Prevalence of joint-related pain in the extremities and spine in five groups of top athletes.

PURPOSE: Joint-related pain conditions from the spine and extremities are common among top athletes. The frequency of back pain has, however, been studied in more detail, and the frequency of low-back pain in top athletes in different high-load sports has been reported to be as high as 85%. Sport-related pain from different joints in the extremities is, however, infrequently reported on in the literature. METHODS: Seventy-five male athletes, i.e. divers, weight-lifters, wrestlers, orienteers and ice-hockey players and 12 non-athletes (control group) were included in the study. A specific self…

The development and evaluation of the subaxial injury classification scoring system for cervical spine trauma.

BACKGROUND: Fractures and dislocations of the subaxial cervical spine may give rise to devastating consequences. Previous algorithms for describing cervical trauma largely depend on retrospective reconstructions of injury mechanism and utilize nonspecific terminology which thus diminish their clinical relevance add to the difficulty of educating doctors and performing prospective research. QUESTIONS/PURPOSES: We characterized the potential benefits of the Subaxial Injury Classification (SLIC) scale which considers three major variables that influence spinal stability: morphology, integrity of…

Successful treatment of a bifocal eosinophilic granuloma of the spine with CT-guided corticosteroid injection.

Observation and immobilization is adequate for most patients with spinal eosinophilic granuloma; however, in patients with symptomatic lesions, treatment other than simple observation or biopsy alone is recommended. In view of the benign clinical course of eosinophilic granuloma, a simple, minimally invasive, outpatient treatment with a low complication rate such as computed tomography (CT)-guided intralesional corticosteroid injection may be considered the treatment of choice. This article presents a case of a 21-year-old man with a symptomatic bifocal eosinophilic granuloma at the vertebral

The influence of postoperative acromial and scapular spine fractures on the results of reverse shoulder arthroplasty.

Fractures of the acromion or scapular spine are recognized complications of reverse arthroplasty. This study reviewed the outcome of reverse arthroplasties with such fractures and compared the results to arthroplasties without the fractures. A consecutive series of 125 reverse arthroplasties were reviewed for the development of a postoperative acromial or scapular spine fracture. Nine cases were identified, all treated nonoperatively. Five fractures occurred from falls, and the remainder were fatigue fractures. These results were compared to 67 patients with similar diagnoses in the series wit…

[Mathematical calculation of strength of the vertebral column in surgical treatment of unstable fractures of the spine].

A mathematical model of a three-vertebra complex was developed in order to make an exact calculation of loss of supporting ability of the vertebral column in trauma. Mathematical description of the dynamic processes was based on Lagrange differential equation of the second order. The degree of compression and instability of the three-vertebra complex, established using mathematical modeling, determines the decision on the surgical treatment and might be considered as a prognostic criterion of the course of the compression trauma of the spine. The method of mathematical modeling of supporting a…

Thoracic spine manipulation for the management of patients with neck pain: a randomized clinical trial.

DESIGN: Randomized clinical trial. OBJECTIVES: To investigate if patients with mechanical neck pain receiving thoracic spine thrust manipulation would experience superior outcomes compared to a group not receiving thrust manipulation. BACKGROUND: Evidence has begun to emerge in support of thoracic thrust manipulation as an intervention n the management of mechanical neck pain. However, to make a strong recommendation for a clinical technique it is necessary to have multiple studies with convergent findings. METHODS AND MEASURES: Forty-five patients (21 females) were randomly assigned to 1 o…

Surgical treatment of post-traumatic kyphosis in the thoracolumbar spine: indications and technical aspects.

Indications for correction of post-traumatic kyphotic deformity of the spine and technical aspects of the surgical procedure are reviewed. Surgical correction of post-traumatic deformity of the spine should be considered in patients presenting a local excess of kyphosis in the fractured area superior to 20 degrees with poor functional tolerance. Severe pain, explained by objective factors such as canal or neuroforamen compromise with or without peripheral symptoms, angular deformity, non-union, focal instability, adjacent painful compensatory deformity such as lumbar hyper-lordosis or thoracic…

Pathological dislocation of the dorsal spine following granulocytic sarcoma in a non-leukaemic patient.

We describe a previously healthy, non-leukaemic young male presenting with neurological deficit and a pathological dislocation of D8 over D9 vertebra. The magnetic resonance imaging showed an enhancing soft tissue tumour. His basic laboratory workup as well as a bone marrow biopsy was normal. Through a single midline posterior approach, he underwent a decompressive laminectomy of D8 and D9 vertebra, anterior column reconstruction with a meshed titanium cage and posterior pedicle screw instrumentation. The histological diagnosis of granulocytic sarcoma was confirmed by appropriate immuno-histoc

Impact of body habitus on perioperative morbidity associated with fusion of the thoracolumbar and lumbar spine.

OBJECTIVE: Spinal fusion is performed in patients ranging from young and healthy to aged and frail. Although recent population trends in the United States are toward obesity, no large-scale study has evaluated how body habitus affects mortality, complications, and resource utilization for lumbar spine fusion. Such information is important for patient selection and to confirm the safety of such procedures in this population. METHODS: Data for 244 170 patients who underwent thoracolumbar or lumbar spine fusion for degenerative disease between 1988 and 2004 were collected from the Nationwide Inp…
