Exendin-4, a glucagon-like peptide-1 receptor agonist, alleviates muscular dysfunction and wasting in a streptozotocin-induced diabetic mouse model compared to metformin.

Diabetic muscular atrophy is becoming a fast-growing problem worldwide, including sarcopenia, which is associated with substantial mortality and morbidity risk. Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have been marketed and suggested to exert protective effects on not only glycemic control but also diabetic complications in diabetic patients. In this study, we investigated the therapeutic use of GLP-1RAs exendin-4, compared to antidiabetic drug metformin, for the intervention of muscular dysfunction during diabetic conditions using a streptozotocin (STZ)-induced diabetic mouse mod…

Innovations in the Assessment of Skeletal Muscle Health: A Glimpse into the Future.

Skeletal muscle is the largest organ system in the human body and plays critical roles in athletic performance, mobility, and disease pathogenesis. Despite growing recognition of its importance by major health organizations, significant knowledge gaps remain regarding skeletal muscle health and its crosstalk with nearly every physiological system. Relevant public health challenges like pain, injury, obesity, and sarcopenia underscore the need to accurately assess skeletal muscle health and function. Feasible, non-invasive techniques that reliably evaluate metrics including muscle pain, dynamic…

Late-onset hypogonadism: Clinical evidence, biological aspects and evolutionary considerations.

The growing life expectancy in modern societies has raised scientific interest in identifying medical interventions to alleviate age-associated pathologies such as vascular calcification, cognitive decline, sarcopenia, osteoporosis and sexual dysfunction. Although no such single treatment has thus far been established in humans, some clinicians and patients have set their hopes on testosterone replacement therapy (TRT) as a potential "fountain of youth" for aging men. While TRT has proven effective in ameliorating distinct symptoms of late-onset hypogonadism (LOH), its safety remains to be dem…

mTORC1 underlies age-related muscle fiber damage and loss by inducing oxidative stress and catabolism.

Aging leads to skeletal muscle atrophy (i.e., sarcopenia), and muscle fiber loss is a critical component of this process. The mechanisms underlying these age-related changes, however, remain unclear. We show here that mTORC1 signaling is activated in a subset of skeletal muscle fibers in aging mouse and human, colocalized with fiber damage. Activation of mTORC1 in TSC1 knockout mouse muscle fibers increases the content of morphologically abnormal mitochondria and causes progressive oxidative stress, fiber damage, and fiber loss over the lifespan. Transcriptomic profiling reveals that mTORC1's …

Inpatient functional rehabilitation after liver transplantation: clinical benefit for functional recovery and indicators for early intervention - a retrospective observational study.

Sarcopenia and frailty are common in liver transplant candidates and often deteriorate early posttransplantation, negatively affecting recovery and survival. Since 5 years, we refer liver transplant recipients who, despite being medically stable, are considered unfit for safe discharge home because of physical, social, or functional limitations for continued care at an independent rehabilitation hospital. This retrospective observational study evaluated safety and functional outcomes of this rehabilitation program and explored clinical characteristics associated with referral. We reviewed all …

Association of Muscle-specific Strength With Impaired Physical Function and Prognosis in Heart Failure: A Comparison With Muscle Strength.

OBJECTIVES: The Global Leadership Initiative on Sarcopenia has proposed muscle-specific strength, defined as muscle strength relative to muscle mass. Evidence in patients with heart failure (HF) is scarce; therefore, we compared the associations of muscle-specific strength with physical function and mortality in older patients with HF, in comparison with handgrip strength (HGS) alone. DESIGN: A prospective multicenter study. SETTING AND PARTICIPANTS: This secondary analysis of the FRAGILE-HF cohort included patients aged ≥65 years hospitalized for HF. METHODS: Four muscle-specific strength …

Comparative effectiveness of exercise modalities and nutritional supplementation for sarcopenic obesity in older adults: a network meta-analysis based on randomized controlled trials.

INTRODUCTION: Sarcopenic obesity is highly prevalent among older adults and is associated with adverse clinical outcomes. However, direct comparative evidence on the relative efficacy and safety of different exercise-based rehabilitation strategies, with or without nutritional supplementation or high-protein intake, remains limited. This study aimed to compare and rank the effects of diverse rehabilitation interventions using a systematic review and network meta-analysis. METHODS: PubMed, Embase, the Cochrane Library, and Web of Science were systematically searched from database inception to

Leveraging Multiomic Signatures to Predict Body Composition.

