Hand and wrist.

Some basic principles of orthopedic management of common conditions of the hand and wrist have been presented. These basic principles and an awareness of the potential complications should aid radiologists in evaluation of postoperative films. The interested reader is referred to Green's Operative Hand Surgery and Chapman's Operative Orthopaedics for more detailed information.

Extensor pollicis longus rupture at the tip of a prominent fixation screw: report of three cases.

Rupture of the extensor pollicis longus tendon at the tip of a prominent fixation screw occurred in two cases after fixation of fractures of the radial shaft and in one case after fixation of a fracture of the scaphoid. Patients were referred because of their inability to extend the thumb. Active thumb extension was restored in two patients by screw removal and tendon transfer. The third patient elected nonoperative care. Close scrutiny of good-quality intraoperative x-ray films after internal fixation about the wrist may detect screws that are too prominent and thereby prevent rupture of the

Open reduction and internal fixation of radial head fractures.

Open reduction and internal fixation of displaced fractures of the radial head were reviewed in 14 elbows. Follow-up averaged 32 months. The average elbow score for Mason type II fractures was 96.8 points, corresponding to 100% good or excellent results. Average flexion was 142.5 degrees, and the mean fixed flexion deformity was 3.9 degrees. There was no loss in grip strength. An almost normal elbow was the expected result. Good or excellent results were achieved in only 33% of Mason type III fractures treated with open reduction and internal fixation. The average elbow score was 72.9 points,

Should we remove flexible intramedullary nails after diaphyseal forearm pediatric fracture? A cohort study of 288 patients.

BACKGROUND AND PURPOSE: Routine removal of elastic stable intramedullary nails (ESIN) after pediatric diaphyseal forearm fractures remains common practice despite limited supporting evidence. We aimed to evaluate refracture risk following ESIN retention compared with implant removal and assessed complications and healthcare resource use. METHODS: In this retrospective cohort study, patients treated with ESIN for diaphyseal forearm fractures at a single center were included. Following a policy change, routine implant removal was discontinued in favor of retaining implants unless symptomatic. P

Anteromedial coronoid facet fractures with associated injuries have inferior outcomes to isolated fractures: a clinical outcomes comparison.

BACKGROUND: Varus posteromedial rotatory instability typically results in an isolated anteromedial coronoid (AMC) fracture without an associated radial head fracture or elbow dislocation. However, recent series have shown concomitant radial head fractures, and elbow dislocations may be seen in association with AMC fractures in other injury patterns with an unclear effect on patient outcomes. Therefore, our objective was to compare postoperative patient outcomes in AMC fractures with and without concomitant radial head fractures or elbow dislocations. METHODS: A retrospective review of patient

Radiocarpal Fracture Dislocations: A Retrospective Review on Operative Fixation and Postoperative Range of Motion.

BACKGROUND: Radiocarpal dislocation (RCD) and radiocarpal fracture dislocation (RCFD) are rare but severe injury patterns with multiple types of fixation techniques described. The purpose of this study was to determine the outcomes of RCD and RCDF treated at our institution. METHODS: Patients were identified using our institution's electronic medical records between 2013 and 2022. Seventeen patients met criteria who suffered either RCD or RCFD. Patient charts were reviewed retrospectively with a focus on demographics, mechanism of injury, smoking status, open injury, direction of dislocation,

Certain fracture patterns in children/adolescents would be better called 'Barton equivalent' fractures.

We have encountered consecutive children/adolescents with a volar Barton fracture (VBF) pattern without involving the physis. These were managed by buttress plating; thus, we would like to report the radiological and clinical outcomes through retrospective case series and suggest the revisiting of the 'VBF category' in this population. We screened children/adolescents with a diagnosis of trauma to the bony structures in the wrist from 2008 to 2019. Of these patients, 16 who met our inclusion/exclusion criteria were investigated. At the final follow-up performed at least 2 years postoperatively

Comparison of two closed surgical techniques at isolated pediatric radial neck fractures.

INTRODUCTION: This paper aims to compare the clinical and radiological results of children operated using elastic stable intramedullary nailing as described by Métaizeau to those with the closed reduction and percutaneous pinning (CRPP). METHODS: Results of 21 pediatric patients with radial neck fracture who were treated by two pediatric surgeons between January 2011 and December 2013 were reviewed retrospectively. 10 were treated with the Métaizeau method versus 11 with the CRPP. Operation time, fluoroscopic exposure time, Mayo elbow performance score (MEPS) and radiological assessment at fi

Wrist and scaphoid fractures: a 17-year review of NHSLA litigation data.

Wrist and scaphoid fractures are common injuries seen and treated in everyday orthopaedic practice. The purpose of the study was to assess the trends and reasons for litigation related to wrist and scaphoid fractures within the NHS. Under a freedom of information (FOI) request, data from 1995 to 2012 were provided by the National Health Service Litigation Authority (NHSLA) on all litigation claims made as a result of wrist and scaphoid fractures. A total of 1354 claims relating to the hand and wrist were identified. Wrist and scaphoid fractures accounted for 492 (36.3%) of these claims, and 36

Does early radiography alter remanipulation rates in paediatric forearm fractures?

