Early versus late ureteric stent removal after kidney transplantation.

BACKGROUND: Kidney transplantation is the treatment of choice for patients with end-stage kidney disease. In a previous review we concluded that the routine use of ureteric stents in kidney transplantation reduces the incidence of major urological complications (MUC). Unfortunately, this reduction appears to lead to a concomitant rise in urinary tract infections (UTI). For kidney recipients UTI is now the commonest post-transplant complication. This represents a considerable risk to the immunosuppressed transplant recipient, particularly in the era of increased immunologically challenging tran

Antibiotic prophylaxis versus no prophylaxis for preventing infection after cesarean section.

BACKGROUND: The single most important risk factor for postpartum maternal infection is cesarean section. Although guidelines endorse the use of prophylactic antibiotics for women undergoing cesarean section, there is not uniform implementation of this recommendation. This is an update of a Cochrane review first published in 1995 and last updated in 2010. OBJECTIVES: To assess the effects of prophylactic antibiotics compared with no prophylactic antibiotics on infectious complications in women undergoing cesarean section. SEARCH METHODS: We searched the Cochrane Pregnancy and Childbirth Group

Comparative efficacy of lithium carbonate loading dose versus standard dosing, each combined with quetiapine, in acute mania of bipolar I disorder: a randomized single-blind controlled trial.

INTRODUCTION: Bipolar disorder type I is defined by the occurrence of at least one manic episode, presenting with elevated mood, hyperactivity, and restlessness. Effective management relies primarily on pharmacotherapy, with lithium carbonate and quetiapine proven beneficial as monotherapies. Given the chronic nature of the illness and alternating manic–depressive phases, acute mania requires rapid symptom control to prevent functional decline and harm. Although lithium is a cornerstone mood stabilizer, its clinical effect may take days to emerge. Quetiapine, an atypical antipsychotic, provide

C-reactive protein point-of-care testing for safely reducing antibiotics for acute exacerbations of chronic obstructive pulmonary disease: the PACE RCT.

BACKGROUND: Most patients presenting with acute exacerbations of chronic obstructive pulmonary disease (AECOPD) in primary care are prescribed antibiotics, but these may not be beneficial, and they can cause side effects and increase the risk of subsequent resistant infections. Point-of-care tests (POCTs) could safely reduce inappropriate antibiotic prescribing and antimicrobial resistance. OBJECTIVE: To determine whether or not the use of a C-reactive protein (CRP) POCT to guide prescribing decisions for AECOPD reduces antibiotic consumption without having a negative impact on chronic obstru

Peginterferon alpha-2a versus peginterferon alpha-2b for chronic hepatitis C.

BACKGROUND: A combination of weekly pegylated interferon (peginterferon) alpha and daily ribavirin still represents standard treatment of chronic hepatitis C infection in the majority of patients. However, it is not established which of the two licensed peginterferon products, peginterferon alpha-2a or peginterferon alpha-2b, is the most effective and has a better safety profile. OBJECTIVES: To systematically evaluate the benefits and harms of peginterferon alpha-2a versus peginterferon alpha-2b in head-to-head randomised clinical trials in patients with chronic hepatitis C. Pharmacological interventions for acute attacks of vestibular migraine.

BACKGROUND: Vestibular migraine is a form of migraine where one of the main features is recurrent attacks of vertigo. These episodes are often associated with other features of migraine, including headache and sensitivity to light or sound. The unpredictable and severe attacks of vertigo can lead to a considerable reduction in quality of life. The condition is estimated to affect just under 1% of the population, although many people remain undiagnosed. A number of pharmacological interventions have been used, or proposed to be used, at the time of a vestibular migraine attack to help reduce th

Multi-professional simulation training on prolonged labor in Tanzania: A quasi-experimental, pre-post interventional study.

