Understanding barriers to immunization among women with zero-dose or under-immunized children in an urban area in Cameroon.

BACKGROUND: Zero-dose children represent a major public health challenge in low-income countries. In 2024, the Nkolndongo Health District in Yaoundé, Cameroon, recorded the highest number of zero-dose children in the country, according to the District Health Information Software 2 (DHIS2) data. OBJECTIVE: This study aimed to identify and describe the main reasons for non-vaccination and the obstacles to vaccination among zero-dose and under-immunized children in the Ndolndongo Health District, Yaoundé. METHODS: A cross-sectional study was conducted from December 2024 to July 2025. The study

A qualitative journey mapping study of prenatal care experiences among rural pregnant women in Northeast China.

BACKGROUND: The participation rate of prenatal healthcare in China has increased slowly, and the imbalances and inadequacies in the development of maternal and child health between urban and rural areas are particularly prominent. A key reason for this is that a comprehensive understanding of rural women's utilization of prenatal healthcare services and their needs has not been fully established, thus failing to fully leverage pregnant women's agency in prenatal healthcare. The study aimed to identify the multi-dimensional needs of rural women in the process of utilizing prenatal healthcare se

Comparison of Surveillance Strategies to Assess Cesarean Delivery Surgical Site Infections: A Quality Improvement Initiative.

Cesarean delivery-associated surgical site infections (CD-SSIs) significantly increase maternal morbidity but have not been systematically reported. Surveillance strategies have been inconsistent in assessing CD-SSIs. This study aimed to compare the different methods of assessing CD-SSI incidence. Two surveillance strategies have been evaluated: electronic medical record (EMR) review and postpartum phone call. The difference in the CD-SSI incidence assessed through EMR review (4.7%) and postpartum phone call (8.9%) was statistically significant (P < 0.001); this suggests that EMR review alone

Structural heat adaptation and education in a rural setting of Pakistan: protocol for a randomised controlled trial.

INTRODUCTION: Pakistan is one of the countries most affected by climate change, where peak temperatures in certain areas frequently exceed 40°C. Despite evidence that vernacular designs and behavioural adjustments can reduce indoor temperatures, heat-adaptation efforts remain concentrated in urban or high-resource settings, with limited evidence for vulnerable rural populations in low- and middle-income countries, including Pakistan. METHODS AND ANALYSIS: This trial aims to evaluate the effectiveness of a comprehensive Resilience and Heat Adaptation Bundle (ReHAB) that integrates behavioural

When ART is not a magic bullet: Tailored care for vulnerable people living with hiv in the era of treat-all in Shinyanga, Tanzania.

Community health workers (CHWs) play a crucial role in supporting retention in HIV care in Tanzania under the World Health Organization's Treat-All policy. While the WHO promotes the differentiated service delivery (DSD) model for stable clients, not all people living with HIV (PLHIV) can achieve sustained viral suppression independently. Using ethnographic case studies of one CHW and three vulnerable clients in Shinyanga, Tanzania (2022-2023), we examine how CHWs adapt patient-centred care in the current era of Treat-All. The findings demonstrate that treatment adherence for highly vulnerable

Exploring generational differences in the impact of stigma on mental health among people affected by leprosy in rural India: a qualitative study.

Leprosy continues to be highly stigmatised in India, resulting in the continuation of poor mental health outcomes for affected individuals and delays in seeking treatment. Whilst previous research suggests that the effectiveness of intervention strategies to reduce stigma and improve mental health can vary across generations, little is known about how leprosy-related stigma specifically impacts the mental health of different generational groups. This study, therefore, explored these generational differences in rural India. Through qualitative in-depth interviews conducted between June and July

Global Hesitancy of COVID-19 Vaccine Among Vulnerable Population From 2020 to 2023: A Systematic Review and Meta-Analysis.

Vaccination is an effective response to the COVID-19 pandemic, but vaccine hesitancy is a major challenge. This study aims to explore the pooled prevalence and factors of COVID-19 vaccine hesitancy. We searched the studies published from January 2020 to December 2023 in PubMed, Web of Science, and Embase. The studies with complete start time of the study and national information were included in the generalised additive model to explore the factors affecting the COVID-19 vaccine hesitancy rate by calculating OR and 95%CI. A total of 629 studies were included. The pooled global hesitancy rates

A Randomized Control Trial of Two Interventions Compared with Usual Care for Increasing Cervical Cancer Screening among Women Living in the Rural Midwest.

BACKGROUND: Lower cervical cancer screening rates are associated with higher cervical cancer mortality among women living in rural compared with urban areas (defined by rural-urban community codes). The study purpose was to examine the effectiveness of a mailed digital video disc (DVD) versus DVD plus patient navigation (PN) versus usual care (UC) on increasing the percentage of rural women up to date (UTD) with cervical cancer screening guidelines. METHODS: Rural women (ages 50-74 years) who were not UTD for cervical cancer screening (n = 553) were consented and randomized 2:2:1 (DVD, DVD +

Cleaning and Disinfection Practices of Reused Alcohol-Based Hand Rub Containers in Health Care Settings: Evidence from Five Rural Districts in Uganda.

