Reduction of unnecessary transfusion and intravenous fluids in severely malnourished children is not enough to reduce mortality.

AIM: To test whether standardising the use of blood transfusions and intravenous (IV) infusions could reduce fatality in severely malnourished children admitted to Mulago Hospital, Kampala. METHODS: Improved adherence to the WHO protocol for blood transfusion and IV fluids was effected in patients with severe malnutrition by continuous medical education. A 'before and after' design was used to study 450 severely malnourished children (weight-for-height < -3 Z-score or presence of oedema) under 60 months of age. A total of 220 pre- and 230 post-'improved practice' patients were enrolled consec

Cross-Cutting Implementation Strategies to Integrate Hypertension Care Into HIV Care in Africa: Results From Six Country Studies in the HLB-SIMPLe Alliance.

INTRODUCTION: The National Heart, Lung and Blood Institute (NHLBI), in 2020, funded six implementation research trials in Africa (Botswana, Mozambique, Nigeria, South Africa, Uganda and Zambia) to test implementation strategies for integrating evidence-based cardiovascular disease (CVD) care into primary care for people living with HIV. We identified cross-site implementation strategies used by the six trials to integrate CVD care for people living with HIV in these diverse settings. METHODS: Between December 2022 and September 2023, we reviewed protocols for each trial and followed up with s…

Virological failure and treatment interruption among people with unsuppressed viral load on antiretroviral therapy: a cross-sectional analysis of Population-based HIV Impact Assessment surveys in seven sub-Saharan African countries.

BACKGROUND: Viral load suppression among people with HIV on antiretroviral therapy (ART) remains a priority in sub-Saharan Africa. We aimed to quantify virological failure versus interruption in treatment among people aged 15 years or older with unsuppressed viral load. METHODS: In this cross-sectional analysis of nationally representative Population-based HIV Impact Assessment (PHIA) household surveys conducted in Eswatini, Lesotho, Malawi, Mozambique, Tanzania, Uganda, and Zimbabwe during 2019-23, we included participants aged 15 years or older with HIV who met the harmonised PHIA analytica…

Understanding the pathogenesis of lean non-autoimmune diabetes in an African population with newly diagnosed diabetes.

AIMS/HYPOTHESIS: Apparent type 2 diabetes is increasingly reported in lean adult individuals in sub-Saharan Africa. However, studies undertaking robust clinical and metabolic characterisation of lean individuals with new-onset type 2 diabetes are limited in this population. This cross-sectional study aimed to perform a detailed clinical and metabolic characterisation of newly diagnosed adult patients with diabetes in Uganda, in order to compare features between lean and non-lean individuals. METHODS: Socio-demographic, clinical, biophysical and metabolic (including oral glucose tolerance test…

Potential of Zanthoxylum leprieurii as a source of active compounds against drug resistant Mycobacterium tuberculosis.

BACKGROUND: Tuberculosis (TB) is still a global health problem mainly due to development of resistance and co-infection with the Human immune Virus (HIV). Treatment of multi and extensively drug resistant TB requires use of second line drugs which are less efficacious, expensive and very toxic. This has necessitated a need to search for new treatment regimens especially from medicinal plants. Zanthoxylum leprieurii, a plant species from Rutaceae is used locally in the treatment of tuberculosis in Uganda. The aim of the study was to isolate, identify and characterize bio active compounds from Z…
