[Normalizing effect of the pineal gland peptides on the daily melatonin rhythm in old monkeys and elderly people].

In the course of aging both monkeys and people reveal decreased night and average daily level of melatonin in the blood plasma and reduced hormone circadian rhythm amplitude, which evidence the disorder of the pineal gland melatonin releasing function. Peptide preparations of the pineal gland (Epithalamin--a complex of peptides isolated from the pineal gland and Epitalon--synthetic tetrapeptide) recover night release of endogenous melatonin and lead to the normalization of the hormone circadian rhythm in the blood plasma. In elderly people Epithalamin and Epitalon modulate pineal gland functio

Ethacrynic acid can be effective for refractory congestive heart failure and ascites.

Ethacrynic acid is a loop diuretic little used today because of its side-effect profile and the availability of multiple alternative agents. However, in our clinical experience, ethacrynic acid can alleviate acute congestive heart failure and ascites resistant to other diuretics. Two patients aged 89 and 94 in life-threatening pulmonary edema were stabilized by ethacrynic acid after furosemide proved ineffective. A third patient, aged 83, with a pleural effusion and ascites secondary to end-stage hepatitis B and C, responded to ethacrynic acid when spironolactone and furosemide produced little

The effects of severity of hearing impairment and locus of control on the denial of hearing impairment in the aged.

This is a pilot study exploring the effects of locus of control and severity of hearing impairment on the amount of denial manifested by older people with hearing losses. Forty-one participants from a geriatric clinic were given questionnaires to determine their locus of control on a I-E scale. In addition, they were given a questionnaire that measured self-reported hearing losses. A pure tone audiometric test was given each participant in order to ascertain actual hearing thresholds. This study predicted that the less severe the impairment, the greater the denial that would be manifested. Als

Histopathological study of intestinal metaplasia of postmortem stomachs in the aged Japanese.

The incidence and extent of intestinal metaplasia of the stomach in aged Japanese has been studied. Histological studies were carried out on 422 stomachs, obtained at autopsy in Tokyo Geriatric Hospital from 1957 to 1969. The average age of the subjects in this series was 76 yr. Intestinal metaplasia was classified into six types according to its distribution: antral type 14.5%, intermediate zone type 1.7%, cardiac type 4.0%, cardiac and antral type 3.8%, diffuse type 62.8%, and nonmetaplastic type 13.3%. Diffuse type was found to gradually increase in incidence with increasing age and a corre

Looks and old age.

In geriatric examinations, the old patients' looks should be observed. Based on the observation of 100 old subjects (aged 65 to 92), the author classifies the old people into 4 categories of looks. 1. Cheerful-eyed 2. Sad-eyed 3. People with fixed (lifeless) gaze 4. Wicked (aggressive) looking people. Category I can be treated easily; category 2 will co-operate in anti-depression therapy; category 3 exhibits mainly Parkinson's disease, in need of anti-sclerotic treatment. In the case of people with fixed gaze, the side-effects of drugs should also be suspected. Subjects in category 4 are diffi

Biofeedback training to overcome poststroke foot-drop.

The technique has exciting potential for elderly hemiplegic patients, even those who are confined to a nursing home. There are limiting factors, of course; the method is time-consuming and the initial outlay for equipment is high. But the rewards can be well worth the time, effort, and cost involved. The recovery potential of some geriatric patients after a cerebrovascular accident may seem bleak because of multiple disabilities, e.g., paralysis, delayed reflexes, aphasia. Fortunately, these disorders do not necessarily decree failure of biofeedback training. Neither does advanced age. The str

[The Unique Challenges of Treating and Caring for Older Adult Patients With Cancer].

In older age, tissue degeneration, decreased cellular competition, and the declining efficiency of clearance mechanisms lead to higher rates of survival and accumulation for cells with adaptive mutations, which increase the risk of cancer cell development (Laconi et al., 2020). Thus, age is a significant risk factor for cancer. It is projected that by 2050, the global population aged 60 and above will rise to 2.1 billion, with those aged 80 and above expected to reach 426 million (World Health Organization, 2024). Taiwan, which became an aging society in 2018, is expected to be a nation in whi

Small bowel obstruction and frailty: a systematic review.

PURPOSE: To explore frailty within the context of small bowel obstruction (SBO) and identify the frailty screening tools used. Surgical decision-making in small bowel obstruction among older, frail adults poses distinct challenges for emergency surgical teams, often resulting in variation of care. Conservative management is frequently successful, yet the diminished physiological and functional reserve characteristic of frailty necessitates tailored treatment guided by comprehensive geriatric assessment (CGA). METHODS: Following PRISMA principles, a systematic literature search was conducted a

Self-reported frailty: good enough for perioperative practice?

Patient frailty is known to correlate with poor outcomes after surgery, however, routinely measuring frailty has proved an ongoing hurdle in perioperative practice. Self-reported patient frailty could represent a novel approach to the issue. Alkadari and colleagues report a largely successful experience with self-reported frailty in 531 older adults undergoing noncardiac surgery, although some challenges remain.

