Management of non-compressible torso hemorrhage of the abdomen in civilian and military austere environments: a scoping review.

BACKGROUND: Non-compressible abdominal hemorrhage (NCAH) is the leading cause of potentially preventable deaths in both civilian and militar … three research questions. Research question 1 addressed the effectiveness of REBOA, damage control resuscitation, and damage control surgery in managing NCAH in austere environments. No effectiveness studies were found on this topic. Research question 2 identified three knowledge gaps in NCAH management in austere environments. The analysis identified early he …

Successful use of extracorporeal membrane oxygenation for pulmonary embolism, prolonged cardiac arrest, post-partum: a cannulation dilemma.

UNLABELLED: A 30-year-old female gravida 1 (37 weeks + 5 days gestation) underwent a crash Cesarean section for evidence of fetal distress, with the presumed diagnosis of placental abruption. Immediately post-op, the patient had a complete cardiovascular collapse with pulseless electrical activity, requiring cardiopulmonary resuscitation (CPR). Two doses of thrombolytics (Tenecteplase) were administered during the resuscitation, with a presumed diagnosis of a pulmonary embolism. After approximately 45 minutes into the resuscitation, the cardiac surgery team was called to initiate extracorporea…

Severe Hand Ischemia Following Distal Transradial Coronary Intervention for Cardiac Arrest.

BACKGROUND: Despite growing interest in the distal transradial approach (dTRA) for coronary interventions, the risk of hand ischemia remains insufficiently elucidated. CASE PRESENTATION: A 72-year-old woman who experienced an out-of-hospital cardiac arrest was successfully resuscitated and diagnosed with acute myocardial infarction. Primary percutaneous coronary intervention was performed via dTRA with intra-aortic balloon pump support. Post-percutaneous coronary intervention, she developed systemic inflammatory response syndrome due to hypoperfusion during resuscitation, requiring high-dose …

Treatment of a gunshot wound (birdshot) patient with traumatic shock in a hybrid emergency room.

BACKGROUND: Although the Hybrid Emergency Room System (HERS) is a relatively novel method for treating severe trauma patients, there have be … nd reconstruction of the left chest wall, he was discharged uneventfully. CONCLUSIONS: HERS during resuscitation was useful for helping clinicians not only to find the bullets' distribution and judge the severity of injury caused by the gunshot, but also to develop subsequent therapeutic strategies for rescuing the gunshot wound patient from a life-threatening …

Induced Hypothermia to 4.2°C with Neurologically Intact Survival: A Forgotten Case Series.

The lowest recorded core temperature from which a person with accidental hypothermia has survived neurologically intact is 11.8°C in a 2-y-o … a core temperature below 11.8°C, but this case series suggests that the lower limit for successful resuscitation may be far lower. We advise against using core temperature alone to decide whether a hypothermic patient in cardiac arrest has a chance of survival.

Management of post-cardiac arrest syndrome.

Post-cardiac arrest syndrome is a complex and critical issue in resuscitated patients undergone cardiac arrest. Ischemic-reperfusion injury occurs in multiple organs due to the return of spontaneous circulation. Bundle of management practicies are required for post-cardiac arrest care. Early invasive coronary angiography should be considered to identify and treat coronary artery obstructive disease. Vasopressors such as norepinephrine and dobutamine are the first-line treatment for shock. Maintainance of oxyhemoglobin saturation greater than 94% but less than 100% is recommended to avoid fatal

Point-of Care Ultrasonographically Guided Proximal External Aortic Compression in the Emergency Department.

In cases of severe subdiaphragmatic vascular trauma, only in extremis interventions such as emergency thoracotomy with aortic cross clamping or resuscitative endovascular balloon occlusion of the aorta are available for temporization until definitive care. This case report proposes a noninvasive approach consisting of localizing the proximal aorta with ultrasonographic guidance and applying a compressive force to occlude the aorta and limit distal flow. Using point-of-care ultrasonography allows precise compression, continuous monitoring of its efficacy, and early detection of return of sponta

Body mass index and thoracic subcutaneous adipose tissue depth: possible implications for adequacy of chest compressions.

