Rugby injuries.

OBJECTIVES: The purpose of this chapter is to review critically the existing studies on the epidemiology of pediatric rugby injuries and discuss suggestions for injury prevention and further research. DATA SOURCES: Data were sourced from the sports medicine and science literature mainly since 1990, and from a prospective injury surveillance project in rugby undertaken by the University of New South Wales (UNSW) in Sydney during 2002. Literature searches were performed using Medline and SportsDiscus. MAIN RESULTS: Reported injury rates were between 7 and 18 injuries per 1,000 hours played, wi

[Arthroscopic revision of the sub-acromial space in impingement syndrome].

Degenerative lesions of the subacromial structures can cause chronic anterior pain and disability of the shoulder. They are most frequently found in patients whose arms are repeatedly exposed to force while raised above head level, e.g. during tennis, volleyball or swimming. This is often due to impingement of the supraspinatus tendon, the subacromial bursa or the biceps tendon against the anterior edge and lower surface of the anterior third of the acromion, the coracoacromial ligament and, in some cases, the acromioclavicular joint. Open anterior acromioplasty as described by Neer is therefo

Development and validation of THUMS version 5 with 1D muscle models for active and passive automotive safety research.

Accurately predicting the occupant kinematics is critical to better understand the injury mechanisms during an automotive crash event. The objectives of this study were to develop and validate a finite element (FE) model of the human body integrated with an active muscle model called Total HUman Model for Safety (THUMS) version 5, which has the body size of the 50th percentile American adult male (AM50). This model is characterized by being able to generate a force owing to muscle tone and to predict the occupant response during an automotive crash event. Deformable materials were assigned to

Simulation of a functional neuromuscular stimulation powered mechanical gait orthosis with coordinated joint locking.

The purpose of this study was to examine a hybrid orthosis system (HOS) for walking after spinal-cord injury (SCI) that coordinates the mechanical locking and unlocking of knee and ankle joints of a reciprocating gait orthosis (RGO), while propulsive forces are injected and unlocked joints controlled with functional neuromuscular stimulation (FNS). The likely effectiveness of the HOS in terms of forward progression, stability, and posture of paraplegic gait was determined in this simulation study. A three-dimensional computer model of a HOS combining FNS with an RGO incorporating feedback cont

[Treatment of acromioclavicular joint dislocation with the Rahmanzadeh joint plate].

Type Tossy III dislocations of the acromioclavicular joint (AC) are treated surgically in German-speaking countries. There is considerable rotation and tilting in the AC joint with elevation or abduction of the shoulder joint over 90 degrees. Thus, in the majority of countless surgical procedures, the abduction and elevation are restricted or immobilized for long periods in abduction strapping or other types of movement restriction. The AC joint plate developed by Prof. Rahmanzadeh enables early functional, complete physical therapy since the plate permits rotation and tilting. Over a period o…

[Therapy and prognosis of bacterial arthritis: a retrospective analysis].

Antibiotic therapy and immediate drainage of the infected joint are established practice in the treatment of septic arthritis. The best method of drainage (repeated puncture, arthroscopy or surgical discharge) remains controversial. We analyze 34 patients with septic arthritis admitted to our hospital from 1984 to 1988. The pathogen was Staphylococcus aureus in 19 cases (56%), streptococcus in 6 (18%), pneumococcus in 2 (6%), other bacteria in (9%) and unknown in 4 (12%). In 62% the infection was blood-borne. The knee was affected in 12 cases (35%) and the shoulder in 10 (30%). A preceding joi

[Effect of physical work on the aging process in certain skeletal bones of the extremities].

The long-term investigation of the influence of physical exercise upon the structure of human skeleton revealed certain peculiar features in the process of developmental reconstruction of joints of the upper (humeral joint, hand joints) and lower (knee joint, the joints of the foot) extremities resulting from the occupation of the persons under study. Roentgen-anatomical studies performed in 433 industrial and 430 office workers established the fact of retardation and deceleration of ageing of the extremities bone system in representatives of physical labour as compared with office workers. Greater tuberosity fragment volume does not predict clinical outcomes or tuberosity healing after reverse shoulder arthroplasty for proximal humerus fractures: a retrospective cohort study.

PURPOSE: Greater tuberosity (GT) fragment healing has been associated with improved outcomes following reverse total shoulder arthroplasty (RTSA) for proximal humerus fractures, yet the relationship between pre-operative GT fragment volume and clinical outcomes remains largely unexplored. We hypothesised that larger GT volumes would be associated with improved outcomes and larger anatomical dimensions. METHODS: We retrospectively studied 153 patients who underwent RTSA for proximal humerus fracture. GT fragment volume was measured on pre-operative computed tomography (CT). Clinical outcomes i

Mid-term clinical and radiographic outcomes at minimum five-year follow-up after stemless reverse shoulder arthroplasty.

