[The hormonal regulation of thymus activity in threatened abortion].

It is stated that in women with the abortion threat at any term of gestation (I, II, III trimesters) the endocrine function of the thymus essentially increases. The role of hormones in the thymic activity disorder is studied. It is shown that insufficiency of the functional activity of the trophoblast ranks first in the mechanism of hormonal regulation of thymic endocrine function in nonbearing of pregnancy. The other significant reason of an essential increase in the titer of thymic serum factor (TFS) is a disorder in interconnections inside the hypophyseal-adrenal system. Determination of TF

Placental changes in fetal triploidy syndrome.

The sonographic findings in fetal triploidy syndrome include intrauterine growth retardation, hydrocephalus, oligohydramnios, and hydropic changes of the placenta. Ultrasonography can establish the diagnosis only when placental findings coexist with a fetus. Although the majority of triploid conceptions abort spontaneously in the first trimester, occasionally they will progress further, but rarely to term. Six cases are presented in which the diagnosis was suspected by early ultrasound examinations, including one case in which there was an unusually large trophoblastic cyst. Determination of t

Successful live birth following robotic-assisted management of a cesarean scar pregnancy: a promising pathway.

Cesarean scar pregnancy (CSP) is a variant of uterine ectopic pregnancy defined by full or partial implantation of the gestational sac in the scar of a previous cesarean section. The most usual recommendation is termination of pregnancy in the first trimester. In this article, we present a case of live birth after detecting CSP in early pregnancy and robotic scar reinforcement surgery, effectively relocating the pregnancy into the cavum. At 25+3 gestational weeks, an emergency caesarean section was performed. A girl weighing 800 g was delivered with APGAR 7/7/9. The percrete area of the placen

Inflammatory bowel disease presenting in pregnancy.

A case of inflammatory bowel disease (IBD) presenting in pregnancy is described. Despite previous reports of severe fulminating disease in this type of patient, this woman did well with an uncomplicated course; she responded to standard medical therapy and there were no fetal complications. IBD should not be a contraindication to pregnancy unless the disease is poorly controlled. Pregnancy does not increase the risk of relapse of IBD, but should this occur it is more likely in the first trimester or in the postpartum period. Treatment of IBD in pregnancy should be much the same as in the nonpr

Increased risk of cytomegalovirus transmission in utero during late gestation.

OBJECTIVE: To determine whether the rate of human cytomegalovirus transmission in utero is related to the gestational age at the time of maternal infection. METHODS: One hundred twenty-three pregnant women followed in our units between 1988 and 1998 were studied retrospectively. Each had developed a primary infection with cytomegalovirus evidenced by a seroconversion, confirmed by specific enzyme immunoassays. Infants were diagnosed by urine culture. RESULTS: Regardless of gestational age at the time of maternal cytomegalovirus seroconversion, the mean rate of intrauterine transmission was 5

Peripartum Cardiomyopathy Associated With Gestational Transient Thyrotoxicosis and Severe Preeclampsia: A Case Report.

Human chorionic gonadotropin (hCG) exerts a thyrotropic effect and contributes to the pathophysiology of gestational transient thyrotoxicosis (GTT), a typically benign, self-limiting form of hyperthyroidism that usually resolves by the late first or early second trimester of pregnancy, which is generally considered to have minimal obstetric or fetal consequences. However, identifying hyperthyroidism in pregnancy remains essential, as maternal and fetal complications may occur depending on the etiology and the presence of overt hyperthyroidism. Thyroid hormones significantly influence cardiovas

Safety and efficacy of manual vacuum aspiration under local anesthesia compared to general anesthesia in the surgical management of miscarriage: a retrospective cohort study.

BACKGROUND: In Japan, dilatation & curettage (D&C) has been performed under general anesthesia as a surgery for an early pregnancy miscarriage for a long time. In 2016, manual vacuum aspiration (MVA) under general anesthesia was introduced at our hospital and has been used as a surgical treatment for first-trimester pregnancy miscarriage, with its utility to date being reported here. In July 2018, our hospital introduced the MVA procedure under local anesthesia. In this study, we evaluated the efficacy and safety of MVA under general and local anesthesia in first-trimester pregnancy miscarriag

Maternal cognitive function during pregnancy in relation to hypo- and hyperthyroxinemia.

OBJECTIVE: To assess a possible relationship between maternal cognitive dysfunction during pregnancy and hypothyroxinemia, adjusted for major confounders. BACKGROUND: Thyroid dysfunction in general is associated with cognitive dysfunction. Cognitive dysfunction is common during pregnancy. DESIGN: Prospective follow-up study from 12 to 32 weeks of pregnancy. PARTICIPANTS: 2082 healthy pregnant women. MEASUREMENTS: Cognitive function, depression and sleeping problems were assessed by self-report questionnaires at 12, 22 and 32 weeks of gestation, higher scores reflecting more symptoms. FT4,

First trimester uterine, placental and yolk sac haemodynamics in pre-eclampsia and preterm labour.