OBJECTIVE: Adequacy of cardiopulmonary resuscitation relies on compression of the thoracic cage to produce changes in intra-thoracic pressures sufficient to generate a pressure gradient. In order to evaluate the efficacy of cardiopulmonary resuscitation in morbid obesity, it is first necessary to determine the depth of thoracic subcutaneous adipose tissue (SAT) and to correlate this with body mass index (BMI). RESULTS: Computerised-tomography images of the thorax of 55 patients with a diagnosis of obesity or morbid obesity (mean BMI 45.95 kg/m2) were evaluated to determine the depth of SAT at…

The crashing patient: hemodynamic collapse.

PURPOSE OF REVIEW: Rapid restoration of tissue perfusion and oxygenation are the main goals in the resuscitation of a patient with circulatory collapse. This review will focus on providing an evidence based framework of the technological and conceptual advances in the evaluation and management of the patient with cardiovascular collapse. RECENT FINDINGS: The initial approach to the patient in cardiovascular collapse continues to be based on the Ventilate-Infuse-Pump rule. Point of care ultrasound is the preferred modality for the initial evaluation of undifferentiated shock, providing informa…

Complete Recovery With the Chain of Survival After a Prolonged (120 Minutes) Out-of-Hospital Cardiac Arrest Due to Brugada Syndrome: A Case Report.

Out-of-hospital cardiac arrest (OHCA) is a crucial public health problem. To improve outcomes of patients after cardiac arrest, the American Heart Association promotes the concept of the chain of survival. We report a case of a 19-year-old man with no markedly past medical history who suffered from OHCA, and he was resuscitated with cardiopulmonary resuscitation, without interruption, during the rescue process for 120 minutes until return of spontaneous circulation (ROSC). Electrocardiogram on admission showed right bundle branch block and ST segment elevation in leads V1-V2, and the patient's …

Haemodynamic and ventilator management in patients following cardiac arrest.

PURPOSE OF REVIEW: The purpose of this study is to review the recent literature describing how to assess and treat postcardiac arrest syndrome associated haemodynamics and manage oxygenation and ventilation derangements. RECENT FINDINGS: Postcardiac arrest syndrome is a well described entity that includes systemic ischemia-reperfusion response, myocardial dysfunction and neurologic dysfunction. Continued resuscitation in the hours to days following return of spontaneous circulation (ROSC) is important to increase the likelihood of long-term survival and neurological recovery. Post-ROSC hypote…

Cervical lung herniation complicating a case of acute asphyxial asthma in a child.

The abrupt onset of respiratory failure secondary to asthma, known as acute asphyxial asthma (AAA) in adults, is uncommonly reported in children. Here, we report a case of a child with the acute onset of respiratory failure consistent with AAA complicated by the finding of a neck mass during resuscitation. This 11-year-old boy with a history of asthma initially presented in respiratory failure with altered mental status after the complaint of difficulty in breathing minutes before collapsing at home. Initially, his respiratory failure was thought to be secondary to status asthmaticus, and trea…

Implantable cardioverter-defibrillators in children: a single-institutional experience.

BACKGROUND: Implantable cardioverter-defibrillators have been infrequently used in children as therapy for resuscitated sudden death and syncope due to ventricular arrhythmias unresponsive to antiarrhythmics. METHODS: The medical records of 5 children with implantable cardioverter-defibrillators were retrospectively reviewed. All patients had experienced syncope and 3 (60%) an out-of-hospital cardiac arrest. Underlying pathology included hypertrophic cardiomyopathy in 2, long QT syndrome in 2, and ventricular arrhythmia after remote repair of congenital heart disease in 1. Open thoracotomy wi

Current management of small-bowel obstruction.

In conclusion, most of the recent advances in the management of small bowel obstruction consist of developments in the imaging modalities av … ted complete obstruction. However, almost all patients do benefit from an initial 12 to 24 hours of resuscitation and decompression in cases of complete obstruction; resuscitation and decompression can usually be extended for a longer period of time in those patients with partial obstruction who exhibit no signs of progression (Fig 6). It is encouraging, howeve …