BACKGROUND: Stemless reverse total shoulder arthroplasty (rTSA) was developed to preserve humeral bone stock and facilitate revision surgery. While short-term outcomes are promising, mid-term data beyond five years remain limited. The purpose of this study was to evaluate clinical and radiographic outcomes at a minimum follow-up of five years after implantation of a stemless reverse shoulder prosthesis. We hypothesized that metaphyseal fixation would demonstrate sustained clinical function and stable radiographic integration at mid-term follow-up. METHODS: This retrospective case series inclu

Humeral component retroversion in the Comprehensive Reverse Shoulder System arthroplasty: rotations, clinical outcomes, and quality-of-life analysis in a prospective randomized study.

BACKGROUND: The literature highlights various factors influencing rotations in reverse total shoulder arthroplasty (rTSA), including humeral retroversion. Although several studies have noted that increased humeral retroversion leads to reduced internal rotation (IR) and increased external rotation (ER), there have been no randomized trials investigating the relationship between rotations and humeral retroversion. This study aims to determine the optimal degree of retroversion (0°-30°) to achieve the best rotational mobility of the shoulder and to analyze the mobility, clinical functional score

Intraoperative repair of functional subscapularis during RSA by deltopectoral approach could improve internal rotation but does not prevent anterior dislocation.

INTRODUCTION: The role of the subscapularis following reverse shoulder arthroplasty (RSA) remains controversial as repair could restore adequate internal rotation and shoulder stability, but might compromise external rotation. The purpose of this multi-centre study, on a large cohort of RSAs performed using the deltopectoral approach, was to determine the effect of subscapularis preoperative status and intraoperative repair on range of motion, clinical scores and rates of complications at a minimum follow-up of 2 years. HYPOTHESIS: Repair of a functional subscapularis grants better internal r

Mixed-reality improves execution of templated glenoid component positioning in shoulder arthroplasty: a CT imaging analysis.

INTRODUCTION: Glenoid placement is critical for successful outcomes in total shoulder arthroplasty (TSA). Preoperative templating with three-dimensional imaging has improved implant positioning, but deviations from the planned inclination and version still occur. Mixed-Reality (MR) is a novel technology that allows surgeons intra-operative access to three-dimensional imaging and templates, capable of overlaying the surgical field to help guide component positioning. The purpose of this study was to compare the execution of preoperative templates using MR vs.standard instruments (SIs). METHODS

Clinical and Radiographic Outcomes of a Posteriorly Augmented Glenoid Component in Anatomic Total Shoulder Arthroplasty for Primary Osteoarthritis with Posterior Glenoid Bone Loss.

BACKGROUND: The primary objectives of this study were to evaluate the ability of a posteriorly stepped augmented glenoid component, used in patients with primary glenohumeral osteoarthritis with B2 or B3 glenoid morphology, to correct preoperative retroversion and humeral head subluxation and to identify factors associated with radiographic radiolucency and patient-reported clinical outcomes. METHODS: We identified 71 shoulders with B2 or B3 glenoid morphology that underwent anatomic total shoulder arthroplasty with use of a posteriorly stepped augmented glenoid component and with a preoperat

Does Postoperative Glenoid Retroversion Affect the 2-Year Clinical and Radiographic Outcomes for Total Shoulder Arthroplasty?

BACKGROUND: While glenoid retroversion and posterior humeral head decentering are common preoperative features of severely arthritic glenohumeral joints, the relationship of postoperative glenoid component retroversion to the clinical results of total shoulder arthroplasty (TSA) is unclear. Studies have indicated concern for inferior outcomes when glenoid components are inserted in 15° or more retroversion. QUESTIONS/PURPOSES: In a population of patients undergoing TSA in whom no specific efforts were made to change the version of the glenoid, we asked whether at 2 years after surgery patient

Radiographic changes and clinical outcomes associated with an adjustable diaphyseal press-fit humeral stem in primary reverse shoulder arthroplasty.

BACKGROUND: Press-fit humeral fixation in reverse shoulder arthroplasty (RSA) has become increasingly popular; however, radiographic analysis of these stems is limited. We aimed to evaluate the radiographic and clinical outcomes of an adjustable diaphyseal press-fit humeral stem in primary RSA. METHODS: We conducted a retrospective review of 232 primary RSAs in 219 patients performed by a single surgeon using this system. Radiographic outcomes were evaluated in patients with at least 2 years of radiographic follow-up. Standardized postoperative digital radiographs were analyzed for loosening,