INTRODUCTION: In many middle-income countries, cesarean section rates have increased substantially and reached levels of more than 50% in urban regions. Prolonged labor and non-reassuring fetal heart rate are the most common indications for intrapartum cesarean section. This intervention can be a life-saving procedure; however, unnecessary cesarean sections can be harmful for the mother and the baby. Reducing the cesarean section rates is highly warranted. We aimed to investigate whether intrapartum cesarean section rates could be reduced by multiprofessional simulation training, and whether t

Autophagy as a therapeutic linchpin in metabolic diseases and obesity-associated diabetes.

Autophagy, a conserved lysosomal degradation pathway, is increasingly recognized as a central regulator of metabolic health. Its impairment contributes directly to obesity and type 2 diabetes by disrupting nutrient sensing, stress adaptation, and organelle quality control. Hyperactivation of MTORC1 with insufficient AMPK and SIRT1 signaling suppresses autophagic flux, driving lipid accumulation, insulin resistance, and mitochondrial dysfunction. Clinically relevant consequences include adipose inflammation and hypertrophy, hepatic steatosis with impaired β-oxidation, pancreatic β-cell failure

Postbiotic Nagqu4580 Attenuates Ulcerative Colitis and Suppresses Ferroptosis in Association with the Microbiota-Tryptophan-AhR/Nrf2 Axis.

Background/Objectives: Ferroptosis, an iron-dependent cell death driven by lipid peroxidation, is implicated in the pathogenesis of ulcerative colitis (UC). Tryptophan metabolism and its interaction with the aryl hydrocarbon receptor (AhR) and nuclear factor erythroid 2-related factor 2 (Nrf2) axis represent a crucial regulatory network in intestinal homeostasis. This study aimed to investigate whether the probiotic fermentation product postbiotic Nagqu4580 alleviates UC by modulating this network to inhibit intestinal epithelial ferroptosis. Methods: An acute UC model was induced in mice usin

Impact of intercostal nerve cryoablation on opioid use in pleural mesothelioma surgery: results from a high-volume integrated regional center.

BACKGROUND: Pleural mesothelioma is a rare and aggressive malignancy treated with a multimodal approach that includes surgery. Unfortunately, pleurectomy and decortication surgery is highly morbid and is associated with significant postoperative pain. To reduce harmful side effects associated with opioid use, intercostal nerve cryoablation has emerged as a potential adjunct for thoracic pain control. However, its role among patients with pleural mesothelioma has not been established. We aimed to evaluate whether intercostal nerve cryoablation reduces inpatient opioid use among patients undergo

Effects of yeast β-glucan on fermentation parameters, microbial community structure, and rumen epithelial cell function in high-concentrate-induced yak rumen acidosis in vitro.

This study employed a completely randomized design (CRD) to evaluate the effects of graded levels of yeast β-glucan supplementation on subacute ruminal acidosis (SARA) induced by high-concentrate diets in yaks. Seven treatment groups were established: a low-concentrate control (LC; 50 % concentrate + 50 % forage), a high-concentrate control (HC; 80 % concentrate + 20 % forage), and HC diets supplemented with 0.5 %, 1 %, 2 %, 4 %, or 8 % yeast β-glucan. Rumen pH, gas production, and volatile fatty acid (VFA) profiles were measured at 3, 6, 9, 12, and 24 h of in vitro fermentation. At 9, 12, and

Green synthesis of silver nanoparticles using Padina commersonii: Characterization, hypoglycemic effects, and antimicrobial potential against human pathogenic bacteria.

BACKGROUND: Nanotechnology is a dynamic field comprising chemistry, physics, biology, and engineering, with a high proficiency in disease treatment delivery methods. Metallic precursors (e.g.,AgNO3) are widely used to synthesize NPs. But, owing to their harmful nature and the cost of these metallic nanoparticles, the focus is shifting toward "green synthesis" of nanoparticles using fungi, bacteria, plants, and algae. Hence, we used Padina commersonii to green synthesize NPs. We focused on the antioxidant, hypoglycemic, and antimicrobial efficacy of biologically synthesized AgNPs from P. commer