BACKGROUND: We hypothesized that impaired trophoblast invasion leads to umbilicoplacental blood flow disturbances that could be detected by Doppler ultrasonography during the first trimester of the pregnancy. METHODS: After successful fresh IVF or ICSI programme, 41 of 47 enrolled subjects were followed up every 1-2 weeks between weeks 6 and 11 of gestation. Ten patients who later developed pre-eclampsia and/or preterm labour formed the study group and the control group consisted of 31 uncomplicated IVF/ICSI pregnancies. Doppler parameters of uterine, spiral, intraplacental, chorionic, umbili

Deciding the timing of aneurysm coiling and obstetric delivery in pregnant patients presenting with ruptured intracranial aneurysms: a case report and review.

The optimal treatment strategy for intracranial aneurysms (IA) in pregnancy is unclear, particularly in the third trimester of pregnancy and during the intrapartum period. We discuss a case of a ruptured IA at 39 + 3 weeks of gestation, during prelabour and review literature focusing on the order of treatment and delivery. A 4 × 6 × 3 mm aneurysm arising from the left posterior communicating segment of the internal carotid artery was embolised with multiple detachable coils, followed by a caesarean section (CS) with good maternal and foetal outcomes. Our review of the literature identified 29

Pregnancy outcomes following gestational exposure to papaverine: An observational comparative study.

AIMS: Papaverine is indicated for abdominal pain of various aetiologies. However, data on maternal and foetal safety following gestational exposure are lacking. The aim was to examine whether first trimester exposure to papaverine is associated with increased risk for major malformation and whether gestational exposure at any stage is associated with increased risk for preterm delivery, lower birthweight, small for gestational age, caesarean section (CS), lower Apgar score and perinatal death. METHODS: A retrospective comparative study consisted of pregnant women treated with papaverine betwe

Tobacco use during pregnancy.

The use of tobacco during pregnancy is the leading preventable cause of pregnancy complications and adverse birth outcomes. In high-income countries, around one in 10 pregnant women smokes tobacco, while smokeless tobacco is the primary form of tobacco used in many low- and middle-income countries. Although the risk of tobacco-related harms can be reduced substantially if mothers cease smoking in the first trimester of pregnancy, the proportion of women who successfully quit smoking during pregnancy remains modest. Psychosocial interventions are first-line treatment, with some high-quality evi

Patterns of gestational weight gain and birthweight outcomes in the Eunice Kennedy Shriver National Institute of Child Health and Human Development Fetal Growth Studies-Singletons: a prospective study.

BACKGROUND: Inadequate or excessive total gestational weight gain is associated with increased risks of small- and large-for-gestational-age births, respectively, but evidence is sparse regarding overall and trimester-specific patterns of gestational weight gain in relation to these risks. Characterizing the interrelationship between patterns of gestational weight gain across trimesters can reveal whether the trajectory of gestational weight gain in the first trimester sets the path for gestational weight gain in subsequent trimesters, thereby serving as an early marker for at-risk pregnancies

Varicella Zoster Virus Infection and Pregnancy: An Optimal Management Approach.

Varicella-zoster virus is an α-herpes virus with a double-stranded DNA genome, which causes two main clinical pictures: varicella or chickenpox and herpes zoster. Chickenpox is the primary infection, predominantly affecting children, and it presents with fever and a cutaneous eruption consisting of a vesicular, pruritic, and painful rash. Herpes zoster is a viral infection that typically develops in adulthood as a result of the reactivation of the varicella-zoster virus. If acquired during pregnancy, chickenpox may be responsible for serious complications for the mother, the fetus, or the newb

Maternal hemoglobin drop in multiple pregnancy is associated with higher gestational age at birth and birthweight.

INTRODUCTION: This retrospective study investigated the hypothesis that maternal hemoglobin (Hb) levels in twin pregnancy fall between the first and second trimesters and that higher falls are associated with higher gestational age at birth and higher birthweight (BW). MATERIAL AND METHODS: The study population was defined as pregnant women with twin pregnancies delivering two live, phenotypically normal neonates, after 24+0 weeks of gestation, between October 2009 and September 2021 at an inner London maternity unit. Maternal Hb and mean corpuscular volume (MCV), at ≤14+0 weeks of gestation

Oxygen modulates human decidual natural killer cell surface receptor expression and interactions with trophoblasts.

Decidual natural killer (dNK) cells have been shown to both promote and inhibit trophoblast behavior important for decidual remodeling in pregnancy and have a distinct phenotype compared to peripheral blood NK cells. We investigated whether different levels of oxygen tension, mimicking the physiological conditions of the decidua in early pregnancy, altered cell surface receptor expression and activity of dNK cells and their interactions with trophoblast. dNK cells were isolated from terminated first-trimester pregnancies and cultured in oxygen tensions of 3%, 10%, and 21% for 24 h. Management of Hypothyroidism in Pregnancy and Its Impact on Maternal and Perinatal Outcomes: A Single-Center Retrospective Cohort Study.

BACKGROUND: Hypothyroidism is one of the most common endocrine conditions during pregnancy and has been associated with poor obstetric and perinatal outcomes. There is still a lack of data from Middle Eastern populations, despite its clinical significance. This study aimed to evaluate thyroid management patterns during pregnancy and examine the association between thyroid function control and maternal and perinatal outcomes in women with hypothyroidism at a tertiary care center in Qatar. METHODS: A retrospective cohort study including 379 pregnant women with hypothyroidism diagnosed between J