Local alcohol-based hand rub (ABHR) production systems in low-resource settings, such as in health care facilities (HCFs) in low- and middle-income countries, frequently reuse containers for storing and dispensing ABHR. Cleaning/disinfection (C/D) of ABHR containers is necessary to safely reuse them and is an integral part of the WHO's guidelines on local ABHR production. However, HCFs may not be aware of the need for C/D; combined with suboptimal implementation, this poses a risk of contamination of ABHR. As part of district-led ABHR production in HCFs in five rural districts in Uganda, we de

The Impact of Coronavirus Disease 2019 (COVID-19) on Vulnerable Communities: What the Clinician Should Know.

The COVID-19 pandemic unmasked striking health disparities within the US healthcare system and significantly impacted vulnerable populations. While much attention on vulnerability has been directed toward older individuals with underlying medical conditions, key populations-including the uninsured, racial and ethnic minorities, and pregnant individuals-were considerably affected and require intervention. This review examines the barriers these populations encountered and outlines how clinicians can engage at the provider, practice, community, and national levels to advance health equity and im

Staying afloat on a realm of shifting sands: carers navigating palliative care in their rural settings.

INTRODUCTION: Carer roles in palliative care are complex and have many location-based issues, including for people in rural areas. Meaningful support for carers in rural areas needs to be informed by carers' actual experiences rather than relying on routine data collection. To embrace the complexity of carer experiences, we framed caregiving in palliative care as a journey that involves actions and needs to be navigated. We deliberately chose to use the verb 'navigate' as an action-based metaphor. Thus, we positioned ourselves to view carers' involvement in palliative care in rural areas as be

Implementation of a Technology-Based Mobile Obstetric Referral Emergency System (MORES): Qualitative Assessment of Health Workers in Rural Liberia.

BACKGROUND: Maternal mortality remains a persistent challenge in low- and middle-income countries, where evidence-based interventions of obstetric triage and prehospital communication remain sparse. There is limited implementation evidence for technology-based approaches to improve obstetric care in such contexts. Liberia struggles with maternal mortality, particularly in rural areas where deaths are attributable to delays from absent triage and interfacility communication. We implemented a Mobile Obstetric Referral Emergency System (MORES) in rural Bong County to improve prehospital transfer,

Migraine among adults with atopic dermatitis: a cross-sectional study in the All of Us research programme.

Atopic dermatitis (AD) has been previously associated with migraine headaches in paediatric and adolescent populations, though there is less evidence for this relationship among adults. In this cross-sectional study, we investigated the association between AD and migraine among a cohort of US adults in the All of Us research programme. After controlling for common comorbidities, we found that adults with AD were 89% more likely to have a diagnosis of migraine (OR = 1·89, P < 0·001).

Effect of Metformin Use on Vitamin B12 Deficiency Over Time (EMBER): A Real-World Evidence Database Study.

OBJECTIVE: To examine the extent to which metformin increases the risk of vitamin B12 deficiency and borderline deficiency over time in participants with type 2 diabetes mellitus (T2DM). METHODS: Using the All of Us database, adults aged ≥18 years with T2DM and a documented history of metformin use were included for the evaluation of B12 deficiency. Those with B12 deficiency before metformin use were excluded. Adjusted logistic regression models were used to evaluate the association between metformin use and long-term metformin use (≥4 years) and the risk of B12 deficiency. We conducted a sub

Maternal Mortality in a Rural District of Pakistan and Contributing Factors.

INTRODUCTION: Pakistan is among the ten countries that account for 60% of global maternal mortality. Lack of accurate data on maternal mortality and a complex interrelation of access and quality of healthcare services, healthcare delivery system, and socio-economic and demographic factors contribute significantly to inadequate progress in reducing maternal mortality. MATERIAL AND METHODS: A population-based prospective cohort study was conducted in a rural district of Pakistan using data obtained from an enhanced surveillance system. A total of 7572 pregnancies and their outcomes were recorde

Progress Toward Poliomyelitis Eradication - Worldwide, January 2021-March 2023.

Since the World Health Assembly established the Global Polio Eradication Initiative (GPEI) in 1988, two of the three wild poliovirus (WPV) serotypes (types 2 and 3) have been eradicated, and global WPV cases have decreased by more than 99.9%. Afghanistan and Pakistan remain the only countries where indigenous WPV type 1 (WPV1) transmission has not been interrupted. This report summarizes progress toward global polio eradication during January 1, 2021-March 31, 2023, and updates previous reports (1,2). In 2022, Afghanistan and Pakistan reported 22 WPV1 cases, compared with five in 2021; as of M

Association of step counts over time with the risk of chronic disease in the All of Us Research Program.

The association between physical activity and human disease has not been examined using commercial devices linked to electronic health records. Using the electronic health records data from the All of Us Research Program, we show that step count volumes as captured by participants' own Fitbit devices were associated with risk of chronic disease across the entire human phenome. Of the 6,042 participants included in the study, 73% were female, 84% were white and 71% had a college degree, and participants had a median age of 56.7 (interquartile range 41.5-67.6) years and body mass index of 28.1 (

Association of regional socioeconomic deprivation and rurality with global developmental delay in early childhood: Data from mandatory school entry examinations in Germany.

BACKGROUND: From birth to young adulthood, health and development of young people are strongly linked to their living situation, including their family's socioeconomic position (SEP) and living environment. The impact of regional characteristics on development in early childhood beyond family SEP has been rarely investigated. This study aimed to identify regional predictors of global developmental delay at school entry taking family SEP into consideration. METHOD: We used representative, population-based data from mandatory school entry examinations of the German federal state of Brandenburg
