A 10-year review of sleep electroencephalogram (EEG)-frequency analysis in depression reveals several consistent microarchitectural abnormalities. Decreased delta amplitude or incidence, particularly in the first 100 min of sleep, has been reported. Elevated fast-frequency EEG has been shown in both remitted and symptomatic depressed patients, especially in the right hemisphere. Further, interhemispheric coherence is reduced in both depressed groups. These microarchitectural features may not be present in narcolepsy, obsessive-compulsive disorders, or schizophrenia, despite similarities in sle…

Important pathophysiological mechanisms in obstructive sleep apnea at night are increasing pleural pressure swings, hypoxia and hypercapnia, as well as central nervous arousals with consecutive fragmentation of regular sleep structure. They influence the cardiovascular system, at first only at night and at a later stage also during the day. This might result in cardiac structural changes: dilation and hypertrophy of the right ventricle, hypertrophy of the left ventricle, (especially of the muscular ventricular septum), dilation of right and left atrium. It is suggested that these cardiac struc…

Patients with sleep apnea syndrome (SAS) suffer considerable morbidity and increased mortality. The most common symptoms of SAS include excessive daytime sleepiness, nocturnal breath cessation, snoring and gasping sounds. We reviewed the characteristics of 20 Saudi patients with sleep apnea (15 males, five females) who were studied at our tertiary care center in 1992 and 1993. The ages ranged from 15 to 61 years. All were obese. Two each were acromegalic and hypothyroid. Hypertension was noted in seven (35%) patients and one had symptomatic bradycardia. Two (10%) patients presented with recurr…

Morphine effects on many neural and behavioral measures, including tests of learning and memory, are attenuated by increased circulating glucose levels. Using systemic injections, we investigated the ability of glucose to attenuate sleep deficits induced by morphine administration in the rat. Morphine at 1 mg/kg produced a moderate decrease in slow wave sleep which was prevented by concomitant administration of 100 mg/kg of glucose. A higher dose of morphine (10 mg/kg) severely delayed the onset of both slow wave sleep and REM sleep. These delays were attenuated by concurrent administration of…

UNLABELLED: Babies born after in vitro fertilisation (IVF) are increasing in number, and, although these babies are considered as very precious, no data are available regarding their risk for sudden infant death or apnoea. To evaluate the respiratory maturity of IVF babies, we evaluated the incidence of apnoea during an 8-h polysomnography in 50 consecutively presented IVF babies and in a group of 50 unselected naturally conceived babies. All infants were in good health and matched for term (born > 38 weeks of gestation), birth weight, sex and age at the time of investigation (6-11 weeks post

In infancy, prior to cranial suture and fontanel calcification, the craniofacial skeleton can be easily deformed by an externally exerted force. In this study, the relationship between the sleep position and skull morphology was investigated. A group of 81 cleft lip and/or palate infants without other systemic anomalies was first seen in the craniofacial center at approximately 1 month of age. The sleep position of each infant was recorded as supine, prone, or mixed type. The body and skull growth were longitudinally measured at 1, 3, and 6 months of age. Infants sleeping in the supine sleep p…

Infants with repeated apnoea during sleep have received great attention for the assumed reason of being at-risk for sudden infant death. The present paper reports findings which indicate a different risk, namely for neurological impairment during infancy due to repeated hypoxia. A very strong correlation exists between the respiratory measurements based on a polygraphic all-night recording (PtcO2 drops and apnoea incidence and duration) and the impairment of the spontaneous movement repertoire in 114 infants, aged between 3 and 26 weeks. All infants have been video recorded at the same day as …

Ninety-six patients with bipolar disorder who attended a lithium clinic were reviewed in a retrospective study. Sleep disorders were studied in 85 depressive episodes. Eighty-one percent of the subjects presented with insomnia; the mixed type being the most frequent (49%) followed by early awakening (25%). The evolution of depression in the patients was compared according to the treatment received for insomnia: sedative antidepressants vs other anxiolytic or hypnotic drugs. Fifteen percent of patients shifted to mania, this group more frequently receiving sedative antidepressants (p < 0.05). M…

We describe polygraphic respiratory alterations during sleep in a child with neurofibromatosis. The patient, a four-year-old boy, had a medical history of neurofibromatosis and recurrent acute respiratory failure responsive to mechanical ventilation. All-night polysomnography showed severe nocturnal hypoventilation with marked hypercapnia (TcPaCO2 70 mmHg) and hypoxemia (SaO2 less than 40%). Nocturnal hypoxemia and hypercapnia and depressed response to the hyperoxic hypercapnic test confirmed the diagnosis of central hypoventilation syndrome. Cerebral magnetic resonance imaging disclosed lucen…

