[Traumatic shoulder dislocation in young athletes--open or arthroscopic stabilization?].

In case of younger patients up to the 30th year with high sports and professional shoulder stress unacceptable recurrence rates up to 49% following arthroscopic stabilization were published. For these patients we recommend open stabilization also in case of primary dislocation. To evaluate our own results following open stabilization with suture-anchors and capsular shift we examined 15 patients with traumatic anterior shoulder dislocation treated between 1.1.1994-31.12.1997 in our clinic retrospectively. Because of the known prognosis factors all patients were classified as high-risk for recu

Scar quality from partial or complete wound closure using the Cincinnati incision for clubfoot surgery.

The method of skin closure in using the Cincinnati incision for clubfoot surgery may influence the quality of the scar obtained and the postoperative outcome. We retrospectively reviewed two groups of patients who underwent either primary skin closure (14 feet in 11 patients) or partial wound closure and healing by secondary intention (15 feet in 11 patients). The quality of the scars from both of these methods were equally acceptable cosmetically, and neither technique predisposed the patient to any long-term wound complications. In addition, we noted that closing the skin may act as a soft t

Core decompression for avascular necrosis of the femoral head.

The results of core decompression as the sole treatment for Ficat stages 1, 2 and 3 avascular necrosis of 51 femoral heads in 39 patients were studied. The mean follow-up time was 24 months (6-47 months); 19 hips (38.8%) were good and 30 hips (61.2%) had failed (two hips were lost to follow-up). The good results as stated in the literature could not be obtained, but we believe core decompression can delay the need for total hip replacement in avascular necrosis of the femoral head.

[Endocrine reactions, circulatory and resuscitation behavior in ketamine-midazolam anesthesia. A comparative study of ketamine racemate vs. (S)-ketamine in knee surgery].

UNLABELLED: Clinically used ketamine is a racemic mixture of two isomers, (S)- and (R)-ketamine, in equal amounts. Previous investigations showed the anaesthetic potency of (S)-ketamine to be three times higher than that of (R)-ketamine. The aim of this study was to compare the effects of (S)-ketamine/midazolam and racemic ketamine/midazolam on endocrine and cardiovascular parameters, recovery, and side effects in unpremedicated patients during knee surgery. METHODS: 41 patients scheduled for elective knee surgery were investigated in a prospective, double-blind, and randomised design. For in

Platelet activity (malondialdehyde production) after orthopaedic surgery: the effect of aspirin.

Using a biochemical assay of the prostaglandin endoperoxide metabolite malondialdehyde, platelet activity was evaluated in 147 patients who were undergoing orthopaedic surgical procedures. Assays were performed before and after operation and the results were correlated with the type of operation, the sex of the patient, and the use of aspirin as an antiplatelet-aggregation agent. Postoperatively, statistically significant elevations of platelet activity were found in patients who were not taking aspirin, while highly statistically significant suppressions of this activity were found after tota

Association of preoperative cognitive frailty with postoperative complications in older patients under general anesthesia: a prospective cohort study.

BACKGROUND: Cognitive frailty (CF) is characterized by the coexistence of physical frailty and cognitive impairment, and it is associated with adverse health outcomes. Older adults are particularly vulnerable to CF due to factors such as age-related brain changes and the presence of comorbidities. OBJECTIVE: To investigate the effect of preoperative CF on postoperative complications in older patients. METHODS: This prospective cohort study was conducted among 253 patients aged 60-85 years, who underwent elective orthopedic and abdominal surgery (with a postoperative hospital stay of ≥ 3 days

Reduction in Mortality following Elective Major Hip and Knee Surgery: A Systematic Review and Meta-Analysis.