BACKGROUND: In paediatric patients with a forearm fracture, treated with manipulation under anaesthesia with fluoroscopic guidance, protocol varies whether a day 1 post-operative radiograph is performed. The primary aim of this study is to evaluate whether this investigation changes the management of a patient. Secondary aims evaluated number of reinterventions and factors associated with loss of position. METHODS: A retrospective study design was used. Cases were identified from the Wellington Regional Hospital radiology database between 1 January 2010 and 30 July 2011. Ethical approval was

Exposed versus buried intramedullary implants for pediatric forearm fractures: a comparison of complications.

BACKGROUND: The purpose of this study was to compare the rate of complications between buried and exposed intramedullary implants after fixation of pediatric forearm fractures. METHODS: A retrospective comparative cohort study of 339 children treated with intramedullary fixation for displaced forearm fractures between 2004 and 2009 was performed. Implants were left exposed in 128 patients (37.8%) and buried beneath the skin in 208 patients (61.4%); 3 patients had buried and exposed hardware (0.9%). Data on demographics, injury, surgical technique, and complications were analyzed. RESULTS: Th

Radial head fractures.

The radial head is the most commonly fractured bone of the elbow, with most fractures occurring in women older than 50 years. The radial head is an important stabilizer for valgus, axial, and posterolateral rotational forces. Loss of articular segments of the radial head negatively affects elbow kinematics and stability. Most fractures are treated nonsurgically. Indications for nonsurgical treatment include nondisplaced and isolated displaced fractures without a block to motion. Fragment excision is indicated when a mechanical block from a small, displaced fragment impedes elbow motion. If tec

Distal radial fractures heal by direct woven bone formation.

BACKGROUND: Descriptions of fracture healing almost exclusively deal with shaft fractures and they often emphasize endochondral bone formation. In reality, most fractures occur in metaphyseal cancellous bone. Apart from a study of vertebral fractures, we have not found any histological description of cancellous bone healing in humans. PATIENTS AND METHODS: We studied histological biopsies from the central part of 12 distal radial fractures obtained during surgery 6-28 days after the injury, using routine hematoxylin and eosin staining. RESULTS: New bone formation was seen in 6 cases. It was

Vascularized bone graft pedicled on the volar carpal artery from the volar distal radius as primary procedure for scaphoid non-union.

INTRODUCTION: The treatment of scaphoid non-union with non-vascularized bone graft leads to non-union in 10 to 20% of cases and up to 50% in case of proximal pole necrosis. Vascularized bone graft improves consolidation rates, but is generally restricted to secondary scaphoid non-union. HYPOTHESIS: This study assessed the value of a primary vascularized bone graft pedicled on the transverse volar carpal artery from the volar aspect of the distal radius as donor site. PATIENTS AND METHODS: This retrospective study included 111 cases of vascularized bone graft for scaphoid non-union as primary

Prevalence of vitamin D insufficiency in African American children with forearm fractures: a preliminary study.

OBJECTIVES: Forearm fractures account for a significant proportion of childhood injuries and seem to be increasing in incidence. Poor vitamin D status increases overall fracture risk in infants with rickets and adults with osteoporosis. Children with vitamin D insufficiency (serum 25-hydroxy vitamin D level <20 ng/mL) have decreased bone mineral density (BMD) compared with children having normal vitamin D status. The relationship between vitamin D status and childhood forearm fracture has not been investigated. METHODS: This prospective study enrolled African American children, aged 5 to 9 ye

Early mobilisation of Colles' fractures. A prospective trial.

The results of a prospective controlled trial of early mobilisation of Colles' fractures in the elderly are presented. Early mobilisation produced less pain and a stronger grip. It did not lead to any greater loss of reduction of the fracture. However, there was no significant improvement in the final range of movement of the wrist. Immobilisation of the wrist for six weeks in plaster is extremely inconvenient for the elderly living alone and the patients greatly appreciated the reduction of this period of time to a minimum.

Ultra Short Osteotomy With Mortise-Tenon Diaphyseal Prosthesis Reconstruction for Forearm Metastases: A Retrospective Study.

OBJECTIVE: Forearm diaphyseal metastases are rarely encountered and are predisposed to pathological fracture because torsional stress is repeatedly generated during pronation-supination. In addition, relatively long osteotomy is often required by conventional intercalary prostheses, and limited bone stock in the radius and ulna may be sacrificed. In this study, the feasibility and short- to mid-term outcomes of an intercalary mortise-tenon diaphyseal prosthesis designed to enable ultra-short osteotomy were evaluated in patients undergoing reconstruction after en bloc resection of forearm metas
