Displaced unstable transverse fractures of the shaft of the proximal phalanx of the fingers in industrial workers: reduction and K-wire fixation leaving the metacarpophalangeal and proximal interphalangeal joints free.

A series of 35 adult male industrial workers with displaced unstable transverse fractures of the shaft of the proximal phalanx of the fingers were treated with reduction and K-wire fixation leaving the metacarpophalangeal and interphalangeal joints free to move immediately after surgery. At final follow-up, the total active motion score of the injured finger was graded as excellent, good, fair, or poor if it was greater than 240°, 220-240°, 180-219°, or less than 180°, respectively. Complications were also documented. The results were compared with our previously published series of these frac

Pediatric seatbelt injuries: diagnosis and treatment of lumbar flexion-distraction injuries.

Motor vehicle accidents are the major cause of flexion-distraction injuries of the thoracolumbar spine. In a retrospective review, we present the results of operative treatment for six pediatric patients who sustained such injuries while wearing seatbelts. There were three purely ligamentous injuries, two bony injuries (Chance fractures), and one combination injury. There were also concomitant neurological and intra-abdominal injuries. Of note is that two patients had either their spinal or abdominal injury missed on initial evaluation. All patients were treated surgically with open reduction

Lumbar Hyperextension Fracture after Direct Anterior Total Hip Arthroplasty.

INTRODUCTION: Patients with stiff and brittle spines, such as those with ankylosing spondylitis (AS), are at an increased risk of intraoperative and post-operative complications. Specifically, patients with AS who undergo total hip arthroplasty (THA) with a direct anterior approach are at risk for vertebral fractures due to patient positioning and manipulation necessary to utilize this approach. To the best of our knowledge, this is the second publication discussing vertebral fracture following THA with direct anterior approach and the first depicting a hyperextension fracture. CASE REPORT: A

Clinical, functional and radiological outcome after osteosynthesis of ankle fractures using a specific provocation test.

BACKGROUND: Ankle fractures are frequent, and despite numerous publications on their treatment and outcome, there is a lack of precise data on the functional results in young, healthy and physically active patients. We hypothesized that patients who underwent open reduction and internal fixation (ORIF) for simple ankle fractures would have similar function compared to a healthy control group, whereas patients with complex fractures will have significant functional deficits. Furthermore, we postulate that there is a discrepancy between the radiological and the functional outcomes. METHODS: A s

The "Bag of Bones" Treatment of Comminuted Intra-articular Distal Humerus Fractures in the Elderly.

BACKGROUND: Extensively comminuted intra-articular distal humerus fractures in the elderly present a challenging therapeutic dilemma. The purpose of this study was to investigate the results of nonoperative treatment of these fractures in a select subset of patients. METHODS: Patients treated with nonoperative management for a comminuted intra-articular distal humerus fracture between 2007 and 2018 were reviewed. Patients were administered 3 elbow-specific functional outcomes instruments. RESULTS: A total of 8 patients (2 men, 6 women) were treated with brief immobilization followed by early

Mini Fragment Locking Compression Plate Fixation of a Rare Case of Displaced Medial End Clavicle Fracture.

Clavicle fractures at the medial end are very rare. Even in cases where there is severe displacement, such fractures have usually been managed nonoperatively. Yet, there are many patients who remain symptomatic over a year following injury, and the non-union rate is also high. Operative intervention for displaced clavicle fractures of the medial end has been more common in the past decade. The possibility of iatrogenic injury due to the near proximity of critical vascular structures continues to be a concern. This case report describes the management of a rare displaced medial end clavicle fra

Relationship between midfacial fractures and maxillary sinus pathology.

BACKGROUND: Midfacial fractures frequently involve the maxillary sinus, leading to maxillary sinus pathology. We aimed to examine the incidence and contributing factors of maxillary sinus pathology in patients who underwent open reduction and internal fixation (ORIF) for midfacial fractures. METHODS: A retrospective analysis was conducted on patients who underwent ORIF for midfacial fractures at our department over the past 10 years. The incidence of maxillary sinus pathology was identified clinically and/or by computed tomography findings. Factors that significantly influenced the groups wit

Supracondylar humerus fractures in children caused by monkey bar-related injuries: a comparative study.

