Trends in the incidence of Non-Hodgkin's lymphoma in India.

Non-Hodgkin's lymphoma is the 11th most common cancer in terms of incidence. It is most frequent in high income countries, with rates more than twice those of middle--to low-income countries. It is usually fatal, with a 5 year survival rate of less than 35 percent. It is not a single cancer, but rather a wide group of cancers (including entities such as Burkitt's lymphoma and diffuse large B-cell lymphoma), each with a district geographical distribution, development path, age profile and prognosis. Non-Hodgkin's lymphoma is increasing in incidence world wide. On this background, in this paper

Non-Hodgkin's lymphoma presenting as multiple bone lesions and hypercalcemia.

We report a rare case of a patient with non-Hodgkin's lymphoma who developed multiple bone lesions and hypercalcemia. A 50-year-old woman complained of drowsiness and multiple bone pain on admission. Radiographic examination revealed multiple bone fractures and osteolytic lesions. She was diagnosed with diffuse large B cell lymphoma by biopsy of an inguinal lymph node. Elevation of parathyroid hormone-related protein (PTHrP) and hypercalcemia were confirmed pretreatment, and those serum levels decreased during chemotherapy for lymphoma. However, the disease was resistant to chemotherapy combin

Conjunctival lymphoma: case report.

OBJECTIVE: To report the diagnosis and therapeutic approach in the case of a patient with conjunctival lymphoma. CASE PRESENTATION: A 54-year-old caucasian female presented to the hospital with a painless conjunctival mass in the right eye of at least three months duration. The ophthalmological examination showed a "salmon patch" conjunctival lesion in the inferior fornix. An incisional biopsy of the conjunctival lesion was performed, and the histopathological examination confirmed the diagnosis of extranodal marginal zone B-cell conjunctival lymphoma. The CT scan showed no systemic involveme

[Current view on non-Hodgkin lymphomas. Origin and evolution in relation to cytogenetic-histologic correlations].

Recent advances in immunology, cytogenetics and molecular genetics has allowed for a better understanding of the origin and evolution of non Hodgkin lymphoma (NHL). Over the last decade a number of recurrent chromosome aberrations has been disclosed and some correlations with well defined histologic subsets of B-cell NHL has been established. Five important cytogenetic-histologic associations has been documented, well defined by combined cytologic, immunologic and genetic investigations: t(14;18) (q32;q21) and NHL of follicle centre cell origin, frequently with follicular histologic pattern; t

Pericardial effusion as first presentation of disseminated non-Hodgkin's lymphoma.

A 46-year-old woman with quiescent lupus presented with worsening pleuritic chest pain and dyspnoea. Bedside echocardiogram confirmed large pericardial effusion with cardiac tamponade. Emergency bedside pericardiocentesis was performed. Pericardial fluid cytology confirmed diffuse large B cell lymphoma, stage four on positron emission tomography. Conventional rituximab, cyclophosphamide, doxorubicin, vincristine and prednisolone chemotherapy achieved good response in all sites except the pericardium. Progressive cardiac involvement was complicated by atrioventricular conduction block requiring

Discovery of a BTK/MNK dual inhibitor for lymphoma and leukemia.

Bruton's tyrosine kinase (BTK) kinase is a member of the TEC kinase family and is a key regulator of the B-cell receptor (BCR)-mediated signaling pathway. It is important for B-cell maturation, proliferation, survival and metastasis. Pharmacological inhibition of BTK is clinically effective against a variety of B-cell malignances, such as mantle cell lymphoma, chronic lymphocytic leukemia (CLL), acute myeloid leukemia (AML) and activated B-cell-diffuse large B-cell lymphoma. MNK kinase is one of the key downstream regulators in the RAF-MEK-ERK signaling pathway and controls protein synthesis v

Multimodality Treatment of Gastric Lymphoma.

