Capnocytophaga spp. involvement in bone infections: a review.

Capnocytophaga are commensal gliding bacteria that are isolated from human and animal oral flora and are responsible for infections both in immunocompromised and immunocompetent hosts. Accumulation of microbial plaque, loss of collagen attachment, and alveolar bone resorption around the tooth can lead to local Capnocytophaga spp. bone infections. These capnophilic bacteria, from oral sources or following domestic animal bites, are also causative agents of bacteraemia and systemic infections as well as osteomyelitis, septic arthritis, and infections on implants and devices. The present literatu…

Brodie's abscess of medial distal femoral condyle after a thorn prick: rare clinical presentation.

Thorn prick injuries are generally conceded frivolous and rarely demand medical attention. Howbeit deep seated injuries are well described in the literature. We presented a case of thorn prick injury to the knee that manifested as Brodie's abscess of the medial distal femoral condyle and synovitis. Magnetic resonance imaging (MRI) and ultrasonography could only construe the affliction but not spot the thorn. Arthrotomy was undertaken for exploration and debridement. Empirical therapy with initial systemic Cefotaxime and subsequent Ofloxacin worked well after the surgery. Deep seated injuries b

Atypical presentations of pediatric actinomycosis: report of a case.

It is well recognized that cervicofacial actinomycosis is rare in children, especially at maxilla. Actinomycosis involving the maxilla usually is seen as a localized intraoral infection in contrast to classical cervicofacial actinomycosis. In this article, we describe an 8-year-old patient who had actinomycosis involving the bone at the left maxillary lateral incisor region. The diagnosis was based on histologic report because of location and development of the lesion with unusual history. The treatment of choice was removal of the soft and hard tissues with concomitant prolonged penicillin ad

The SAPHO syndrome.

The occurrence of musculoskeletal manifestations (including synovitis, chest wall arthro-osteitis and multifocal aseptic osteomyelitis) in association with severe acne, palmoplantar pustulosis and perhaps with some presentations of psoriasis, have been described by many authors in the past 30 years. These different multifaceted descriptions have been designated by a variety of terms. More recently, a possible link between these conditions and spondarthritides has also been underlined by a slightly increased prevalence of HLA-B27 and occasional occurrences of sacroiliitis, chronic inflammatory …

The sub-deltoid approach to the metaphyseal region of the humerus.

In a new surgical approach to the proximal part of the humerus, used in eighteen patients, the distal part of the tendon of the deltoid muscle was divided and the whole muscle was lifted up to give an excellent view of the proximal one-third of the diaphysis, the metaphysis, the surgical neck, and both tuberosities of the humerus. The exposure allowed visualization and protection of the axillary nerve. Postoperatively the deltoid remained functional and only the preoperative condition of the joint seemed to prevent full recovery of function of the muscles in a few patients.

Right Antibiotic, Right Discharge: Ethical Tensions Between Antimicrobial Stewardship and Social Vulnerability.

In a case of a patient with postoperative septic arthritis and hardware-associated osteomyelitis due to methicillin-susceptible Staphylococcus aureus that is susceptible to multiple oral agents, the clinician must navigate not only a treatment decision between oral and intravenous antibiotics but also the ethical tension between evidence-based practice and the patient's social instability. We examined 3 interrelated ethical conflicts: balancing clinician-patient relationship with organizational priorities, weighing the benefits and harms of competing treatment decisions, and considering how pa…

Unique Application of the Jaw-in-a-Day Technique in a Case of Trauma.

Mandibular reconstruction with a fibula free flap is often used for advanced oral malignancies, osteonecrosis, or benign tumors to restore function and appearance after segmental mandibulectomy. The "Jaw in a Day" (JIAD) technique enables immediate restoration of form and function with a fibula free flap, implants, and a temporary prosthesis. This case reports the unique use of the JIAD technique in a young female patient with a traumatic mandibular nonunion fracture and chronic osteomyelitis, demonstrating its efficacy in significantly enhancing her quality of life. Laryngoscope, 135:2331-233…

Brodie's abscess of the calcaneus due to Serratia marcescens.

Brodie's abscess is a subacute or chronic osteomyelitis characterised by an intraosseous abscess. It may present months to years after the inciting event. Staphylococcus aureus is the most common causative organism of Brodie's abscess, while Gram-negative bacteria are uncommon causative organisms. A combination of culture-directed antibiotics and surgical debridement is key to successful management. This case report describes a patient with a history of minor trauma preceding the development of Brodie's abscess of the calcaneus caused by Serratia marcescens This was managed successfully with s…
