Intraoperative trans-laryngeal ultrasound (LUSG) of the vocal cord is a novel method of confirming the recurrent laryngeal nerve (RLN) integrity during thyroid and neck surgery.

BACKGROUND: Eliciting a normal electromyography signal has been the usual method to confirm the functional integrity of the recurrent laryngeal nerve during intraoperative nerve monitoring. Given that oscillations of the vocal cord can be detected with trans-laryngeal ultrasound when the ipsilateral recurrent laryngeal nerve is stimulated with the endotracheal tube in situ, we aimed to compare the accuracy and cost of this novel method with the conventional electromyography method. METHODS: Consecutive patients who underwent elective thyroid, parathyroid or neck dissection procedures were inc…

Chyle fistula as complication of thyroid surgery in malignant pathology.

INTRODUCTION: Cervical chyle fistula (CF) is exceptional, and there is no agreement on its treatment. The study objective was to assess the efficacy of conservative and surgical treatment in this condition. METHOD: Study population: Patients undergoing thyroid surgery who experience CF after the procedure. Study groups: 1) Group with conservative treatment: CF with flow rates less than 300cm3/day, late or collected in the neck; and 2) Group with surgical treatment: early CF with flow rates greater than 300 cm3/day. STUDY VARIABLES: Cure rates and rate of conversion to surgery were analyzed.

The prognostic value of regional lymph node metastases in patients of Guangdong Province, China with differentiated thyroid cancer: A multicenter retrospective clinical study.

Although the prognostic value of nodal metastases in differentiated thyroid cancer remains controversial, it is of interest to evaluate and understand the different characteristics of predictive outcomes.A multicenter retrospective study was conducted in 215 untreated patients with differentiated thyroid cancer from July 1997 to July 2015 in 4 medical centers of Guangdong Province. A total of 107 patients with nodal metastases (group A) were compared to 108 patients without metastases (group B). The 5-year disease-free survival (DFS), overall survival (OS), and postoperative complications in b

Total thyroidectomy and central lymph node dissection. Experience of a referral centre for endocrine surgery.

AIM: Thyroid cancer prognosis is determined by several variables, even with extremely elevated survival rate. The most debated issues are the type of thyroidectomy and extension of lymphadenectomy. Aim of the study is the analysis of benefits of level VI lymphadenectomy associated to total thyroidectomy in the treatment of thyroid cancer. PATIENTS AND METHODS: 316 total thyroidectomy with central node dissection were carried out in the Unit of Endocrine Surgery, University of Perugia. Direct parathyroid auto-implantation was carried out if damage or accidental excision occurred. High risk pat

Reconstruction of combined oral mucosa-mandibular defects using the vascularized myoosseous iliac crest free flap.

The authors present five cases of combined oral mucosa-mandible defects reconstructed with the vascularized internal oblique-iliac crest myoosseous free flap. This technique has many advantages compared to other conventional methods such as the radial flap, scapula flap, and fibula flap. Vascularized iliac crest flaps provide sufficient high-quality bone suitable for reconstructing segmental mandibular defects. Although fibular flaps allow longer donor bone tissue to be harvested, the iliac crest can provide an esthetic shape for mandibular body reconstruction and also provides sufficient bone

Marginal recurrences after selective targeting with intensity-modulated radiotherapy for oral tongue cancer.

BACKGROUND: No universal consensus of optimal radiation target coverage for oral tongue cancer exists, and there is wide variability in practice. Some centers use intensity-modulated radiotherapy (IMRT) to selectively target only certain regions at risk while sparing others; however, patterns of failure after such selective targeting are rarely reported. METHODS AND RESULTS: We critically examined the location of failure in 4 patients with stage III to IV oral tongue cancer who presented to our department with locoregional recurrence after receiving IMRT with selective radiation targeting at

Surgical management of squamous cell carcinoma of the soft palate: factors predictive of outcome.

BACKGROUND: Squamous cell carcinoma of the soft palate (SCCSP) is uncommon. The aim of this study was to report our experience and identify factors predictive of outcome. METHODS: Between 1976 and 2005, 186 patients with SCCSP were treated with curative intent. In all, 150 patients had primary surgery, of whom 112 patients (75%) had cT1/T2 tumors and 103 patients (69%) had cN0 necks. Overall survival (OS), disease-specific survival (DSS), and recurrence-free survival (RFS) were determined by the Kaplan-Meier method and factors predictive of outcome were identified. RESULTS: Five-year OS, DSS

Treatment of N in the upper maxillary tumors.

