In order to clarify the indications of MAGPI procedure (Meatal Advancement and Glanuloplasty Incorporated), 18 patients operated upon have been reassessed after a long follow-up (mean: 30 months). The cosmetic and functional results are good in 16 cases, 2 non-satisfactory results are noticed with a wrong meatal position. The initial position of the meatus (glanular or coronal), the absence of chordee and the initial aspect of the glans (broad and flat) are the three main conditions to obtain a good result.

They had three previous hypospadias repair attempts in their childhood, leaving them with a meatus near the glans but with severe ventral penile curvature. Urethroplasty was carried out with pedicle skin of prepuce in 6 patients with the hypospadias since Feb 1987. The satisfactory results have been achieved and no post-operative complications of urethral fistula and stenosis were found in all patients. The authors consider that the operative procedure is simple, time-saving, and satisfactory for hypospadias.

A 13-year-old hypospadic boy with a urethral stone was successfully treated with extracorporeal shock wave lithotripsy after various unsuccessful attempts at urological extraction. The easiness of ESWL application in this case and the quick elimination of the stone suggest that ESWL should be considered in the management of urethral calculi.

Retrograde ejaculation can be a significant problem when it interferes with fertility. Sympathomimetic drugs and sperm retrieval from the urine with insemination of the wife are the common methods of treatment. When these methods have not been successful we have used the Young-Dees type of bladder neck reconstruction to convert retrograde ejaculation into normal or antegrade ejaculation. The operation has been successful in 4 of 5 patients.

Twenty-six women (aged 26 to 60) with stress incontinence underwent urodynamic investigation before and after oral administration of 5 mg of midodrine, a new adrenergic agent with alpha-stimulant activity. A significant increase in maximum urethral closing pressure combined with a nonsignificant increase in flow time and maximum flow was observed after treatment.
