Factors influencing practices for chronic prostatitis: a nationwide survey of urologists in South Korea.

BACKGROUND: We investigated the influence of physicians' attitudes on the treatment of chronic prostatitis, and attempted to correlate demographic and professional characteristics with patterns of therapy regarding chronic prostatitis. METHODS: Probability samples were drawn from the Korean Urological Association Registry of Physicians, and a random sample of 850 Korean urologists were asked to complete a questionnaire which explored practice characteristics, attitudes, and diagnostic and treatment strategies for the management of chronic prostatitis. The returned questionnaires were statisti

Equivocation in patients' decisional preference about life-sustaining treatments.

OBJECTIVE: Communication is essential for achieving shared decision-making. We focus on how patients design decisional preferences to allow or decline life-sustaining treatments (LST). We examine "equivocation", a practice through which patients refer to two preferences in response to the physician asking for only one. METHODS: Conversation analysis of 89 conversations with physicians in which patients allow or decline LST. RESULTS: We identified three types of decisional preference formats. In "unequivocal preferences", patients express a clear-cut preference towards or against an intervent

Awareness of indications for device therapy among a broad range of physicians: a survey study.

AIMS: Cardiac resynchronization therapy (CRT) and primary prophylactic implantable cardioverter-defibrillators (ICDs) are underutilized in heart failure (HF). This may originate from an unawareness of device benefits and indications among physicians responsible for HF care and referral. We aimed to describe the awareness of indications for device therapy in a generalized sample of Swedish physicians. METHODS AND RESULTS: A randomly selected sample of Swedish physicians specializing in cardiology, internal medicine, and family medicine and interns (5% of eligible physicians, n = 519) was invit

Comparison of the appropriate use of antibiotics based on clinical guidelines between physicians in-training versus practicing physicians.

The inappropriate antibiotic can lead to serious negative effects on health. This has been the cause of emergence of multidrug resistant bacteria and the need of surveillance of antibiotics in the inpatient setting. An adequate knowledge on which and when prescribing antibiotics is essential to avoid these issues. Because of this problems, guidelines have been developed to educate and control the misuse and abuse of antibiotics and improve clinical outcomes. We evaluated the medical knowledge, medical trends, and the effectiveness of professional interventions among Puerto Rico physicians in p

[Abuse of caesarean delivery in Brazil: geographic dimensions of a medical aberration].

"Brazil is the world champion of caesareans." This prowess points out a serious public health problem. While the increase in the percentage of caesareans among all deliveries is an international phenomenon, it is highest by far in Brazil, where caesareans account for nearly 40% of births, that is, three times the maximum recommended by WHO. This abusive practice appears pandemic and has spread in recent years throughout the entire country, although at divergent rates. It weighs heavily on Brazil's national health insurance fund. After a critical examination of the information sources related t

Delivery of HIV prevention counseling by physicians at HIV medical care settings in 4 US cities.

OBJECTIVES: We investigated physicians' delivery of HIV prevention counseling to newly diagnosed and established HIV-positive patients. METHODS: A questionnaire was developed and mailed to 417 HIV physicians in 4 US cities. RESULTS: Overall, rates of counseling on the part of physicians were low. Physicians reported counseling newly diagnosed patients more than established patients. Factors associated with increased counseling included having sufficient time with patients and familiarity with treatment guidelines. Physicians who perceived their patients to have mental health and substance ab

Engaging Dialysis Teams in Shared Decision-Making Conversations With Patients to Improve Rates of Kidney Transplantation.

RATIONALE & OBJECTIVE: Increasing access to kidney transplantation requires patients to be active on the waiting list and empowered to consider donor offers that are consistent with their preferences. We sought to develop and refine patient-centered communication tools for shared decision-making conversations within dialysis facilities about active status on the waiting list, consent for hepatitis C and high-kidney donor profile index organ offers, and living donation. Conversations may help patients improve their probability of kidney transplantation. STUDY DESIGN: A qualitative study conven

Recommendations for optimal use of imaging studies to clinically stage mediastinal lymph nodes in non-small-cell lung cancer patients.

Appropriate clinical staging of mediastinal lymph nodes in non-small-cell lung cancer (NSCLC) patients has important therapeutic and prognostic implications. Because of the wide variations in practice patterns among community and academic physicians, we reviewed the literature so that we could provide evidence-based recommendations on the use of imaging studies in the pretreatment clinical staging of NSCLC patients. We concluded that the most sensitive and accurate method of noninvasive mediastinal nodal staging is a positron emission tomography/computed tomography fusion scan; we believe this

In many voices: exploring end-of-life care through patient, caregiver and physician narratives.

