trauma and hip dislocation. Surgical hip dislocation (SHD), particularly the Ganz trochanteric flip osteotomy, was developed to provide extensive intra-articular access while preserving the femoral head's vascular supply. However, a consolidated synthesis of its outcomes is needed. This systematic review aims to evaluate the clinical and radiological outcomes, complication profiles, and key technical considerations of SHD for the management of femoral head trauma. A systematic literature search was conducted in accor

Factors associated with mechanical complications following primary total hip arthroplasty through a modified direct anterior approach using custom stems.

PURPOSE: To determine the rate and risk factors of mechanical complications (MCs) within two years following total hip arthroplasty (THA) through direct anterior approach (DAA) using custom stems, and to compare findings to other surgical approaches in the literature. METHODS: The study included a consecutive series of 479 patients (529 hips) that had undergone primary THA through the DAA using uncemented custom stems between 02/2014 and 04/2017. There were 301 men and 228 women, with a mean age of 55.9 ± 11.6, and a mean body mass index (BMI) of 25.8 ± 4.3 kg/m2. All patients were contacted

Revision total hip replacement without trochanteric osteotomy.

Revision total hip replacement has traditionally required a trochanteric osteotomy for successful cement removal and component reinsertion. In this study the authors have concluded that in most instances the revision total hip replacement procedure can be successfully performed without trochanteric osteotomy. The advantages are underscored by the high percentage of trochanteric complications with trochanteric osteotomy for revision total hip replacement and the ease of rehabilitation without trochanteric osteotomy. Also, improved functional results without trochanteric osteotomy were noted. Th

Diagnosis and conservative management of great toe pathologies: a review.

Acute great toe (Hallux) pain is a common complaint encountered by the primary care physician. Pathological conditions can vary from acute trauma to acute exacerbation of underlying chronic conditions. Delay in treatment or misdiagnosis can lead to debilitating loss of function and long-lasting pain. This review endeavors to discuss the pertinent history, physical exam findings, radiographic evidence, conservative treatment options, and surgical management for the musculoskeletal causes of acute and acute on chronic great toe pain in the adult population. The acute pathologies discussed in thi

Traumatic displacement of a maxillary primary canine tooth into the middle nasal concha presenting as chronic facial pain: a case report.

The case of a 32-year-old woman who sustained a nasal bone fracture and dental trauma at the age of 9 is described in this article. Misdiagnosis of the dental displacement into the middle turbinate at the initial examination led to chronic facial pain. The cause of the pain was incorrectly diagnosed or misinterpreted by several medical specialists, including an otolaryngologist, neurologist, physiatrist, ophthalmologist, internist, radiologist, oral surgeon, dentist, and the patient's family physician. Finally, 23 years after the dental trauma, a multislice computed tomogram revealed that the

Postmortem MSCT diagnosis of whiplash injuries in a traffic accident: a case report and review of the literature.

A 45-year-old male car driver died in a traffic accident of four cars rear-end collision on the highway. He was found to have died after a respiratory and cardiac arrest at the scene. No sign of skin injuries was observed from the external inspection. The autopsy was not permitted by the family members because of the local culture. Multislice computed tomography (MSCT) was applied to the current case, showing dislocation of C3-4 cervical vertebrae with II degree, C4 vertebral plate fractures, and spinal stenosis. Post-mortem MSCT confirmed the diagnosis as whiplash injuries. MSCT was verified

Isolated complete volar dislocation of the capitate: a case report.

Total dislocation of the capitate is an extremely rare event. We report on one such unusual case. The complete expulsion of the capitate from its physiological position is difficult to diagnose. Standard parameters of the antero-posterior and lateral radiological do not head to a definite diagnosis. In our patient, the only real diagnostic tool was the clinical assessment and the Gilula arches alteration to the standard antero-posterior projection. In the period following trauma, the patient reported a very high level of pain in the wrist. Since this could not be correlated to the lesions that

Neurologic deficits in major pelvic injuries.

Nerve injury is a common complication of major pelvic injuries. Of 73 patients examined who suffered major pelvic injuries, 24 (33%) had resultant neurologic deficits. The extent of nerve injury is proportionate to the severity of the posterior pelvic bone injury. Sacroiliac dislocation and sacral fractures may be equally complicated by nerve injury. This study suggests that nerve recovery starts three months after injury and stops after two years. Complete recovery was not recorded in severe nerve injuries. Eighty percent of patients recovered from impotence. Nerve injury was found to affect

[Monteggia injuries].

Sixty-three recent Monteggia lesions are reported, which were all treated operatively. In 52 patients, the ulna was stabilized by a compression plate. Dislocation of the radial head was reduced by a closed procedure in 24 patients and 38 patients an open operation was performed (including fractures, sutures of the annular ligament, and resections of the radial head). After 38 months, the results were better in comparison to series using conservative or medullary pinning (88% good and very good). Poor results must be expected if the operation is delayed for 2 or more weeks, if the radial head h

Management of complex vascular injuries to the extremities.