[The nervous system and communication: polyneuropathy and the role of the peripheral nervous system].

The central nervous system (CNS) processes information, coordinates movements, and enables communication-both within the body and with the environment: Perception and response to sensory impressions are controlled by the brain, with afferent and efferent pathways passing through the spinal cord. The peripheral nervous system (PNS) mediates between the environment and the CNS. As we age, the PNS undergoes changes, resulting in a weakening of muscle reflexes and proprioception. This must be distinguished from polyneuropathies (PNP), which are pathological changes in the PNS. The causes are manif

Timed Up-and-Go as a predictor of fracture risk: a systematic review and meta-analysis in a population of over 1.6 million people.

OBJECTIVES: This systematic review aimed to synthesize evidence from prospective cohort studies assessing the Timed Up-and-Go (TUG) test as association of fracture risk in older people, and to quantify the association through meta-analysis using hazard ratios (HR). METHODS: Two authors independently conducted systematic searches in PubMed/MEDLINE, Scopus, Web of Science, CINAHL Complete, and the Psychology and Behavioral Sciences Collection. MEDLINE was accessed through PubMed and was therefore not treated as a separate database. The Psychology and Behavioral Sciences Collection was included

Implementing Systematic Assessment of Physical Function in Hospitalised Medical Patients: Description and Evaluation Guided By the Quality Implementation Framework.

RATIONALE: Although older adults comprise 16%-22% of the population, they account for up to 75% of hospital admissions and nearly half of related costs. Frailty, multimorbidity and reduced physical function are common, requiring assessment and follow-up. Systematic assessment of physical function using the Cumulated Ambulation Score (CAS), 30-second sit-to-stand test (30s-STS), and handgrip strength (HGS) has shown predictive value for outcomes such as length of hospital stay, complications, readmission and survival in hospitalised older patients. This underscores the importance of early inter

Assessment of frailty in patients with heart failure: A new Heart Failure Frailty Score developed by Delphi consensus.

AIMS: The Heart Failure Frailty Score (HFFS) is a novel, multidimensional tool to assess frailty in patients with heart failure (HF). It has been developed to overcome limitations of existing frailty assessment tools while being practical for clinical use. The HFFS reflects the concept of frailty as a multidimensional, dynamic and potentially reversible state, which increases vulnerability to stressors and risk of poor outcomes in patients with HF. METHODS AND RESULTS: The HFFS was developed through a Delphi consensus process involving 54 international experts. This approach involved iterativ

Assessment of oral hypofunction and its association with age among Korean community-dwelling older adults.

BACKGROUND: Due to the increasing proportion of older adults in Korea and growing interest in aging, the concepts of oral aging and oral hypofunction have recently been introduced. Thus, it is necessary to investigate the age-specific oral function levels of Korean older adults and develop expert intervention methods for healthy aging. METHODS: Dysphagia, independence of daily living, and oral hypofunction were assessed in 206 older adults living in Wonju, Gangwon State, South Korea. Subjective dysphagia was assessed through self-report questionnaires using the Dysphagia Handicap Index (DHI),

Vigor to Frailty As a Continuum-A New Approach in the Study of Muscle, Mobility, and Aging Cohort.

BACKGROUND: Frailty can occur in older adults without disability or multimorbidity. Current methods focus on the most frail, but poorly discriminate among those "not frail." METHODS: The Study of Muscle, Mobility, and Aging (SOMMA) included 879 adults aged 70 years and older without mobility disability. We operationalized frailty domains using: peak oxygen consumption (endurance), digit symbol substitution test (speed), leg power (strength), perceived fatigability, D3 creatine dilution (sarcopenia), and accelerometry (sedentary behavior) to construct a frailty score of 0-12 summing tertiles (

Social Connectedness Resource Preferences of Older Adults in Assisted Living: A Scoping Review Based on the WHO-ICF Framework.

The current scoping review identified emerging evidence on social connectedness resource preferences of older adults in assisted living facilities (ALFs) and the community. A literature search was performed using several databases. We included review articles published between January 2000 and September 2022 in English and related to social connectedness resources in ALFs and the community. Of 134 titles and abstracts, eight studies were included. Study participants comprised 2,482 older adults from 233 ALFs in the United States. Themes were framed using the World Health Organization's Interna

Dysphagia is closely related to frailty in mild-to-moderate Alzheimer's disease.

INTRODUCTION: Physical phenotype and the cumulative deficit model are two well-known concepts of frailty. One of the main components of frailty is loss of muscle mass and function, which may also include swallowing muscles, therefore is a risk factor for dysphagia. Since dysphagia is seen starting from the early stages of Alzheimer's Disease (AD), in this study we aimed to reveal the relationship between frailty and dysphagia and dysphagia-related quality of life through Swallow Quality of Life (SwalQoL) tool in patients with AD and compare them with cognitively intact older adults. METHODS:
