A Simple Method for Positioning the Traction Table during Fixation Surgery for a Displaced Femoral Trochanteric Fracture in a Patient Following Ipsilateral Above-the-knee Amputation: A Case Report.

INTRODUCTION: It is difficult to use a traction table during surgery for an ipsilateral displaced femoral trochanteric fracture following above-the-knee amputation. However, there are few reports regarding such cases. We describe the simple method we used for positioning the traction table and reducing fracture site during fixation surgery for a displaced femoral trochanteric fracture in this patient following above-the-knee amputation. CASE REPORT: An 80-year-old man was injured in a head-on collision with an oncoming vehicle. We diagnosed him with traumatic gastric perforation, multiple rig

Electrophysiological Guidance of Epidural Electrode Array Implantation over the Human Lumbosacral Spinal Cord to Enable Motor Function after Chronic Paralysis.

Epidural electrical stimulation (EES) of the spinal cord has been shown to restore function after spinal cord injury (SCI). Characterization of EES-evoked motor responses has provided a basic understanding of spinal sensorimotor network activity related to EES-enabled motor activity of the lower extremities. However, the use of EES-evoked motor responses to guide EES system implantation over the spinal cord and their relation to post-operative EES-enabled function in humans with chronic paralysis attributed to SCI has yet to be described. Herein, we describe the surgical and intraoperative ele

Prospective Randomized Trial Comparing Femoral Nerve Block With Intraoperative Local Anesthetic Injection of Liposomal Bupivacaine in Total Knee Arthroplasty.

BACKGROUND: Postoperative pain after total knee arthroplasty (TKA) may impact long-term results and incidence of complications. Femoral nerve block (FNB) provides excellent pain relief after TKA, although associated risks include weakness, delayed participation in therapy, and nerve injury. Liposomal bupivacaine (LB) is a potentially longer acting local anesthetic that may reduce postoperative pain. METHODS: We performed a prospective, randomized, double-blind study of 373 TKA patients randomized to receive either an FNB (control group), or an intraoperative periarticular injection (PAI) with

Initiation and modulation of locomotor circuitry output with multisite transcutaneous electrical stimulation of the spinal cord in noninjured humans.

The mammalian lumbar spinal cord has the capability to generate locomotor activity in the absence of input from the brain. Previously, we reported that transcutaneous electrical stimulation of the spinal cord at vertebral level T11 can activate the locomotor circuitry in noninjured subjects when their legs are placed in a gravity-neutral position (Gorodnichev RM, Pivovarova EA, Pukhov A, Moiseev SA, Savokhin AA, Moshonkina TR, Shcherbakova NA, Kilimnik VA, Selionov VA, Kozlovskaia IB, Edgerton VR, Gerasimenko IU. Sensorimotor function as a predictor of chronic ankle instability.

BACKGROUND: Recurrent ankle injury occurs in 70% of individuals experiencing a lateral ankle sprain. The cause of this high level of recurrence is currently unknown. Researchers have begun to investigate sensorimotor deficits as one possible cause with inconclusive and often conflicting results. The purpose of this study was to further the understanding of the role of sensorimotor deficits in the chronically unstable ankle by establishing which specific measures best distinguish between chronically unstable and healthy ankles. METHODS: Twenty-two participants with chronic ankle instability an

[Endoscopic gastrocnemius recession as therapy for gastrocnemius equinus].

AIM: Endoscopic gastrocnemius recession is a new technique to treat gastrocnemius equinus. Smaller incisions and the ability to perform the technique in a supine position are purported advantages. This study was designed to evaluate the results and possible complications of this new technique. METHODS: 47 patients undergoing 54 endoscopic gastrocnemius recessions were followed in a prospective study. Pre-operative criteria were a lack of ankle dorsiflexion with the knee extended to create a 90 degrees relationship of the foot to the leg in symptomatic patients. Pre- and postoperative ankle do

Proximal origin variants of the plantaris muscle: a classification-based review with surgical and radiologic implications.

BACKGROUND: The plantaris muscle (PM) is a highly variable structure whose proximal origin demonstrates significant morphological diversity. While traditionally considered clinically insignificant, recent evidence highlights its role in sports injuries, Achilles tendinopathy, and even vascular entrapment syndromes. A structured classification-based approach is therefore essential to improve diagnostic accuracy and surgical safety. METHODS: A classification-focused narrative review of anatomical, radiological and clinical literature was performed using PubMed and Web of Science, supplemented by

Forensic Analysis of Decapitation in an Agricultural Worker: A Case Report on Farm Tractor-Related Fatality.

The agricultural sector relies heavily on farm workers who operate tractors and other associated heavy machinery, mostly attached to the vehicle itself. With that in mind, they are at high risk of occupational and road traffic-related fatalities. Decapitation resulting from these accidents is an exceptionally rare outcome. A 61-year-old male was found decapitated on a farm field he was working at, next to a running farm tractor. The body was delivered with a decapitated head, of which the spinal cord was striking through. Also, the left earlobe was completely amputated. On the upper part of th

Comparing the effect of short foot exercise and toe curl exercise on plantar pressure during single-leg standing in individuals with flatfoot: A randomized controlled trial.

BACKGROUND: Controlling flatfoot biomechanics is crucial in preventing lower limb musculoskeletal injuries. Short foot exercise (SFE) and toe curl exercise (TCE) are interventions for flatfoot; however, their effects on foot biomechanics remain inconsistent. We aimed to compare the impact of SFE and TCE on plantar pressure during single-leg standing (SLS) in individuals with flatfoot. METHODS: This randomized controlled trial recruited 34 participants with flatfoot (Foot Posture Index-6 > 7). Participants were randomly assigned to SFE or TCE groups and completed 8 weeks of exercise. Plantar p