We evaluated the functional and radiological outcomes of surgically treated supracondylar humerus fractures based on injury mechanisms, as well as the therapeutic and prognostic aspects of monkey bar-related injuries, in this study. Between December 2014 and December 2018, pediatric patients who had undergone surgical treatment for supracondylar humerus fractures were analyzed retrospectively. According to the mechanism of injury, patients were divided into two groups: monkey bar-related injuries and other injuries. Patients were evaluated functionally using the Mayo elbow performance score, w

Comparison of Three Different Surgical Fixation Techniques in Pediatric Forearm Double Fractures.

Introduction Although forearm fractures are one of the most common fractures in children, controversy remains about the optimal fixation technique in the surgical treatment of these fractures. The aim of this study was to compare the results of pediatric patients with a forearm double fracture who underwent open reduction and internal fixation (plate-screw group), elastic intramedullary nailing to both bones (ESIN), and hybrid fixation (the radius or ulna is fixed with the help of ESIN and the other forearm bone is fixed using plate-screw). Methods A retrospective scan was made of the records

The increasing role of reverse total shoulder arthroplasty in the treatment of proximal humerus fractures.

INTRODUCTION: Hemiarthroplasty of the shoulder is a well established treatment for proximal humerus fractures not amenable to open reduction internal fixation. However, orthopedic surgeons have recently increased utilization of reverse total shoulder arthroplasty (RTSA) in the treatment of these injuries. The purpose of this study was to evaluate the use of hemiarthroplasty and RTSA between 2009 and 2016 for the treatment of proximal humerus fractures within a large United States integrated healthcare system. MATERIALS AND METHODS: A descriptive study was conducted using our integrated health

Therapeutic effects of volar anatomical plates versus locking plates for volar Barton's fractures.

Volar Barton's fractures are uncommon and extremely unstable. Various surgical techniques have been reported in the literature, but open reduction and internal fixation using a volar plate system is currently advocated for the treatment of volar Barton's fractures. The purpose of this study was to compare the therapeutic effects of volar anatomical plates and volar locking plates in the treatment of volar Barton's fractures. A retrospective comparative study was performed of 33 patients with volar Barton's fractures treated between October 2005 and November 2010. One group was treated with a v

Pediatric Humeral Shaft Fracture Nonunion: A Case Report.

Humeral shaft fractures in the pediatric population are a commonly encountered injury in everyday practice. Most patients with these injuries are treated without surgery and go on to have an uneventful recovery. Nonunion of these injuries in the pediatric population has been reported only once in the literature. This case report follows a 13-year-old female after a seemingly standard transverse humeral shaft fracture. The patient was treated with a fracture brace initially. No signs of healing were noted at the eight-week post-injury follow-up. The family elected for continued conservative man

A new classification of mandibular fractures.

There is no accepted method of reporting mandibular fracture that reflects incidence, treatment and outcome for individual cases. As most series include anatomical site only for all fractures, the aim was to establish a new method to report fractures based on a systematic review of the literature and an internal audit. The classification proposed is: Class I; condyle, II; angle, IIc; II+condyle, III; body/symphysis, IIIc; III+condyle, IV; multiple fractures not including condyle, IVc; IV+condyle, V; bilateral condyle±other fracture(s). A total of 10,971 adult and 914 paediatric cases were anal

Outcome of percutaneous screw fixation of scaphoid fractures.

The optimal treatment of minimally displaced or nondisplaced fractures of the scaphoid is unclear. Traditionally, management of these fractures has been unpredictable with a significant risk of nonunion when treated conservatively. This study examined the results from 32 patients who underwent percutaneous screw fixation using a 3.0-mm AO/ASIF cannulated screw for a nondisplaced or minimally displaced fracture of the scaphoid waist. Eighteen patients were available for final follow up (average 3.2 years) including administration of the DASH questionnaire, a physical examination, and final radi
