Therapeutic Failure or Success Risk Associated With the Dynamic of Immunological Parameters in People Living With HIV-1 on Antiretroviral Treatment in Benin.

Until now, human immunodeficiency virus type 1 infection monitoring is based on plasma viral load and cluster of differentiation (CD)4-positive cells from thymus (CD4+ T cells) count. However, it is increasingly accepted that this biological monitoring should be strengthened with other markers that would reinforce the management. Therefore, we addressed this task by assessing the risk associated with therapeutic failure/success linked to the dynamic of immunological parameters in people living with HIV-1 (PLHIV-1) and on antiretroviral therapy (ART) in order to identify their prognostic values

Independent association of metabolic tumor response on FDG-PET with pulmonary toxicity following risk-adaptive chemoradiation for unresectable non-small cell lung cancer: Inherent radiosensitivity or immune response?

BACKGROUND: In the context of a phase II trial of risk-adaptive chemoradiation, we evaluated whether tumor metabolic response could serve as a correlate of treatment sensitivity and toxicity. METHODS: Forty-five patients with AJCCv7 stage IIB-IIIB NSCLC enrolled on the FLARE-RT phase II trial (NCT02773238). [18F]fluorodeoxyglucose (FDG) PET-CT images were acquired prior to treatment and after 24 Gy during week 3. Patients with unfavorable on-treatment tumor response received concomitant boosts to 74 Gy total over 30 fractions rather than standard 60 Gy. Metabolic tumor volume and mean standar

Building a model for the differential diagnosis of non-tuberculous mycobacterial lung disease and pulmonary tuberculosis: A case-control study based on immunological and radiological features.

BACKGROUND: Although the incidence of non-tuberculous mycobacterial pulmonary disease (NTM-PD) is increasing annually, it is easily misdiagnosed as pulmonary tuberculosis (PTB). This study aimed to screen and identify the immunological and radiological characteristics that differentiate NTM-PD from PTB and to construct a discriminatory diagnostic model for NTM-PD, providing new tools for its differential diagnosis. METHODS: Hospitalised patients diagnosed with NTM-PD or PTB between January 2019 and June 2023 were included in the study. Immunological and radiological characteristics were compa

Approach and Management of Hypertension After Kidney Transplantation.

Hypertension is one of the most common cardiovascular co-morbidities after successful kidney transplantation. It commonly occurs in patients with other metabolic diseases, such as diabetes mellitus, hyperlipidemia, and obesity. The pathogenesis of post-transplant hypertension is complex and is a result of the interplay between immunological and non-immunological factors. Post-transplant hypertension can be divided into immediate, early, and late post-transplant periods. This classification can help clinicians determine the etiology and provide the appropriate management for these complex patie

The Wonder Years: What Can Primary School Children Teach Us About Immunity to Mycobacterium tuberculosis?

In high burden settings, the risk of infection with Mycobacterium tuberculosis increases throughout childhood due to cumulative exposure. However, the risk of progressing from tuberculosis (TB) infection to disease varies by age. Young children (<5 years) have high risk of disease progression following infection. The risk falls in primary school children (5 to <10 years), but rises again during puberty. TB disease phenotype also varies by age: generally, young children have intrathoracic lymph node disease or disseminated disease, while adolescents (10 to <20 years) have adult-type pulmonary d

Dynamics of Tissue-Specific CD8+ T Cell Responses during West Nile Virus Infection.

The mouse model of West Nile virus (WNV), which is a leading cause of mosquito-borne encephalitis worldwide, has provided fundamental insights into the host and viral factors that regulate viral pathogenesis and infection outcome. In particular, CD8+ T cells are critical for controlling WNV replication and promoting protection against infection. Here, we present the characterization of a T cell receptor (TCR)-transgenic mouse with specificity for the immunodominant epitope in the WNV NS4B protein (here referred to as transgenic WNV-I mice). Using an adoptive-transfer model, we found that WNV-I

Role of Shigella infection in endometriosis: a novel hypothesis.

Endometriosis is the presence of endometrial cells and stroma at ectopic sites outside the uterine cavity. The natural history of endometriosis is uncertain, its etiology unknown, the clinical presentation inconsistent, diagnosis difficult and the treatment poorly standardized. It causes significant morbidity due to pelvic pain and infertility among 15-25% of women during their reproductive age. The benign disease causes peritoneal inflammation, fibrosis, adhesions and ovarian cysts but displays features of malignancy, like neo-vascularization, local invasion and distant metastasis. Mechanical

Application of small interfering RNAs and drug combination for pancreatic cancer therapy.

In recent years, pancreatic cancer has become one of the major causes of death among cancers. It has been confirmed that changes in the expression of various genes are important factors associated with the occurrence and development of pancreatic cancer. Small interfering RNAs (siRNAs) are 21-25nt ribonucleotides that target specific DNA sequences and reduce their expression levels. An siRNA delivery system is required as a common and effective method for cancer treatment. Silencing the expression of critical genes (K-Ras, GJB4, SIX4, RARγ, etc) via siRNAs can effectively block the development

Mast Cells Selectively Deliver Extracellular Vesicle-Encapsulated mRNA to Colorectal Cancer Cells.

