Cancer cachexia as an immunometabolic syndrome: targeting the immunometabolic interface with traditional Chinese Medicine monomers and formulas.

Cancer cachexia is conventionally viewed as muscle wasting but is increasingly recognized as a tumor-driven immunometabolic syndrome. This systemic disorder is characterized by a vicious cycle of metabolic dysregulation, systemic inflammation, and immune evasion, severely compromising the efficacy of immune checkpoint inhibitors (ICIs). Conventional single-target therapies have largely failed to disrupt this redundant pathological network. Here, we comprehensively review the therapeutic potential of Traditional Chinese Medicine (TCM) as multi-target network modulators acting at the immunometab

Spatial ecology of breast cancer reveals co-evolution of proliferative and dormant niches.

BACKGROUND: Cancer progression involves not only uncontrolled proliferation but also the strategic entry of tumour cells into reversible (quiescent) or irreversible (senescent) states of cell cycle arrest (G0). These states can give rise to rare persister-like cancer cells that survive hostile tumour microenvironment conditions, facilitating drug resistance, metastasis and disease relapse. Despite their importance, identifying and understanding the mechanisms regulating these cell populations remains challenging. METHODS: We leveraged single-cell and spatially profiled primary breast tumours

Interleukin 27 (IL-27) in gastrointestinal cancers: from dual roles in tumor immunity to immunotherapeutic opportunities.

Gastrointestinal (GI) cancers continue to account for a major global cancer burden, with immune dysregulation recognized as a key determinan … tumor progression is determined by the intrinsic characteristics of each tumor, the composition of tumor microenvironment (TME) and the interaction between immune and epithelial cells. Understanding the mechanisms that drive the dual roles of IL-27 across different GI cancers will be important for designing rational immunotherapeutic strategies that either enhance or i …

Protein palmitoylation and immune regulation in pancreatic ductal adenocarcinoma: Integrating insights from cross-cancer studies.

Pancreatic ductal adenocarcinoma (PDAC) is characterized by an immunologically "cold" tumor microenvironment (TME), marked by a dense stromal barrier, impaired antigen presentation, and extensive infiltration of immunosuppressive cells. In recent years, protein S-palmitoylation, a reversible lipid modification, has emerged as a critical mechanism regulating immune signaling, receptor trafficking, and membrane localization of immune-related proteins. This review highlights the multifaceted roles of S-palmitoylation in shaping the immunological landscape of PDAC. We systematically discuss its in…

Targeting metabolic reprogramming to enhance adoptive immunotherapy: emerging mechanisms and translational perspectives.

BACKGROUND: Adoptive cell therapy (ACT) has achieved breakthrough advances in the treatment of malignancies, demonstrating particularly remarkable efficacy in hematologic cancers. However, the therapeutic efficacy of ACT remains limited by multiple factors, including low tumor antigen expression, the limited in vivo persistence of infused immune cells, and the immunosuppressive characteristics of the tumor microenvironment (TME). Meanwhile, metabolic reprogramming, a hallmark of cancer, plays a pivotal role in immune cells by directly regulating their differentiation, functional states, and lo…

Neural regulation of tumor progression: implications for hepatocellular carcinoma.

The nervous system is increasingly recognized as an active regulator of oncogenesis rather than a passive structural component of the tumor microenvironment (TME). Neural activity promotes tumor proliferation, invasion, metastasis, therapeutic resistance and immune evasion through dynamic, bidirectional interactions with malignant cells and stromal components. These effects are mediated by diverse mechanisms, including synaptic and pseudo-synaptic communication, paracrine release of neurotransmitters and neuropeptides, metabolic reprogramming, and neuro-immune crosstalk. Importantly, tumors do…

Multiscale systems modelling of communication networks in brain metastasis.

Brain metastasis remains a major clinical challenge because intracranial progression and therapeutic resistance arise not only from tumour-intrinsic programmes but also from coordinated communication across molecular, cellular, spatial and systemic scales. In this Review, we integrate findings from spatial transcriptomics/proteomics, single-cell and multimodal omics, and computational systems modelling to frame brain metastasis as a multiscale communication disorder in which immune, glial, vascular and systemic networks collectively shape tumour-permissive ecosystems. We discuss how network-ba

Targeting Microbiome-Derived Exosomes to Overcome Cancer Therapy Resistance: Mechanisms, Implications, and Opportunities.