Gastric lymphoma is rare, accounting for 3% of gastric neoplasms and 10% of lymphomas. Treatment should be stratified based on histologic type, stage, Helicobacter pylori infection, and t(11;18) translocation status. Surgery is no longer a mainstay for treatment and should be reserved for rare situations such as perforation, fistula formation, and severe bleeding. Multimodal treatment, including H pylori eradication, radiation therapy, chemotherapy, and immunotherapy, should be provided as appropriate and can result in excellent outcomes.

Classification of 1,198 cases of bovine lymphoma using the National Cancer Institute Working Formulation for human non-Hodgkin's lymphomas.

A retrospective histologic study was made of 1,198 cases of bovine lymphoma using the National Cancer Institute Working Formulation for human non-Hodgkin's lymphoma. This classification scheme was found to be readily applicable to bovine lymphoma. Most of the cell types described in the National Cancer Institute Working Formulation occurred in this series of bovine lymphomas, but the distribution of cell types varied markedly compared to that of human beings. Eighty-nine percent (1,067/1,198) of bovine lymphomas were high-grade tumors. The diffuse large cell type and its cleaved variant compri

Immunopathology of non-Hodgkin lymphomas.

The immunopathological features of lymphocytic non-Hodgkin lymphomas (NHL), follicular centre cell tumours and "large-cell" NHL are described with particular emphasis on in situ evaluation of cryostat sections using monoclonal antibodies. The immunological phenotypes of tumour cells and the pattern of "reactive" cellular infiltrates with characteristic differences for the various subtypes are summarized. Immunological membrane features of the following lymphocytic NHL of B lymphocyte origin are discussed in detail: B-CLL (most common phenotype of the lymphoma cells: VIB-C5, To-15, Ia, Leu-1),

Histopathological Characteristics of Lymphomas in the Upper Aerodigestive Tract. A Single-Institute Study in Japan.

We analyzed the histopathological characteristics of lymphomas biopsied from the upper aerodigestive tract between 2000 and 2014 at the National Cancer Center Hospital in Japan. Of a total of 309 consecutive cases, the following incidences were observed: mature B-cell neoplasms, 77% (n = 239); mature T- and NK-cell neoplasms, 20% (n = 63); classical Hodgkin lymphomas, 0.7% (n = 2); and lymphoblastic lymphomas, 2% (n = 5). Lymphomas were most frequently (57%) detected in the oropharynx. The majority of cases (89%) were mature B-cell neoplasms (diffuse large B-cell lymphoma, 60%; follicular lymp

Clonal analysis of a case with fluid overload-associated large B-cell lymphoma evolving from primary large B-cell lymphoma of the central nervous system.

A 62-year-old female was referred to our hospital because of left pleural effusion. She had a history of primary large B-cell lymphoma (LBCL) of the CNS (PCNSL), which was treated with chemoradiotherapy 14 years ago. Imaging studies revealed no other lesions. Pathologically, large atypical lymphoid cells were present in the pleural fluid and positive for CD20 and CD79a, while EBV and HHV-8 were negative. She was diagnosed with HHV8-negative effusion-based lymphoma (EBL) and achieved complete response after two cycles of rituximab monotherapy. We performed targeted-capture sequencing of lymphom

Proliferation versus inflammation: Immunohistochemical profiling of feline chronic rhinitis and nasal lymphoma.

Lymphoma is the most common nasal cavity tumor in cats. While most recent research efforts have characterized neoplastic cell markers in feline alimentary lymphoma, only few reports are dedicated to immunohistological features of nasal lymphoid tumors beyond T- or B-cell phenotyping. In comparison, feline lymphoplasmacytic chronic rhinitis represents chronic, progressive lymphoplasmacytic inflammation, causing cyclical tissue damage and repair. Sixty-five cases of lymphoplasmacytic rhinitis and 15 cases of feline diffuse, large cell, immunoblastic nasal lymphoma were selected and submitted for

Detection of new drivers of frequent B-cell lymphoid neoplasms using an integrated analysis of whole genomes.