Mast cells (MCs), a type of granulocytic immune cell, exert contrasting effects on tumorigenesis. The anti- or pro-tumorigenic activity of MCs depends on the cancer type, tumor microenvironment, and MC localization within the tumor. Consequently, their role remains controversial and poorly understood across multiple cancer types, including colorectal cancer (CRC). Most proposed mechanisms underlying MC activity in CRC have focused on MC secretion of biological factors. In this study, we demonstrated that MCs transfer extracellular vesicles containing mRNAs and proteins to CRC cells. This proce

Gut-Brain Inflammation and Disrupted Homeostasis Due to Activation of Mast Cells and Microglia.

Recent data from the Centers for Disease Control (CDC) indicate that the incidence of Autism Spectrum Disorder (ASD), a neurodevelopmental disorder characterized by deficits in social communication and the presence of restricted interests and repetitive behaviors, has increased to 1 in 31 children. Individuals with ASD have a constellation of neurological, behavioral, sensory, feeding, gastrointestinal, and immunological issues. Even though there is some genetic component to the pathogenesis of ASD, accumulation of environmental and pathogenic toxins could contribute to disruption of the gut-b

Inflammation drives TGFβ1 activation via the αvβ6 integrin-mechanotransduction pathway in human skin.

Transforming growth factor β (TGFβ) is a critical regulator of skin homeostasis and inflammation, including psoriasis. Multiple mechanisms can regulate TGFβ signaling, such as the activation of latent TGFβ1 through integrin-dependent pathways. We investigated the molecular mechanisms of TGFβ1 activation during cutaneous inflammation using reconstructed human epidermis treated with inflammatory cytokine cocktail. We report αvβ6 integrin upregulation and enhanced binding of TGFβ1 latency-associated peptide (LAP), which, however, did not result in active TGFβ1 release, restricting the TGFβ1 activ

The emerging cellular metal ions-immune axis in cancer.

Cancer immunotherapy is frequently hindered by the immunosuppressive tumor microenvironment. Although metal ions have been viewed as essential nutrients or cytotoxic payloads, emerging evidence reveals that they are potent immune modulators that orchestrate the cancer-immunity cycle. Here, we synthesize how ionic competition dictates immune cell plasticity and metabolic fate. We detail how metal ions regulate cell death pathways and immune signaling cascades, including the cyclic GMP-AMP synthase-stimulator of interferon genes and nuclear factor kappa B pathways, as well as immunological synap

The Immunopathology of Preeclampsia.

Preeclampsia (PE) is a hypertensive disorder of pregnancy characterized by target organ damage, affecting approximately 5% of pregnancies. Complex neuroendocrine alterations, vascular imbalances, excessive oxidative stress, environmental factors, and inappropriate immune responses drive the pathology of this condition. Inadequate remodeling of the uterine spiral arteries serves as a fundamental marker of the disease. PE is heavily mediated by an inflammatory cascade involving complement system activation, decreased tolerogenicity of natural killer (NK) cells, M1 macrophage polarization, and de

Thirty years in, regulatory T cells are ready for medicine.

Regulatory T (Treg) cells are a specialized subset of CD4+ T cells indispensable for the establishment and maintenance of immunological tolerance. Treg cells employ diverse mechanisms of immune regulation mediated by a broad repertoire of immunosuppressive molecules under the control of the lineage-specifying transcription factor FoxP3. Functional defects in Treg cells disrupt immune homeostasis and are implicated in the pathogenesis of autoimmune diseases, chronic inflammatory disorders, and cancer. In this article, we revisit three decades of progress since Sakaguchi's seminal discovery of C

[Type 1 diabetes, towards new horizons].

Type 1 diabetes (T1D) is an autoimmune disease characterized by the destruction of pancreatic β-cells, leading to lifelong insulin dependence. Although significant advances have been made in insulin therapy and glucose monitoring, these approaches are implemented at a late stage of the disease, when β-cell destruction is already well underway. Recent advances in the understanding of its pathophysiology have highlighted the complexity of interactions between genetic, environmental, and immunological factors, as well as the existence of distinct endotypes that may influence disease progression.

The host immune response to Mycobacterium tuberculosis determining protection or disease progression.

Tuberculosis (TB) remains a global health issue and leading cause of death. Understanding the immune response to Mycobacterium tuberculosis infection is critical to advance TB control. Although immune factors involved in protection against TB have long been identified, these cannot predict the efficacy of TB vaccines, nor which individuals will control the infection or progress to active TB disease. This is especially challenging given the complexity of M. tuberculosis infection states and the breadth of TB disease severities, which is increasingly apparent from basic, clinical and translation
