Optic nerve thickening and infarction as the first evidence of orbital involvement with mucormycosis.

We report a 53-year-old female with uncontrolled diabetes mellitus who presented with decreased vision in the right eye for a few hours duration. Orbital computed tomography and magnetic resonance imaging were performed at presentation and showed a thickening and infarction of the right optic nerve with no other orbital abnormalities. A few days later, the patient developed necrosis in the region of the right medial canthus and nasal mucosa. Tissue biopsy confirmed the diagnosis of mucormycosis.

Radiation-induced cavernous malformations of the cauda equina mimicking carcinomatous or infectious meningitis. A case report.

The authors present a case of multiple radiation-induced cavernous malformations of the cauda equina in a patient with a remote history of testicular cancer and extended field radiation therapy. Magnetic resonance imaging (MRI) demonstrated multiple nodular areas of enhancement coating the nerve roots of the cauda equina, mimicking an aggressive leptomeningeal process such as carcinomatous or infectious meningitis. Biopsy of one of these lesions revealed ectatic vascular channels devoid of intervening neuroglial tissue consistent with cavernous malformation.

Lasers in surgery for chronic ear disease.

This article describes specific situations in which the laser has been found to be useful in surgery for chronic ear disease. In the opinion of the authors, the most important application is the atraumatic removal of cholesteatoma from a mobile stapes. Additional uses include precise and hemostatic removal of diseased tissue (polyps, granulations, adhesions) and manipulations upon an intact ossicular chain without induction of vibrational trauma. Potential complications such as facial nerve and inner ear injury are considered.

Neurophysiological evaluation of disorders and procedures affecting the spinal cord and the cauda equina.

Neurophysiological tests that asses afferent or efferent pathways of the spinal cord and of nerve roots are used for clinical diagnosis, intraoperative monitoring, and judgement of prognosis. Most tests can readily be performed in awake subjects, whereas many difficulties arise in anesthetized patients in whom neurophysiological testing is the only way of monitoring functional integrity of neural tissue. The research, therefore, that has been carried out in this field during the past year is of special interest.

The surgical treatment of thyroid cancer. The primary disease.

Controversy surrounding the surgical treatment of thyroid cancer is discussed. The various approaches are lobectomy, isthmusectomy and lobectomy, subtotal thyroidectomy (sparing the capsule and some tissue on the contralateral side in an effort to decrease the complications of bilateral recurrent nerve paralysis and hypoparathyroidism), and total thyroidectomy. The surgical treatment options for each histologic type of thyroid gland malignancy are also discussed. For well-differentiated carcinomas of the thyroid, a strong case is made for total thyroidectomy.

From blueprint to build: Metal ions in peripheral nerve development and engineering regeneration.

Peripheral nerve injury (PNI) poses a substantial global health burden, affecting over 20 million individuals annually with frequent suboptimal recovery and persistent disability. The inherent limitations of nerve autografts, such as donor site morbidity and limited supply, underscore the clinical importance of nerve guidance conduits (NGCs) as a promising alternative. Nonetheless, the efficacy of current NGCs remains limited by their inability to recapitulate the spatiotemporally precise molecular cues of the native regenerative microenvironment. Our network meta-analysis highlights a signifi

Laser speckle flowgraph reveals dynamic characteristics and clinical relevance of choroidal watershed and peripapillary hypoperfusion zones.

The hemodynamic characteristics of the choroidal watershed zone (CWZ) have not been fully quantified. We aimed to characterize the blood flow and pulsatility within the choroidal hypoperfusion zone (CHZ) and investigate its relationship with adjacent optic nerve head (ONH) perfusion in healthy eyes, using laser speckle flowgraphy (LSFG). Blood flow and pulsatile waveform metrics were compared within and outside the CHZ and in optic nerve head (ONH) sectors in 100 healthy participants. Two patients with non-arteritic anterior ischemic optic neuropathy (NAION) were also reviewed as the illustrat

A Case Report of Right Phrenic Nerve Palsy Following Pulsed Field Ablation in a Patient with Persistent Atrial Fibrillation.

Pulsed field ablation (PFA) is a nonthermal ablation modality that achieves myocardial ablation through irreversible electroporation and has been associated with a reduced risk of collateral tissue injury. Phrenic nerve injury is uncommon with PFA, and persistent injury has rarely been described. A 61-year-old woman with symptomatic persistent atrial fibrillation refractory to anti-arrhythmic drug therapy underwent catheter ablation using the PulseSelect™ PFA system (Medtronic, Minneapolis, MN, USA). Circumferential PFA applications were delivered to all four pulmonary veins, followed by addit

An Optimized Ex Vivo Protocol for Quantitative Electrophysiological Assessment of Neuromuscular Junctions and Skeletal Muscle Function Using the Aurora System.

