[Bullous pemphigoid induced by photochemotherapy of psoriasis. Apropos of 2 cases, with a review of the literature].

PUVA-induced generalized bullous pemphigoid is reported is 8 patients with psoriasis and compared with the bullous pemphigoid described in 25 patients treated by other techniques (Goeckerman, UVB, anthralin, sun exposure, topical steroids). Furthermore they must be differentiated from all the other types of bullae induced by photochemotherapy in psoriatic patients. Diagnostic criteria should be improved in order to allow a better classification of these bullous diseases and to separate the photo-toxic bullae which are the commonest, from other bullae such as epidermolysis bullosa acquisita, lo…

Influence of UVA on the erythematogenic and therapeutic effects of UVB irradiation in psoriasis; photoaugmentation effects.

The effect of repeated exposure to an additive dose of long ultraviolet (UVA) radiation on the erythemogenic and therapeutic effects of middle ultraviolet (UVB) irradiation was investigated in 8 patients with psoriasis. The surface of the backs of these patients was divided into 2 parts, 1 of which received only UVB irradiation 4 times a week and the other UVA + UVB. UVB was provided by Philips TL-12 lamps and UVA by glass-filtered Philips TL-09 lamps. UVA was held constantly at 10 J/cm2, whereas UVB was increased. The erythemogenic effects of UVA + UVB and UVB alone were evaluated by 4 tests …

Azaribine therapy for psoriasis. Evaluation of potential effects on the liver and other organ systems.

An open study of azaribine was carried out on 38 psoriatic patients for two years. Thirty-five patients had previously taken methotrexate. Thirty-four patients had liver biopsies performed before azaribine therapy and 14 had liver biopsies performed after azaribine therapy. Of these 14, five had grade 1 liver biopsies before and after therapy; one had grade III before and after therapy; and three had grade IV before and after therapy. Liver biopsy findings were slightly worse in two patients, and three patients showed improvement in liver biopsy findings.

The association of age at psoriasis onset and HLA-C*06:02 with biologic survival in patients with moderate-to-severe psoriasis: a cohort study from the British Association of Dermatologists Biologics and Immunomodulators Register (BADBIR).

BACKGROUND: Few studies have used real-world data to investigate the association between biologic therapy survival and age at psoriasis onset or HLA-C*06:02 status in patients with moderate-to-severe psoriasis. The robustness of these studies is limited by small sample size, short follow-up and diverse safety and effectiveness measures. OBJECTIVES: To describe biologic survival and explore whether the response to biologics is modified by age at psoriasis onset or HLA-C*06:02 status in patients with moderate-to-severe psoriasis. METHODS: Data from patients in the UK and the Republic of Irelan…

Sub-population analysis of patients with plaque psoriasis treated with calcipotriol/betamethasone aerosol foam; effectiveness and patient satisfaction, according to baseline disease severity and prior psoriasis treatment experience, in a Greek population.

Fixed combination calcipotriol/betamethasone (Cal/BD) aerosol foam has been shown to be effective in psoriasis treatment in clinical trials, but real-world evidence is currently sparse. The real-world CELSUS study in Greece found that Cal/BD aerosol foam treatment was effective and associated with satisfaction in psoriasis patients. Patients from the CELSUS study (N = 400) were stratified by baseline disease severity according to physician's global assessment (PGA) score (mild vs. moderate vs. severe) and by previous psoriasis treatment (naïve vs. treatment-experienced). Proportions of patient…

Absolute and Relative Psoriasis Area and Severity Indices (PASI) for Comparison of the Efficacy of Ixekizumab to Etanercept and Placebo in Patients with Moderate-to-severe Plaque Psoriasis: An Integrated Analysis of UNCOVER-2 and UNCOVER-3 Outcomes.

Treatment goals defined by the absolute Psoriasis Area and Severity Index (PASI) scores offer certain advantages in the clinical setting. In order to investigate potential treatment targets, this study evaluated absolute PASI outcomes relative to other measures of response using data from two randomized clinical trials of patients with moderate-to-severe psoriasis treated with ixekizumab, etanercept, or placebo (n=2,567). Response was assessed throughout 12 weeks as the proportion of patients achieving absolute PASI band cut-offs who also reached established response criteria. Most PASI band ≤…

IL-23 blockade with guselkumab potentially modifies psoriasis pathogenesis: rationale and study protocol of a phase 3b, randomised, double-blind, multicentre study in participants with moderate-to-severe plaque-type psoriasis (GUIDE).

BACKGROUND: Guselkumab is an interleukin (IL)-23 pathway blocker with proven efficacy in patients with moderate-to-severe plaque psoriasis. Early intervention with guselkumab may result in changes to the clinical disease course versus later intervention. METHODS AND ANALYSIS: Here we present the rationale and design of a phase 3b, randomised, double-blind, multicentre study (GUIDE), comparing treatment effects of guselkumab in patients with short (≤2 years) or longer (>2 years) duration of plaque-type psoriasis, measured from first appearance of psoriatic plaques. Participants achieving skin …

Overcoming the cutaneous barrier: Topical delivery platforms for targeted gene-based therapeutics in psoriasis.

Psoriasis is a chronic immune-mediated skin disorder characterized by persistent inflammation, epidermal hyperproliferation, and complex interactions among immune cells, keratinocytes, cytokines, and genetic factors. Although conventional topical agents, systemic small molecules, and biologics have improved disease management, their clinical utility is often limited by insufficient efficacy, adverse effects, treatment resistance, and poor patient adherence. Recent advances in molecular biology have identified numerous pathogenic genes, signaling pathways, and regulatory RNAs involved in psoria…

Single-Cell Regulatory Network Analysis Identifies Adjunctive Drug Candidates in Early Risankizumab-Treated Psoriasis.

INTRODUCTION: Risankizumab has demonstrated remarkable efficacy in the treatment of psoriasis; however, its long-term use faces multiple challenges, including high costs, reduced efficacy over time, and potential safety concerns, such as infections and malignancies. Therefore, identifying potential alternative or adjunctive therapies to risankizumab has significant clinical importance. METHODS: We analyzed single-cell RNA sequencing data from mature dendritic cells (mDCs) and CD4⁺ T cells collected before risankizumab treatment and at days 3 and 14 post-treatment. Differential expression, cel…