B-cell lymphoproliferative disorders exhibit a diverse spectrum of diagnostic entities with heterogeneous behaviour. Multiple efforts have focused on the determination of the genomic drivers of B-cell lymphoma subtypes. In the meantime, the aggregation of diverse tumors in pan-cancer genomic studies has become a useful tool to detect new driver genes, while enabling the comparison of mutational patterns across tumors. Here we present an integrated analysis of 354 B-cell lymphoid disorders. 112 recurrently mutated genes were discovered, of which KMT2D, CREBBP, IGLL5 and BCL2 were the most frequ

Characterization and clinical impact of the tumor microenvironment in post-transplant aggressive B-cell lymphomas.

Post-transplant lymphoproliferative disorders (PTLD) are iatrogenic immune deficiency-associated lymphoid/plasmacytic proliferations developing due to immunosuppression in solid organ or hematopoietic stem cell allograft patients. PTLD are characterized by abnormal proliferation of lymphoid cells and have a heterogeneous clinical behavior. We profiled expression of >700 tumor microenvironment (TME)-related genes in 75 post-transplant aggressive B-cell lymphomas (PTABCL). Epstein-Barr virus (EBV)-positive PT-ABCL clustered together and were enriched for type I interferon pathway and antiviral-r

Mathematical Model of CAR T-Cell Therapy for a B-Cell Lymphoma Lymph Node.

CAR T-cell therapies have demonstrated significant success in treating B-cell leukemia in children and young adults. However, their effectiveness in treating B-cell lymphomas has been limited in comparison to leukemia. In this paper we present a mathematical model that elucidates the dynamics of diffuse large B-cell lymphoma and CAR T-cells in a lymph node. The mathematical model aids in understanding the complex interplay between the cell populations involved and proposes ways to identify potential underlying dynamical causes of treatment failure. We also study the phenomenon of immunosuppres

Role of ultrasound-guided core biopsy in the evaluation of spleen pathology.

Core biopsy has not traditionally been recommended in the study of spleen nodules due to the supposed fragility of this organ leading to a high risk of post-core biopsy complications. A total of 13 patients who presented solid spleen nodules, diffuse splenomegaly, or both on imaging studies (CT, MR, US) were biopsied under ultrasound control with 18G BioPince needles. Cytological (imprints and cytocentrifugates) and histological material were obtained for diagnosis in every case. Malignant lymphomas were the most commonly found pathology (four diffuse large B-cell lymphomas, two follicular lym

High-Dose Chemotherapy and Autologous or Allogeneic Transplantation in Aggressive B-Cell Lymphoma-Is There Still a Role?

Novel therapies such as CAR-T, BTK inhibitors and PD-1 inhibitors have changed the management of aggressive B-cell lymphomas. Nonetheless, these novel therapies have their own risk of late toxicities including second malignancies. They also create a subgroup of patients with relapse, treatment failure, or indefinite maintenance. We discuss the current role of autologous and allogeneic stem cell transplantation in this context. In patients with recurrent diffuse large B-cell lymphoma, CAR-T cell treatment has largely replaced autologous transplant. Autologous transplant should be considered in

Immunohistologic characterization of two malignant lymphomas of germinal center type (centroblastic/centrocytic and centrocytic) with monoclonal antibodies. Follicular and diffuse lymphomas of small-cleaved-cell type are related but distinct entities.

The relationship between follicular lymphomas and diffuse lymphomas of small-cleaved-cell type was investigated with the use of a panel of antibodies against B-cell differentiation antigens. Follicular lymphomas, regardless of histologic subtype, were immunologically homogeneous: Ig+ B1+ B2+ CALLA+ Ia+. Two cases were Ig-negative, and 4 were CALLA-negative. Diffuse small-cleaved-cell (centrocytic) lymphomas were more heterogeneous. The majority were Ig+ B1+ B2+ Ia+ T1+ CALLA-. A minority were B2-negative, T1-negative, or CALLA-positive. An increased frequency of Ig heavy chain class switching
