This is the first report describing the use of gas chromatography-mass spectrometry for detection of muramic acid in infected synovial fluid (SF). Muramic acid is a ubiquitous component of bacterial cell walls, and it has been proposed that it could serve as a chemical marker for the presence of live bacteria or bacterial debris in rheumatoid joints. Cutaneous infections with Staphylococcus aureus, Streptococcus pyogenes, and Pseudomonas aeruginosa are major complications of epidermolysis bullosa. Application of impermeable occlusive dressings over denuded skin colonized with these bacteria results in rapid multiplication and the hazard of severe pyoderma. Approaches to the prophylactic treatment of these infections during the long-term management of epidermolysis bullosa are considered.

Two cases of Hemophilus influenzae septic arthritis are reported, one in a rheumatoid patient and the other in a healthy young woman after meningitis. The evolution of this rare infectious arthritis was not as good as usual in spite of early therapy. The first patient later presented a Staphylococcus albus bacteriemia and the other a post-infectious persisting arthritis.

Malakoplakia is a chronic granulomatous inflammatory disorder. It is suspected clinically by the presence of chronic infection and diagnosed by histologic examination of affected tissues. Studies of 4 patients with malakoplakia--2 renal transplant recipients, 1 patient with systemic lupus erythematosus, and 1 patient with polymyositis--are reported. Five patients with nonoliguric adult respiratory distress syndrome (ARDS) secondary to severe sepsis showed improved blood oxygenation after up to 36 h of conventional therapy and mechanical ventilation with optimal positive end-expiratory pressure. A study of excision arthroplasty (Girdlestone's pseudarthrosis) for infected total hip replacements is presented. Twenty-two patients were reviewed with a minimum follow-up of one year. Reduction in pain was significant but the functional results were poor. Fifty four patients with hydrocephalus treated with surgical derivations were evaluated in retrospect. The total number of derivations made to this group was 76. Initially 30 valves were from VA type and the remaining 24 from VP type, but reintervention was needed in 15 and 11 respectively. From 244 patients with leukaemia 34 (13,9%) suffered from septicaemia. Although all forms of leukaemia had been concerned, lymphocytic leukaemias (acute and chronic) predominated. Thirty-one cases of fetal scalp abscess secondary to internal fetal monitoring over a 32 month period were reviewed. The over-all incidence was 1:230 monitored labors, or 0.4 per cent. A method of treatment of pyogenic arthritis of the adult knee employing wide open arthrotomy and early active motion, as well as synovectomy in some knees, was used in thirty-four patients with pyogenic arthritis of the knee. Of these patients, sixteen had a good result, twelve had a fair result, and six had a poor result. This method of treatment was used as a salvage procedure in most of the patients, but may have more general application in the treatment of pyogenic arthritis of the knee.

Of 783 newborn infants with spina bifida cystica and cranium bifidum treated in the 6 years to December 1966, 11% died of ventriculitis. 24% of all deaths were due to this cause. Experience with treatment of ventriculitis with 2 of the antibiotic drugs, gentamicin and cloxacillin, during an 18-month period is presented.