As the focus on end-of-life care intensifies, so too does the need to better understand the experiences of patients, caregivers and physicians. Delivering empathetic care requires a shared understanding of illness and its meaning and impact. The narratives of patients, caregivers and physicians each offer a distinct perspective on clinical experiences, yet comparative research is uncommon. This study compares written narratives about end-of-life illness and care by these groups. We created an archive of 332 first-person written narratives about end of life (patient=65, caregiver=156, physician=

Association of Primary Care Practice Location and Ownership With the Provision of Low-Value Care in the United States.

IMPORTANCE: Hospital-employed physicians provide primary care within the hospital or within community-based office practices. Yet, little is understood regarding the influence of hospital location and ownership on the delivery of low-value care. OBJECTIVE: To assess the association of hospital location and hospital ownership with the provision of low-value health services. DESIGN, SETTING, AND PARTICIPANTS: This study compared low-value service use after primary care visits at hospital-based outpatient practices from January 1, 1997, to December 31, 2011, vs community-based office practices

Referral patterns, lesion prevalence, and patient care parameters in a clinical oral pathology practice.

OBJECTIVE: The purpose of this study was to determine the following parameters in a referral-based private practice oral and maxillofacial pathology clinic: (1) sources of clinical referrals; (2) types of problems referred; and (3) clinical effectiveness of treatment. STUDY DESIGN: Clinical charts were reviewed for a cohort of 362 patients seen over a 2 1/2-year period (1993-1995). From these charts, we determined the source of referral and the final diagnosis for each patient. In addition, 50 patients were randomly selected and surveyed by telephone; each was asked a series of questions to d

Analysis of "Local Guide" Google Reviews for Orthopaedic Spine Surgeons.

This study aimed to investigate the content and trends of online reviews of orthopaedic spine surgeons in Florida, as left by "Local Guide" reviewers on Google Maps. The data collected were categorized and analyzed to identify patterns related to physician age, academic status, gender, practice location, and the nature of comments left by reviewers. The results showed that higher ratings were associated with comments related to shorter wait times, a competent medical staff, the physician's knowledge, and pain improvement. On the other hand, lower ratings were associated with comments related t

Practices and perceptions of 5-aminosalicylic acid use in Crohn's disease: a nationwide survey of physicians in Korea by KASID Guidelines Taskforce Team.

BACKGROUND/AIMS: Despite international guidelines recommending against the use of 5-aminosalicylic acid (5-ASA) for Crohn's disease (CD), it remains widely prescribed. This study aimed to investigate current patterns of 5-ASA use and physicians' perceptions of its efficacy among Korean specialists. METHODS: A nationwide online survey was conducted in August 2025 targeting Korean gastroenterologists and colorectal surgeons managing inflammatory bowel disease. The questionnaire included 19 items addressing prescribing behaviors, perceived efficacy, and clinical decision-making regarding 5-ASA i

The role of medical students' stereotype of physicians in sex education.

The way a medical student perceives the sexual attitudes of physicians may affect what is learned from medical school sex education and the manner in which such learning is applied later with patients. Medical students' personal sexual attitudes, their projections of physicians' and patients' attitudes, and the actual attitudes of a random sample of physicians were compared by analyzing data from a modified version of the Sex Knowledge and Attitude Test, as completed by 136 sophomore and 43 senior (control group) medical students and 91 practicing physicians. The medical students believed phys

Office-based Neurophysiological Assessments for Diagnosis and Prognosis of Concussion.

Patients with concussion often receive insufficient and/or ineffective diagnostic workups. A limiting factor for prompt and accurate diagnosis is the scarcity of practical and effective concussion diagnostics that can be used by private-practice physicians. This gap in the diagnostic process can delay the implementation of crucial interventions. The absence of an affirmative concussion diagnosis leaves patients with few options when follow-up treatment would otherwise be indicated, potentially leading to worsening symptoms, prolonged recovery, and a higher likelihood of developing long-term co

Understanding clinician retention: Insights from a national anesthesia practice group.

BACKGROUND: Anesthesia workforce shortages are intensifying, with facility-reported staffing shortages reaching 78% by 2023. Retention is thus critical for anesthesia practices, yet its drivers remain poorly characterized across career stages and clinician types. We hypothesized that retention would be associated with traditional satisfiers: higher compensation, lighter workload, shorter (daytime) hours, and desirable case mix. We tested this hypothesis using data from a large national anesthesia practice. METHODS: We conducted a database cohort analysis of 6273 anesthesia clinicians (2935 ph