Complex vascular injuries to the extremities are challenging and, to this day, continue to cause significant morbidity and tax the ingenuity of the vascular surgeon. we have presented complicating factors and conditions that may jeopardize a successful outcome. Distal ischemia and limb loss may be caused by conditions other than arterial injury. Venous repair for major venous injury, decompression fasciotomy in compartmental hypertension, early recognition and management of associated vascular injuries in fractures and dislocations, sensible use of arteriography and venography, and prevention

Use of miniplates in the treatment of jaw fractures.

The use of miniplate-osteosynthesis in fracture treatment has been evaluated in 47 patients. In 25 of the patients plate fixation of the fracture was used without intermaxillary fixation and in 22 a supplementary IMF was used. Nine of the 47 patients had alcohol or drug addiction problems. The most common cause of fracture was assault and traffic accidents. The rate of complications including sensibility disturbances, occlusal disturbance and infection was evenly distributed among the two groups. The frequency of postoperative sensibility disturbances was however high, fifteen of the patients,

Evaluating the Safety of Prescription Sleep Medications in Patients Undergoing Total Knee Arthroplasty: A Propensity Matched Analysis.

BACKGROUND: Impaired sleep is reported in over 50% of patients after a total knee arthroplasty (TKA). Clinicians often treat postoperative sleep disturbances with medications; however, the risks associated with the use of sleep medications after TKA are not well understood. We assessed the association between sleep medication use after TKA and postoperative complications. METHODS: Using a national administrative claims database, we identified 101,806 patients undergoing primary TKA from 2011 to 2022 who were taking sleep medications within 90 days after surgery and 509,030 patients who were n

Outcomes of Hemi-Hamate Arthroplasty for Proximal Interphalangeal Joint Reconstruction.

BACKGROUND: Hemi-hamate arthroplasty (HHA) is a reconstructive option for the proximal interphalangeal joint (PIP) following fracture-dislocation injuries. This study reports outcomes following HHA, including PIP and distal interphalangeal (DIP) joint range of motion (ROM), complications, and need for revision surgery with intermediate term follow-up. METHODS: Thirty-five patients following HHA were included over a 12-year period from two fellowship-trained surgeons at a single institution. The primary outcome included postoperative PIP and DIP (ROM). The secondary outcome included complicati

Outcomes in Acetabular Revision for Acute Pelvic Discontinuity Utilizing Plating of the Posterior Column.

BACKGROUND: Acute pelvic discontinuity (PD) is a rare, yet challenging complication following total hip arthroplasty (THA). Although several techniques, such as plating of the posterior column and spanning of the discontinuity with a cup cage, have been described for the management of acute PD, few studies have determined the survivorship of acetabular constructs in this setting. The purpose of this study was to determine the outcomes of acetabular fixation of the posterior column in patients who had acute PD following THA. METHODS: We reviewed a consecutive series of 24 patients diagnosed wi

Late-Presenting Septic Arthritis of the Hip in Children: Variations in Presentation and a Review of 25 Hips After Surgical Debridement.

INTRODUCTION: The septic arthritis of the hip (SAH) is one of the most common musculoskeletal infections occurring in pediatric populations requiring urgent intervention. This study discusses the myriad of clinical and radiological presentations of late-presenting SAH in children and the outcomes of surgical management. METHODS: After ethical approval, we did retrospective reviews of children treated for late-presenting SAH (after five days of symptoms). We excluded late cases with established sequelae. We recorded age, duration of symptoms, clinical evaluation, and radiographs. We evaluated

Revision total hip arthroplasty in patients with ankylosing spondylitis: Mid-term results.

BACKGROUND: Few studies have reported on the outcome of revision total hip arthroplasty (THA) for ankylosing spondylitis (AS). Therefore, we sought to investigate the clinical and radiographic outcomes of revision THA in patients with AS. HYPOTHESIS: The mid-term outcome of revision THA in AS patients was comparable to conventional revision THA. MATERIALS AND METHODS: Forty-one patients with AS who underwent revision THA (on a total of 51 hips) between April 2004 and June 2018 were included. The mean follow-up duration was 7.5±3.6 years (3.0-15.8 years). Clinical outcomes were assessed using

The Three Step Approach to the management of acute pediatric Monteggia lesions.

BACKGROUND: The literature places the occurrence of paediatric Monteggia lesions between 1.5% and 3% of all childhood elbow injuries. There are circumstances, which may make early correct diagnosis difficult. Failure to make an early correct diagnosis may have catastrophic consequences on joint range of motion in the chronic stage. The goal of this paper is to describe our three-step approach to the treatment of acute Monteggia lesions based on the stability and radiological appearance of the fracture dislocation, to give an overview of possible pitfalls and clinical and radiological signs tha…