Despite recent advances in both preclinical and clinical cancer therapies, the growing problem of treatment resistance remains one of the most critical challenges in oncology. To overcome the drawbacks of current oncologic treatments, there is a pressing need for new approaches and potential therapeutic strategies. The interaction between the host microbiome and cancer has recently attracted significant research. Among the various routes of microbiome-cancer interaction, microbiome-derived exosomes also offer an intriguing avenue. Exosomes, which are small extracellular vesicles, originate fro

Pitfalls and Emerging Trends in AI-driven Pathological Analysis of Metastatic Brain Tumors.

The integration of artificial intelligence (AI) and computational pathology has the potential to refine prognostic stratification in patients with metastatic brain tumors (MBTs). However, current AI models, which are trained predominantly on extracranial tissues, encounter significant challenges in the unique intracranial microenvironment. Unlike previous reviews that focus on primary brain tumors, this article addresses the challenges of applying AI pathology to MBTs. We discuss three major limitations in AI-driven MBT pathology. First, models struggle with cellular lineage distinction betwee

Programmed cell death inhibitors: a new hope for cancer therapy?

BACKGROUND: Tumor initiation and progression are dynamically regulated by multiple programmed cell death (PCD) pathways. In recent years, beyond apoptosis, novel cell death modalities including necroptosis, pyroptosis, ferroptosis, autophagy cell death, and cuproptosis have been shown to exhibit dual pro-tumorigenic and anti-tumorigenic effects. MAIN BODY: Earlier research primarily focused on their anti-cancer potential through tumor cell elimination; however, emerging evidence reveals that under specific tumor microenvironmental (TME) conditions or genetic contexts, these cell death modalit…

Exosome-Based Strategies in Hepatocellular Carcinoma: Mechanisms, Immunotherapy, and Clinical Challenges.

Hepatocellular carcinoma (HCC) remains one of the most prevalent and lethal malignancies worldwide, characterized by late diagnosis, limited therapeutic options, and poor prognosis. Conventional systemic therapies such as sorafenib and its successors provide only modest survival benefits and are frequently complicated by toxicity and drug resistance. In recent years, immunotherapy has emerged as a promising avenue, yet its efficacy is often restricted by the profoundly immunosuppressive tumor microenvironment (TME). Within this landscape, exosomes-nanoscale extracellular vesicles secreted by t…

Microfluidic chips for decoding cancer-immune crosstalk in immunotherapy.

The crosstalk between cancer cells and the immune system within the tumor microenvironment (TME) governs the efficacy of immunotherapeutic interventions. However, conventional preclinical models fail to recapitulate these dynamic processes. Microphysiological systems, particularly immunocompetent tumor-on-a-chip (TOC) platforms, bridge the gap between simplified two-dimensional cultures and animal models. These devices integrate microfluidic engineering, biomimetic extracellular matrices, and controlled perfusion. These platforms effectively recapitulate the cellular heterogeneity, three-dimen…

Computed Tomography and Magnetic Resonance Imaging.

Imaging in Oncology is rapidly moving from the detection and size measurement of a lesion to the quantitative assessment of metabolic processes and cellular and molecular interactions. Increasing insights into cancer as a complex disease with involvement of the tumor stroma in tumor pathobiological processes have made it clear that for successful control of cancer, treatment strategies should not only be directed at the cancer cells but should also take aspects of the tumor microenvironment into account. This requires an understanding of the complex molecular and cellular interactions in cance…

Tissue-resident immune cells in tumors: Cytokine-mediated regulation, functional plasticity, and therapeutic opportunities.

Traditional tumor immunology research has predominantly focused on circulating recruited immune cells. In contrast, tissue-resident immune cells-a unique subset that stably resides in normal tissues and the tumor microenvironment (TME) for extended periods without entering the systemic circulation-exhibit distinct transcriptional, epigenetic and functional profiles, and their roles in tumor initiation, progression and therapeutic response remain incompletely elucidated. Focusing on the representative subsets of resident T cells, natural killer (NK) cells and macrophages, this review defines th…

Biomaterial-mediated immunomodulation of regulatory T cells in cancer.

Immunosuppressive cancers are characterized by tumor microenvironments (TME) that actively inhibit anti-tumor immune responses. Regulatory T cells (Tregs) within the TME are major players in this disruption, owing to their enhanced suppressive capabilities. Thus, targeting this population may be the key to the success of cancer immunotherapies that fail in certain patients. Notably, Tregs play a critical role in immune homeostasis; therefore, strategies to modulate their function must prioritize selective modulation to reduce the risk of unwanted autoimmunity. Biomaterials are versatile platfo…
