BACKGROUND: Intravesical Bacillus Calmette-Guérin (BCG) therapy is an established treatment for non-muscle-invasive bladder cancer, involving direct instillation of a solution containing attenuated Mycobacterium bovis into the bladder. Although generally well-tolerated, rare vascular complications, including mycotic aneurysms and aortoenteric fistulas, have been reported. CASE PRESENTATION: We report the case of a 76-year-old Japanese man who developed an aortoesophageal fistula three years after completing intravesical BCG therapy for superficial bladder carcinoma. The patient initially prese…

Dihydroquinazolin-4(3H)-imines have emerged as a promising scaffold for developing novel antimycobacterial agents. Here, we integrated QSAR modeling, in vitro screening, molecular dynamics (MD), binding free-energy calculations, and in vivo toxicity assessment to identify potent inhibitors against Mycobacterium tuberculosis (Mtb). Four QSAR models demonstrated strong internal and external reliability (R2 > 0.68; Q2LOO > 0.62; R2extup to 0.82). Additional validation parameters met recommended thresholds (Q2-F1/F2/F3 ≥ 0.72; CCCext ≥ 0.88; r2m aver ≥ 0.72; r2m delta: 0.04), and Williams plots co…

Protein tyrosine phosphatase B (PtpB) is a virulence-associated phosphotyrosine phosphatase secreted by Mycobacterium tuberculosis (Mtb), known to disrupt host immune signaling by dephosphorylating key proteins. Targeting PtpB represents a rational strategy for anti-TB drug discovery. This study presents an integrative computational framework for identifying and evaluating small-molecule inhibitors of Mtb PtpB. QSAR models were constructed using four molecular fingerprint types, CDK, PubChem, MACCS, and AtomPairs2DCount, as regression models predicting pIC50 values. Multiple machine learning a…

BACKGROUND: Diagnostic performance of tongue swab Mycobacterium tuberculosis PCR has been evaluated for facility-based triage of symptomatic tuberculosis (TB). It is unknown whether tongue swab performance differs for detection of asymptomatic TB in community-based screening.
METHODS: Tongue swabs were collected from adult household contacts of TB patients (HHC Cohort), and symptomatic adults presenting to clinics with presumptive TB (Clinic Cohort), at eight South African sites. TB Cases were defined by positive sputum Xpert Ultra or liquid culture, performed in all participants; and matched…

It was shown previously that mycobacteria (Mycobacterium smegmatis and Mycobacterium tuberculosis) accumulate tetramethyl ether of coproporphyrin III (TMC) during transition to dormant state. However, the enzymes involved in TMC synthesis remained unknown. Here we have identified a new M. smegmatis methyltransferase MSMEG_0614 (CPmtA) that catalyzes the S-adenosyl-L-methionine-dependent conversion of coproporphyrin (CP) into TMC through sequential methylation of its carboxyl groups. Overexpression of MSMEG_0614 in both M. smegmatis and M. tuberculosis leads to increase in intracellular TMC lev…

Mycobacterium tuberculosis (MTB), the causative agent of tuberculosis, is currently resistant to antibiotics and has undergone transcriptional adaptation to them, yet only a few transcription factors regulating mycobacterial drug resistance have been identified. In this study, the transcriptional regulatory protein Rv2250c of the TetR family was found to be a regulatory molecule of INH. Compared with wild-type strains, strains lacking the rv2250c gene showed higher minimum inhibitory concentrations (MICs) and greater intracellular survival in macrophages under INH stress. Furthermore, the INH …

In searching for novel molecules to act as antibacterial agents, particularly against Mycobacterium tuberculosis bacteria, three series of C5- and C6-substituted 1,2,3-triazine compounds were investigated: 1,2,3-triazine-4-carboxylate 1-oxide (series 1), 1,2,3-triazine-4-carboxylate (series 2), and 3,6-dihydro-1,2,3-triazine-4-carboxylate 1-oxide derivatives (series 3). Their structural elucidation was confirmed by 1H-NMR, 13C-NMR, and HRMS. We determined their antibacterial activity (MIC value) using the MABA against the M. tuberculosis H37Rv strain, as well as their physicochemical and pharm…