From 2002 to 2008, 86 patients have undergone surgical treatment of malignant upper maxillary tumors at the Maxillo-Facial Surgery Department, Tor Vergata University, Rome. All the N-positive patients at the time of the T therapy have undergone lymph node surgical emptying. In 6 patients, a laterocervical emptying was performed when laterocervical metastases were found. In the remaining 68 patients, with no evidence of N, we did not perform laterocervical emptying. We found in our patients a high percentage of cervical metastasis in T2 squamous cell carcinoma of the maxilla (32.1%). In this ar

Concurrent platinum-based chemotherapy and simultaneous modulated accelerated radiation therapy for locally advanced squamous cell carcinoma of the tongue base.

BACKGROUND: Randomized data support use of chemotherapy concurrently with radiation in treatment of advanced squamous cell carcinoma (SCC) of the oropharynx. Intensity modulated radiation therapy (IMRT) is increasingly being used to deliver such radiotherapy; no published reports specifically describe results of chemotherapy with IMRT for SCC of the base of tongue (BOT). We present outcomes data using simultaneous modulated accelerated radiation therapy (SMART) combined with platinum-based chemotherapy in treatment of locally advanced SCC of the BOT METHODS: The records of the Otolaryngology/H

Surgery in the management of mediastinal carcinoid.

Primary carcinoid tumors of the mediastinum were described for the first time in 1972 as thymic carcinoids. Our experience with 16 patients who underwent diagnostic and surgical procedures at the Mayo Clinic is presented. All of these patients had mediastinal carcinoid. The surgical procedures included node biopsy, anterior mediastinotomy (Chamberlain), median sternotomy and posterior lateral thoracotomy. Complete resection was possible in 9 (56.3%) patients, 3 (18.7%) had partial removal (debulking), and 4 (25%) had diagnostic biopsies only. The operative morbidity was 25%. There were no post

Surgical Management of Oral Squamous Cell Carcinoma With Reconstruction of the Oral Defect Using a Lingual Island Flap: Case Report.

Oral squamous cell carcinoma (OSCC) is the most common malignant neoplasm of the oral cavity and requires surgical resection with adequate oncologic margins and functional reconstruction of the resulting defect. We report the case of a 78-year-old male patient with locally recurrent well-differentiated OSCC who was treated with surgical resection with 10 mm safety margins, reconstruction with an island tongue flap, and selective bilateral Level IA neck dissection. Although microvascular free flaps represent the reconstructive gold standard for extensive defects and the submental flap is a reli…

Gingival Squamous Cell Carcinoma: Diagnostic Challenges and Clinical Implications.

Oral squamous cell carcinoma (OSCC) is one of the highly invasive malignancies globally, accounting for the vast majority of all oral cancer … reatment, which included surgical excision of the tumor with segmental mandibulectomy and selective neck dissection. The patient is still being monitored one and a half years after the treatment and does not show any signs of recurrence. Therefore, the lesion must be identified early to start treatment, stop metastases, and enhance the prognosis.

Surgical management of an intraluminal thrombus in the external jugular vein: A case report with near-infrared visualization as a useful adjunct.

BACKGROUND: Surgical removal of a mass in the external jugular vein requires precise localization to ensure optimal cosmetic outcomes. While preoperative imaging is standard, intraoperative anatomical shifts can occur. We report a case where a near-infrared vein visualization device was used as a supplementary tool during the excision of an intraluminal thrombus. CASE DESCRIPTION: A 50-year-old woman presented, complaining of a subcutaneous mass in the left neck. She used a sports bra for the previous 6 weeks to participate in a marathon race. Ultrasonography revealed an intraluminal mass ari

Course Patterns of the Marginal Mandibular Branch of the Facial Nerve over the Facial Vessels in Relation to the Inferior Mandibular Border.

The marginal mandibular branch of the facial nerve (MMB) is a surgically relevant extracranial branch of cranial nerve VII, vulnerable during submandibular, parotid, and cervicofacial procedures. Its relationship to the facial vessels and the inferior border of the mandible (IBM) is variable and has been studied for more than six decades. This review analyses the course of the MMB as it crosses the facial artery and vein, focusing on its vertical position relative to the IBM, together with branching patterns, fascial relationships, anastomoses, landmarks, injury, and recovery. The MMB usually
