Clinical outcomes in the surgical treatment of idiopathic normal pressure hydrocephalus.

Idiopathic normal pressure hydrocephalus (INPH) is a syndrome of gait disturbance, dementia and urinary incontinence. Outcomes after ventriculoperitoneal shunting for INPH are variable due to a lack of reliable, quantitative outcome data and inconsistent methods of selecting shunt candidates. The aim of this retrospective cohort study was to assess objective and quantitative clinical outcomes of ventriculoperitoneal shunting for INPH. From 2008 to 2013, consecutive patients diagnosed with INPH based on clinical and radiological criteria were included in this single-centre study. All patients r…

Effects of physical exertion on trans-tibial prosthesis users' ability to accommodate alignment perturbations.

BACKGROUND: It has long been reported that a range of prosthesis alignments is acceptable in trans-tibial prosthetics. This range was shown to be smaller when walking on uneven surfaces. It has also been argued that findings on gait with prostheses that were obtained under laboratory conditions are limited in their applicability to real-life environments. OBJECTIVES: This study investigated the hypothesis that efforts to compensate for suboptimal alignments by active users of trans-tibial prostheses become less effective when levels of physical exertion increase. STUDY DESIGN: A 2 × 2 repeat…

The efficacy of powered orthoses on walking in persons with paraplegia.

BACKGROUND: Powered orthoses are a new generation of assistive devices for people with spinal cord injury, which are designed to induce motion to paralyzed lower limb joints using external power via electric motors or pneumatic or hydraulic actuators. OBJECTIVE: Powered gait orthoses provide activated movement of lower limb joints to limit the forces applied through the upper limb joints and trunk muscles during ambulation due to the need to use an external walking aid, while simultaneously improving the kinetics and kinematics of walking in subjects with spinal cord injury. This article revi…

Effect of ankle-foot orthoses on the sagittal plane displacement of the center of mass in patients with stroke hemiplegia: a pilot study.

BACKGROUND: Ankle-foot orthoses (AFOs) have been reported to have positive effects on the temporal-spatial parameters and kinematics and kinetics of gait in patients with stroke. The center of mass (COM) may be used to represent whole body movement and energy cost in gait, and therefore COM movement would also be positively influenced with use of an appropriate AFO. OBJECTIVE: To investigate the effect of AFOs on the sagittal plane displacement of the COM in patients with stroke hemiplegia. METHODS: Five male subjects with stroke hemiplegia participated in this pilot study. The trajectory of…

Identification of subgroups among fibromyalgia patients.

This paper presents some hypotheses concerning the identification of homogeneous subgroups among fibromyalgia (FM) patients in order to improve the management of the disease. It also reviews the available literature about this subject. Three methods for subgrouping are discussed according to clinical features, biomarkers, and gait analysis. Clinical subgrouping based on cluster analysis has been used for the identification of homogeneous subgroups of patients and, more recently, homogeneous clinical features. So far, longitudinal studies using clinical subgroups to direct treatment and predict…

Treadmill exercise activates subcortical neural networks and improves walking after stroke: a randomized controlled trial.

BACKGROUND AND PURPOSE: Stroke often impairs gait thereby reducing mobility and fitness and promoting chronic disability. Gait is a complex sensorimotor function controlled by integrated cortical, subcortical, and spinal networks. The mechanisms of gait recovery after stroke are not well understood. This study examines the hypothesis that progressive task-repetitive treadmill exercise (T-EX) improves fitness and gait function in subjects with chronic hemiparetic stroke by inducing adaptations in the brain (plasticity). METHODS: A randomized controlled trial determined the effects of 6-month T…

Constrained optimization in human running.

Walking humans spontaneously select different speed, frequency and step length combinations, depending on which of these three parameters is specified. This behavior can be explained by constrained optimization of cost of transport (metabolic cost/distance) where cost of transport is seen as the main component of an underlying objective function that is minimized within the limitations of specified constraints. It is then of interest to ask whether or not such results are specific to walking only, or indicate a more general feature of locomotion control. The current study examines running gait…

Deep-Learning Based Multi-Joint Synchronous Tracking for Objective Quantification of Hindlimb Locomotor Kinematics in Rats.

Accurate and objective assessment of rat gait is essential to neuroscience and kinesiology research fields. Conventional systems frequently depend on footprint imaging for indirect gait inference, which cannot fully capture hindlimb multi-joint kinematics. This study establishes a markerless treadmill-based gait analysis system for rodents, integrating custom deep learning algorithms with a programmable weight-support treadmill to enable real-time tracking of multiple lower-limb joints and multidimensional kinematic quantification. The system automatically extracts gait cycle parameters, joint…

Does the Severity of Arm Tremor in Essential Tremor Correlate with Impaired Balance?

BACKGROUND: Motor features aside from tremor are increasingly recognized in essential tremor (ET) patients. The relationship between these features and tremor has received sparse attention. We examined whether the severity of action tremor in the arms was correlated with the severity of tandem gait difficulty and balance confidence. METHODS: 212 ET cases enrolled in a prospective clinical study, from which baseline data on the following variables were analyzed: severity of action tremor (total tremor score [TTS] from the Washington Heights-Inwood Genetic Study of Essential Tremor rating scale…

Sensorimotor function as an early marker of cognitive decline and alzheimer's biomarker burden.

Early identification of markers of cognitive decline in cognitively normal older adults is essential for dementia prevention strategies. Sensorimotor measures, such as gait speed, grip strength, and reaction time, may provide sensitive indicators of current and future impairment. This study examined associations between baseline sensorimotor function and cognitive decline in cognitively normal older adults. In this prospective cohort, 246 cognitively normal older adults from the DRIVES Project completed baseline assessments of grip strength, gait speed, simple reaction time, and the Preclinica…

Effects of ankle exoskeleton assistance and plantar pressure biofeedback on incline walking mechanics and muscle activity in cerebral palsy.

Ankle dysfunction affects more than 50 % of people with cerebral palsy, resulting in atypical gait patterns that impede lifelong mobility. Incline walking requires increased lower limb effort and is a promising intervention that targets lower-limb extensor muscles. A concern when prescribing incline walking to people with gait deficits is that this exercise may be too challenging or reinforce unfavorable gait patterns. This study aims to investigate how ankle exoskeleton assistance and plantar pressure biofeedback would affect gait mechanics and muscle activity during incline walking in CP. We…

A Wearable Haptic Feedback System for Arm-Swing Amplitude Modulation During Overground Walking in Older Adults.

Reduced arm swing frequently occurs in older adults and is associated with declined gait performance. Experimental studies have demonstrated that restricting arm swing decreases stride length and walking speed, whereas deliberately increasing arm swing can improve these gait parameters. This study evaluated whether a wearable haptic feedback system could effectively increase arm-swing amplitude and assess its effects on spatiotemporal gait outcomes during overground walking. Using a within-subject repeated-measures design, twelve community-dwelling older adults (6 males/6 females; 75.8±6.5 yea…