The neuromuscular junction (NMJ) is critical for muscle function, and its dysfunction underlies conditions such as sarcopenia and motor neuron diseases. Current protocols for assessing NMJ function often lack standardized stimulation parameters, limiting reproducibility. This study presents an optimized ex vivo method to evaluate skeletal muscle and NMJ function using the Aurora Scientific system, incorporating validated stimulation protocols for both nerve and muscle to ensure consistency. Key steps include tissue preparation in a low-calcium, high-magnesium solution to preserve NMJ integrity

[Shaft fractures of the upper extremities and concomitant injuries : Epidemiology, classification, diagnostics and clinical relevance].

Shaft fractures of the upper extremities represent a significant challenge in trauma surgery due to their functional importance. The epidemiological analysis reveals characteristic distribution patterns: humeral shaft fractures (1.2-5% of all fractures) show a bimodal age distribution, with peaks at 20-30 years and above 60 years. Forearm shaft fractures (13.8/100,000 inhabitants [1]) occur particularly frequently in children and decrease with advancing age. The classification primarily follows the AO/OTA system, which was comprehensively revised in 2018, supplemented by specific classificatio

Passive Monitoring to Active Regulation: Closed-Loop Piezoelectric Theranostic Platform Enabling On-Demand Vagus Stimulation for Terminating Inappropriate Sinus Tachycardia.

Inappropriate sinus tachycardia (IST), characterized by unexplained acceleration of sinus rhythm, poses significant diagnostic and therapeutic challenges due to its paroxysmal and limited treatment options. This study proposes a closed-loop theranostic platform (TP-IST) based on an implantable piezoelectric fiber patch (iPFP) for on-demand vagus nerve stimulation for real-time monitoring of heartbeat and terminating IST, achieving a shift from traditional passive monitoring to revolutionary active regulation. Due to the enhanced piezoelectric response of the hot-press-treated piezoelectric fib

A responsive cascade drug delivery scaffold adapted to the therapeutic time window for peripheral nerve injury repair.

Peripheral nerve injury (PNI) is a common clinical challenge, requiring timely and orderly initiation of synergistic anti-inflammatory and reparative therapy. Although the existing cascade drug delivery system can realize sequential drug release through regulation of the chemical structure of drug carriers, it is difficult to adjust the release kinetics of each drug based on the patient's condition. Therefore, there is an urgent need to develop a cascade drug delivery system that can dynamically adjust drug release and realize personalized treatment. Herein, we developed a responsive cascade d

Delayed Cranial Nerve Palsies and Chiari Type I Malformation After Epidural Anesthesia in the Setting of Childbirth.

Epidural analgesia is an efficient method of controlling pain and has a wide spectrum of therapeutic and diagnostic applications. Potential complications may occur in a delayed fashion, can remain undiagnosed, and can be a source of significant morbidity. We present a 37-year-old woman presented with severe spontaneous occipital headaches, diplopia, and dizziness that occurred spontaneously six weeks after giving birth. Her primary method of pain control during labor was epidural analgesia. Her neurologic exam revealed a cranial nerve six palsy with ptosis, and her brain MRI demonstrated a Chi

Early disruption of nerve mitochondrial and myelin lipid homeostasis in obesity-induced diabetes.

Diabetic neuropathy is a major complication of diabetes. Current treatment options alleviate pain but do not stop the progression of the disease. At present, there are no approved disease-modifying therapies. Thus, developing more effective therapies remains a major unmet medical need. Seeking to better understand the molecular mechanisms driving peripheral neuropathy, as well as other neurological complications associated with diabetes, we performed spatiotemporal lipidomics, biochemical, ultrastructural, and physiological studies on PNS and CNS tissue from multiple diabetic preclinical model

FGF6 enhances muscle regeneration after nerve injury by relying on ERK1/2 mechanism.

BACKGROUND: Severe peripheral nerve injury leads to skeletal muscle atrophy and impaired limb function that is not sufficiently improved by existing treatments. Fibroblast growth factor 6 (FGF6) is involved in tissue regeneration and is dysregulated in denervated rat muscles. However, the way that FGF6 affects skeletal muscle repair after peripheral nerve injury has not been fully elucidated. METHODS: In this study, we investigated the role of FGF6 in the regeneration of denervated muscles using myoblast cells and an in vivo model of peripheral nerve injury. RESULTS: FGF6 promoted the viabil