INTRODUCTION: Memory CD4+ T cells are central to long-term immunity in tuberculosis (TB), yet their functional roles that define their protective capacity remain unclear. Understanding the immune mechanisms that prevent clinical progression from latent TB infection (LTBI) to active TB disease is critical for the development of next-generation vaccines and biomarkers.
METHODS: We characterized the transcriptomic, metabolic, and functional programs of Mycobacterium tuberculosis (Mtb) antigen-stimulated peripheral CD4+ T stem cell (T-SCM), central (T-CM), transitional (T-TM), and effector (T-EM)…

The emergence of drug-resistant Mycobacterium tuberculosis underscores the need for innovative therapeutic strategies targeting essential metabolic pathways. We designed, synthesized, and evaluated a series of dual inhibitors targeting two key enzymes of the mycobacterial FAS-II system, HadAB and InhA. Using a diaryl ether scaffold, six thiosemicarbazone derivatives and their aldehyde intermediates were prepared and tested for enzymatic and antimycobacterial activity. Thiosemicarbazone derivatives and aldehyde intermediates both strongly inhibited InhA, and the thiosemicarbazones additionally …

BACKGROUND: Aerosol exposure of non-human primates (NHPs) to Mycobacterium tuberculosis (Mtb) typically results in discrete sites of inflammation of the lung that is detectable by 2-deoxy-2-[fluorine-18]fluoro-D-glucose (18 F-FDG)-based PET/CT scans. Such scans are often analyzed using software such as Invicro VivoQuant or OsiriX as 3D images by manual labeling sites of PET signal using 2D slices and by reporting maximal standardized uptake value (SUV max) either of the whole lung or of individual lesions.
METHODS: Here we propose a pipeline for analysis of the same PET/CT scans using Imaris,…

We report a case of localized Mycobacterium abscessus complex pulmonary disease with phenotypic inducible macrolide resistance in a previously healthy woman in her 40s. Chest computed tomography (CT) showed a solitary thin-walled cavitary nodule with perilesional centrilobular nodules, confined to the right upper lobe. Bronchial lavage culture grew M. abscessus subsp. abscessus/bolletii. Clarithromycin susceptibility shifted from susceptible on Day 3 to resistant on Day 14. Combination antimicrobial therapy was initiated with imipenem/cilastatin, amikacin, azithromycin, clofazimine, and sitafl…

Mycobacterium abscessus has emerged as one of the most formidable pathogens affecting people with cystic fibrosis (pwCF), driven by intrinsic antimicrobial resistance, complex taxonomy, and substantial diagnostic uncertainty. Despite major technological advances, establishing a diagnosis of pulmonary disease remains challenging, as traditional culture-based workflows are constrained by limited sensitivity, prolonged turnaround times. However, recent progress in proteomic and genomic technologies has markedly improved organism detection and taxonomic resolution. This review examines the contemp…

The historical model, which posits that β-lactams inhibit bacterial growth while β-lactamase inhibitors (BLIs) merely protect β-lactams from enzymatic degradation, fails to fully explain their activity against Mycobacterium abscessus (Mab). This study demonstrates that synergistic effects extend beyond the traditional one β-lactam + one BLI paradigm, refuting the oversimplified mechanistic framework. First, β-lactam-based BLIs such as clavulanic acid, sulbactam, and tazobactam exhibit intrinsic antibacterial activity against Mab. These agents synergized not only with β-lactams but also with on…

Tuberculosis (TB), caused by members of the Mycobacterium tuberculosis complex, remains a critical global health challenge, affecting humans and a wide range of domestic and wild animals. Despite the availability of anti-TB drugs, cure rates remain suboptimal, exacerbated by the rise of multidrug-resistant TB strains. The Bacille Calmette-Guérin (BCG) vaccine, the only licensed vaccine against TB, has demonstrated efficacy in reducing lesion severity and bacterial burden in animals, as well as lowering TB-related and all-cause mortality in infants. However, BCG presents several safety concerns…

Background/Objectives: Effective prophylaxis for Mycobacterium tuberculosis (Mtb) requires greater understanding of immune correlates of protection. With renewed interest in BCG as an Mtb vaccine, particularly via the intravenous (IV) route, our objective was to characterize both innate and adaptive immune correlates of vaccine-induced pulmonary immunity as potential biomarkers for protective efficacy in a murine model of Mtb infection. Methods: Mice were given BCG via different routes and some boosted with recombinant virus constructs encoding Mtb Ag85B. Responding innate lymphoid cell (ILC) …
