Social adjustment in dysthymia, double depression and episodic major depression.

We contrasted the overall social functioning of pure dysthymics, double depressives, episodic major depressives and normal controls using both interview and self-report measures of social functioning and depression. In addition, we used hierarchical multiple regression to assess the differential impact of several variables (comorbid personality, anxiety and substance use disorders, life stress, duration of dysthymia and severity of depressive symptomatology) on social functioning in the dysthymics and double depressives. Participants included 41 outpatients with early-onset dysthymia alone, 56

A comparison of two structured diagnostic interviews: CIDI and SCAN.

The relationship between and the inter-rater reliability of the Composite International Diagnostic Interview (CIDI) and the Schedules for Clinical Assessment in Neuropsychiatry (SCAN) for anxiety and depressive disorders were explored. The CIDI and the SCAN were administered by trained interviewers in counterbalanced order. A subsample of interviews was observed to determine the inter-rater reliability of the instruments. Subjects were 101 patients accepted for treatment at an Anxiety Disorders Clinic; 29 of the 101 patients participated in the inter-rater reliability study. Concordance betwee

Differential effects of trauma on spouses of traumatized households.

The responses of 60 households subjected to varying degrees of traumatic events were examined. Specific attention was given to the differential effect that traumatic events have on spouses, especially in terms of general anxiety, state anxiety, trait anxiety, depression, and other symptoms related to trauma. The findings showed that degree of traumatization was proportional to number and intensity of stress related symptoms. Mothers, however, manifested depression and anxiety levels twice as high as fathers. The validity of these results was discussed in terms of cultural beliefs and practices

Mild traumatic brain injury does not produce post-traumatic stress disorder.

It has been widely assumed that patients who sustain mild traumatic brain injury (MTBI) or post-concussive syndrome develop post-traumatic stress disorder (PTSD) in response to their cognitive difficulties, diminished coping skills, or other losses. This study examined 70 patients who had previously been diagnosed as having either PTSD or MTBI. Each patient was asked to provide a highly detailed chronological history of the events which preceded, followed, and occurred during the traumatic event, to indicate whether they were rendered unconscious or had amnesia for the event, and to describe t

The Behavior Problem Inventory: a further replication of its factor structure.

The Behavior Problem Inventory was administered to a random sample of people living in a state school. The scores from all items and from self-injury items only were factor analysed. The three scales of the Behavior Problem Inventory were highly internally consistent. Factor analysis of all the items showed some similarities to previous studies, and factor analysis of the self-injury items showed a very close correspondence to two previous studies. The results are discussed in terms of the design of this instrument, the possible multi-factorial nature of self-injury and future research on the

Compulsions in adults with mental retardation: prevalence, phenomenology, and comorbidity with stereotypy and self-injury.

A variety of conceptual similarities between compulsions seen in individuals with obsessive compulsive disorder and stereotypy and self-injury seen in individuals with mental retardation led us to investigate the prevalence, phenomenology, and comorbidity of compulsions in adults with severe or profound mental retardation. We developed simple assessment screening instruments for stereotypy and self-injury and used Gedye's Compulsive Behavior Checklist and found acceptable levels of reliability, stability, and validity for each instrument. Prevalences were as follows: stereotypy: 60.9%; self-in

The reliability of depression diagnosis in chronic low back pain. A pilot study.

The variability in estimated rates of major depressive disorder (MDD) in chronic pain samples may be accounted for by sample and methodological differences. Most studies rely on a single measure of depression and lack independent or repeated measures. This study investigated the prevalence of psychiatric disorders in a convenience sample of 18 patients disabled by low back pain (LBP) referred to, evaluated, and treated in a comprehensive pain rehabilitation program specifically designed to restore work readiness and return to work. Subjects had one or more spinal conditions, had work-related b

Training in evaluation of expressed emotion using the Japanese version of the Camberwell Family Interview.

To evaluate the effect of training in rating expressed emotion (EE) using the Japanese version of the Camberwell Family Interview (CFI), interrater reliability between a certified rater and a trainee was examined. The material was 65 CFI interviews with the families of 46 schizophrenic patients. The two raters independently rated EE status, critical comments (CC), hostility (H), emotional overinvolvement (EOI), warmth (W) and positive remarks (PR). The interrater reliability was satisfactory with regard to EE status, CC, H, EOI and PR. In the category of W, however, the results were unsatisfac

Evaluation of the Eyberg Child Behavior Inventory as a predictor of disruptive behavior during an initial pediatric dental examination.

The purpose of this study was to investigate relationships between the Eyberg Child Behavior Inventory (ECBI), a parent rating scale of children's behaviors at home, and children's disruptive behaviors during an initial dental visit. Behaviors of 18 Headstart children, 40 to 55 months of age, undergoing an initial exam were observed. Four possible negative behaviors based upon the North Carolina Behavior Rating Scale (NCBRS), were recorded by two observers for each subject during five phases of an initial dental visit and statistically compared to the total ECBI score and individual answers wi

Domains of the Childhood Autism Rating Scale: relevance for diagnosis and treatment.

The Childhood Autism Rating Scale (CARS) was factor analyzed. Three factors emerged: Social Impairment (SI), Negative Emotionality (NE), and Distorted Sensory Response (DSR). Unit-weight factor scales showed moderate-to-good internal consistency. Cross-sectional analyses demonstrated that autistic (AUT) subjects were distinguished from subjects with pervasive developmental disorders (PDD) and nonpervasive developmental disorders (NPDD) by higher scores on SI. An SI cutoff score of 26 classified individuals as autistic vs. nonautistic with 78% accuracy. Longitudinal analyses showed that DSR was

Do parental concerns predict a diagnosis of attention-deficit hyperactivity disorder?