Obesity, sarcopenia, malnutrition, and cachexia all include assessment of body composition in their respective clinical guidelines, although patient evaluations frequently require hardware that may not be available. Body mass index often fills this void, but its accuracy as a body composition phenotyping method is limited. Recent advances suggest a new option for assessing patient body composition: omics-derived circulating blood biomarkers applied in combination with demographic information or alone to estimate the mass or volume of clinically relevant body components. The origin of this move…

Tauroursodeoxycholic acid (TUDCA) ameliorates age-related skeletal muscle loss.

Ageing leads to changes in body composition, including increased adiposity and reduced skeletal muscle mass and force. The alterations in ageing skeletal muscle result from impaired proteostasis driven by factors such as chronic inflammation, hormonal changes and reduced nutrient absorption. Those age-related changes in body composition and skeletal muscle compromise mobility and increase the risk of falls, fractures and metabolic disorders. Tauroursodeoxycholic acid (TUDCA), a bile acid with known benefits in chronic diseases, has been shown by our group to improve cognition and metabolic hom

Mitochondria-sarcoplasmic reticulum crosstalk as a modulator of skeletal muscle mass.

Preservation of skeletal muscle mass and function is a key feature of healthy ageing and relies on the tight coordination between protein synthesis and breakdown to maintain proteostatic balance. These processes impose a substantial energetic demand, highlighting the importance of mitochondrial function in skeletal muscle homeostasis. Increasing evidence indicates that mitochondria and the sarcoplasmic reticulum are functionally interconnected. Effective crosstalk between these organelles contributes to the integration of bioenergetic supply, Ca²⁺ handling, and proteostasis. Disruption of this

Weighted cart pull: A novel outcome measure for sustained motor function in mice.

Sarcopenia, the pathological age-related decline in muscle mass and strength, compromises independence and quality of life in older adults. Currently, no effective treatments are available. To enhance translational research using aged mouse models, we developed and validated the weighted cart pull (WCP) as a novel assessment of sustained motor function. The WCP test involved attaching a weighted cart to the tail of a mouse as it climbed a ramp to a 'resting house'. Mass was increased incrementally until failure, defined as either five consecutive hindquarter pokes without progress or sliding b…

Combined Effects of Early Mobilization and Nutrition on ICU-Acquired Weakness.

Intensive Care Unit-Acquired Weakness (ICUAW) is a very common condition in patients admitted to intensive care units (ICUs), even after relatively short stays. This weakness can develop with a pre-existing background of sarcopenia or cachexia, although these conditions are not always the direct cause. Over the years, much of the literature has focused on the nutritional aspect of the issue, leading to the development of widely accepted guidelines recommending the initiation of early nutrition, with the goal of achieving caloric and protein targets within the first five days of ICU admission. …

GDF8 and activin A are the key negative regulators of muscle mass in postmenopausal females: a randomized phase I trial.

Evolutionary pressures to protect against food scarcity likely resulted in highly-conserved pathways designed to minimize energy expenditure, one of which involves the minimization of muscle mass; these mechanisms may be counter-productive in a modern world suffering from obesity and sarcopenia. Growth differentiation factor 8 (GDF8)/myostatin, acting via ActRIIA/B receptors, is the best-characterized negative regulator of muscle mass, leading to therapeutic efforts to augment muscle growth by blocking GDF8 or ActRIIA/B. ActRIIA/B blockade approximately doubles the muscle increase of GDF8 bloc…

Muscle elasticity variations in assessing age-related changes in adults - A systematic review.

AIM: Muscle loss of strength, mass, and function is a natural process that occurs with aging. It is referred to as sarcopenia and represents a major cause of disability. The assessment and quantification of muscle have garnered considerable interest, leading to the recent introduction of shear wave elastography (SWE) as a noninvasive and accessible method for evaluating muscle. Considering this, our systematic review aimed to define the impact of aging on muscle using SWE. METHODS: We examined publications spanning the past decade, specifically focusing on muscle elasticity in the healthy old…

Machine learning-based online web calculator predicts the risk of sarcopenic obesity in older adults.

BACKGROUND: Sarcopenic obesity (SO) has a higher risk of adverse health events compared to having obesity or sarcopenia alone due to the dual burden of both muscle loss and fat gain. The prevalence of SO is progressively increasing as the population ages and the obesity epidemic progresses. Currently, there are no tools for predicting the risk of SO, and this study aimed to construct a well-performing prediction tool based on machine learning. METHODS: The National Health and Nutritional Examination Surveys (NHANES) 1999-2004 dataset was used for the analysis, and the included data were rando…