BACKGROUND: Systematic reviews reporting time trends in mortality following major orthopaedic surgery are few and have limitations. They reported on only a fraction (< 15%) of the available data and did not investigate potential causes of the reduction in mortality. METHODS: We searched PubMed for randomized trials and observational studies, published between 1950 and 2016, reporting on mortality within 3 months of elective total hip and knee replacement (THR/TKR). Mortality risks were estimated for each 5-year interval using a Poisson regression model and presented by study design and mode o

Fifteen-minute consultation: Modern-day art and science of managing cerebral palsy.

While there remains limited intervention to address the damage to the developing brain, current multidisciplinary management of cerebral palsy (CP) needs to minimise the impact of secondary musculoskeletal complications. A focus on comorbidities to maximise function for activity and participation by supporting the child and family in their environment is required. Comprehensive clinical guidance was published by National Institute for Health and Care Excellence (NICE) earlier this year. This article aims to provide a practical clinical approach to the child and family based on:(1) art: empathy

Minimally Invasive Achilles Tendon Stripping for the Management of Tendinopathy of the Main Body of the Achilles Tendon.

Achilles tendinopathy is a common cause of disability. New nerves fibers grow from the paratenon into the Achilles tendon, and they could play a central role in the development of pain. We report the results of minimally invasive Achilles tendon stripping for Achilles tendinopathy in 47 active patients. The Victorian Institute of Sports Assessment-Achilles questionnaire score improved from 53.8 preoperatively to 85.3 postoperatively (p < .001). After a mean follow-up period of 40.5 months, 41 patients had resumed sporting activities at an average of 3.5 months postoperatively. A sural nerve in

Biology of anterior cruciate ligament injury and repair: Kappa delta ann doner vaughn award paper 2013.

Anterior cruciate ligament (ACL) injuries are currently treated by removing the injured ligament and replacing it with a tendon graft. Recent studies have examined alternative treatment methods, including repair and regeneration of the injured ligament. In order to make such an approach feasible, a basic understanding of ACL biology and its response to injury is needed. Identification of obstacles to native ACL healing can then be identified and potentially resolved using tissue engineering strategies-first, with in vitro screening assays, and then with in vivo models of efficacy and safety. High-grade lumbosacral spondylolisthesis reduction and fusion in children using transsacral rod fixation.

PURPOSE: There is no consensus on how to treat surgically high-dysplastic developmental spondylolisthesis in children and adolescents. Although reducing spinal deformity seems mandatory, the issue of surgical reduction versus in situ fusion remains controversial. METHODS: The files of 12 consecutive patients surgically treated for a grade 3 or 4 spondylolisthesis were reviewed. The treatment consisted in L4 to sacrum reduction and fusion by posterior approach. The reduction of lumbopelvic imbalance was made intraoperatively using a trans-sacral rod fixation technique. RESULTS: Mean preoperat

A reconstruction with extracorporeal irradiated autograft in osteosarcoma around the knee.

BACKGROUND AND OBJECTIVES: An extracorporeal irradiated (ECI) autograft was first reported in Israel in 1968 for the treatment of primary malignant bone tumor. However, subsequent reports covered mostly short-term follow up results, and the reported results of reconstruction vary from study to study. We analyze the long-term follow up results of reconstruction using an extracorporeal irradiated (ECI) autograft in osteosarcoma around the knee. METHODS: We retrospectively reviewed 23 osteosarcoma patients who underwent reconstruction with ECI between December 1995 and April 2005. The 15 males a

Outcome of skin graft versus flap surgery in the salvage of the exposed achilles tendon in diabetics versus nondiabetics.

BACKGROUND: Achilles tendon wounds pose a reconstructive dilemma because of the tendon's functional importance and the paucity of soft tissue surrounding the ankle. The currently accepted treatment is to repair the wound with a flap (local, pedicled, or free). In this article, the authors examine whether skin graft coverage of the exposed Achilles tendon is a viable option for reconstruction and whether the comorbidity of diabetes affects the outcome. METHODS: Forty-five consecutive patients presenting with wounds involving the Achilles tendon in 49 limbs were retrospectively evaluated from t