Parents' concerns for their children's behavior were investigated to predict a diagnosis of attention-deficit hyperactivity disorder (ADHD) in this retrospective comparative study of 245 children, aged 4 through 15 years (mean = 8.1 years), consecutively referred for comprehensive pediatric evaluation of school problems between 1981 and 1992. Concerns identified by parents were categorized (inattention, impulsivity, overactivity) and compared to children's final diagnoses of ADHD to measure their sensitivity, specificity, and predictive value. For 92% of subjects, significant school-related pr

Exposure with and without cognitive therapy for generalized social phobia: effects of individual and group treatment.

Patients with generalized social phobia (n = 73) were randomly allocated to two treatment modalities: (1) group or (2) individual treatment, and to three treatment packages: (1) two blocks of exposure in vivo (2) a block cognitive therapy followed by a block exposure in vivo, or (3) two blocks cognitive-behavioural treatment in which both strategies were integrated from the start. All treatments consisted of 16 sessions, given in two treatment blocks of 4 weeks each, separated by a no-treatment phase of 4 weeks. Self-report assessments were held before and after the treatment blocks and at 3-m

The criterion-related validity of the Childhood Autism Rating Scale and the Autism Behavior Checklist.

The present study sought to examine the relationship between two popular instruments for screening autistic children: the Childhood Autism Rating Scale (CARS) and the Autism Behavior Checklist (ABC). The sample included 77 individuals. The records of 48 subjects contained firm diagnoses of autism, while the suggestion of autism was found in the remaining records. Correlations between the two scales ranged from -.16 to .73 (median = .39). The validity coefficient between the two total scores was .67. Using cutoff scores recommended by the authors, an analysis of the sensitivity of the two instr

Defensiveness, trait anxiety, and Epstein-Barr viral capsid antigen antibody titers in healthy college students.

The relationship of individual differences in repressive coping styles with differences in antibody titer to Epstein-Barr viral capsid antigen (EBV-VCA) were investigated in a normal, healthy college population made up of people previously exposed to EBV. Each of 54 1st-year undergraduates completed a battery of physical-status questions and items pertaining to potential behavioral immunomodulatory confounds, along with the Taylor Manifest Anxiety Scale (T-MAS) and the Marlowe-Crowne Social Desirability Scale (MC-SDS). Ss reporting high and middle levels of anxiety had higher antibody titers t

Bromocriptine and cocaine cue reactivity in cocaine-dependent patients.

Based on previous reports that bromocriptine, a postsynaptic dopamine agonist, reduced cocaine craving and prevented relapse in cocaine-dependent subjects, effects of the drug were evaluated in 20 cocaine-dependent males in an inpatient drug rehabilitation programme. The subjective and physiologic effects of exposure to both cocaine-associated and neutral stimuli, presented using videotapes, were measured at one-week intervals. Between laboratory sessions subjects received either bromocriptine (1.25 mg bid) or a matched placebo, administered in double-blind fashion. Compared with the neutral v

[Therapy and course of alcohol dependence. Effects on the patient and family].

100 male and female patients, aged 20 to 60 years with the diagnosis alcohol-addiction (ICD 9 No 303.0) took part in a longitudinal treatment evaluation study. Consecutive admissions from May '83 until July '85 were assessed. All patients participated in a 6-week inpatient group therapy and a 6-week outpatient treatment for alcohol-dependent patients at the Psychiatric University Hospital in Munich. Assessments were performed at the following times: 1. on admission, 2. at the end of inpatient therapy, 3. at the end of outpatient therapy, 4. at a 6-month follow-up and 5. at an 18-month follow-u

Artifacts in "limited prospective" designs? Testing confounding effects on response behaviour of women prior to breast surgery.

Data from a 'limited prospective' study on life events and breast cancer were tested whether they were biased by knowledge of the prospective diagnosis or by related mood states. Data were gathered by semi-structured and tape-recorded interviews. Event classifications were made according to Brown and Harris' Life Events and Difficulties Scale, an interviewer-based rating method. Ratings were performed before the interviewer learned the final diagnosis. Among the women coming to hospital with a suspicious breast lump, 33 had a malignant and 59 had a benign diagnosis. For comparison for mood eff

Multivariate evaluation of health attitudes and behaviors: development and validation of a method for health promotion research.

Many self-reported measures of health status, attitudes, and knowledge used by clinicians and researchers suffer from a variety of shortcomings, including limited empirical justification, excessive complexity, assessments of nonmodifiable historic or hereditary factors, and limited utility for public health program planning. The Health Attitudes and Behavior Scale is an instrument designed to overcome many of these shortcomings and direct public health professionals toward more efficacious interventions. One hundred sixty-four subjects responded to true-false and Likert scale items related to

Social phobia: a comparative clinical study.

Eighty-seven people with the symptom of social phobia were compared with 57 people with the symptom of agoraphobia to determine whether these symptoms were part of distinct syndromes. Comparisons were made on demographic, clinical and questionnaire data. Significant differences were found on important variables. The social phobics were younger and more often male, unmarried, and from social classes I and II. The pattern of phobic situations was different in the two groups and so was the pattern of autonomic symptoms experienced in these situations. Symptoms visible to others were more frequent