Sleep apnea syndrome is a relatively common disorder characterized by periodic cessation of breathing during sleep because of upper airway obstruction (obstructive sleep apnea) or by reduction of ventilatory drive (central sleep apnea). We report the case of a middle-aged, obese man who underwent tracheostomy for the treatment of obstructive sleep apnea documented by polysomnography. Although tracheostomy resulted in marked clinical improvement, relief of upper airway obstruction unmasked severe central sleep apnea. We report the successful treatment of this patient's central sleep apnea syndr…

Obstructive sleep apnea syndrome (OSAS) is a subtle but severe sleep disorder of early childhood. It occurs in 1.6%-3.4% of children between 6 months and 6 years of age. OSAS is often difficult to detect, and may have long-term consequences, including failure to thrive, behavioral disturbances, developmental delay, and cor pulmonale. Pediatric nurses are pivotal in assessing the clinical symptoms of OSAS so that prompt evaluation and treatment can be initiated. Early symptoms of OSAS include awakening with a startle or gasp, restlessness, retractions, inspiratory stridor, mouth breathing, and …

The sleep apnoea syndrome (SAS) involves impaired regular breathing during sleep. It causes poor quality sleep and thus subsequent somnolence in daytime, it leads to periodic oxygen desaturation and has serious cardiovascular consequences. The authors describe methods used for the diagnosis and quantification of the intensity of SAS which they used (all-night extended polysomnography, ambulatory equipment MESAM 4 and afternoon polysomnography) and they list also the main therapeutic procedures used in SAS.

The actual therapy of choice for the common obstructive sleep-apnea syndrome is the application of continuous positive airway pressure (CPAP) via a nose-mask. This ventilation by positive pressure is explained in some detail. Mechanical ventilation for treatment of nocturnal respiratory failure in outpatients of various etiologies is covered in the companion paper by J. C. Chevrolet. Treatment of sleep disorders due to other pulmonary diseases such as bronchial asthma and others require treatment of the underlying disorder.

The main objective of the study was first to develop two prototypes of decision support systems for epilepsy and sleep disorder diagnostics. The second goal was to examine medical decision-making with the help of these program examples and further to assess what requirements should be set to decision support systems for them to find their place in clinical work. Requirements were first defined for the systems to be developed. The requirements were, for the most part, successfully taken into consideration during the development process. Epilepsy Expert, a decision support system based on the In…

Developmentally normal twins, joined from the mid-sternum to the umbilicus in a face-to-face position, were observed in 90-minute sessions twice daily from postnatal days 12 to 16. Sleep-waking states were coded in 10-second epochs. Within-twin data were analyzed for bout lengths, interbout intervals, transition probabilities and 60-minute autocorrelation patterns. Between-twin data were studied for simultaneous occurrences of sleep-waking states and 60-minute cross-correlation patterns. Epochs per bout and interbout intervals for quiet and active sleep, active sleep epochs with rapid eye move…

We investigated whether the degree of plagiocephaly (P) (head slant) would predict the particular "position of comfort" adopted prior to sleep. A marked flattening of the right occipital bone (left-dominant occipital plagiocephaly) was noted for the majority of our subjects, coupled with a tendency to prefer a right-sided "position of comfort". Two subjects exhibiting right-dominant occipital P adopted an extreme left-sided sleep position, providing further support for this relationship. We call attention to the possibility that the "position of comfort" adopted prior to sleep, may be a variab…

In previous studies, we have observed that young normal subjects show an increase in intraocular pressure (IOP) after sleep. Here we describe three experiments which investigated: (i) the effects of sleep in five groups of subjects: glaucoma, suspect glaucoma, young high-normal IOP, old high-normal IOP groups and an elderly control group, (ii) the effect of exposure to bright light (2500 lux) during sleep on associated IOP changes, and (iii) the relationship between changes in IOP and plasma melatonin during sleep. For all experiments IOP was measured before and after sleep. We found that IOP …

Obstructive sleep apnea syndrome (OSAS) is associated with severe cardiac arrhythmias and conduction abnormalities. Cor pulmonale and right-sided heart failure may ensue. Uvulopalatopharyngoplasty (UPPP) is one of several treatment modalities suggested for OSAS. Tracheotomy and CPAP treatment in adult OSAS patients and adenotonsillectomy in children with OSAS were shown to lead to improvement in some cardiac parameters. Cardiac function was prospectively evaluated in 19 OSAS patients before and after UPPP. No significant changes after surgery were noted on electrocardiographic studies. Improve…
