Effect of transcatheter pulmonary valve implantation on short-term right ventricular function as determined by two-dimensional speckle tracking strain and strain rate imaging.

Transcatheter pulmonary valve implantation (PVI) is an emerging therapy for right ventricular (RV) outflow dysfunction in congenital heart disease. We investigated, for the first time in children after surgery for congenital heart disease, the short-term effects of PVI on RV and left ventricular (LV) function using 2-dimensional speckle tracking echocardiography and tissue Doppler imaging. We hypothesized that the short-term RV and LV function would improve. Two-dimensional speckle tracking echocardiograms and pulsed tissue Doppler images were obtained before and 1 to 2 days after PVI (18-mm M

Delayed Repair of an Iatrogenic Perimembranous VSD After Redo Aortic Valve Replacement: Timing Over Urgency.

BACKGROUND: Iatrogenic ventricular septal defect (VSD) is an uncommon complication of redo aortic valve surgery, particularly during prosthesis explantation near the membranous septum. CASE SUMMARY: A 71-year-old woman with a prior 19-mm bioprosthetic aortic valve underwent redo aortic valve replacement for severe prosthetic dysfunction. Explantation created a perimembranous VSD that was initially closed transaortically; intraoperative transesophageal echocardiography showed a trivial residual shunt. On postoperative day 5, transthoracic echocardiography revealed a new 0.5-cm VSD with left-to

Cardiovascular magnetic resonance imaging in mitral valve disease.

This paper describes the role of cardiovascular magnetic resonance (CMR) imaging in assessing patients with mitral valve disease. Mitral regurgitation (MR) is one of the most prevalent valvular heart diseases. It often progresses without significant symptoms, leading to left ventricular overload, dysfunction, frequent decompensated heart failure episodes, and excess mortality. Cardiovascular magnetic resonance assessment is recommended for MR when routine ultrasound imaging information is insufficient or discordant. A well-planned CMR can provide an in-depth assessment of the mitral valve appa

Mitochondrial CaMKII causes adverse metabolic reprogramming and dilated cardiomyopathy.

Despite the clear association between myocardial injury, heart failure and depressed myocardial energetics, little is known about upstream signals responsible for remodeling myocardial metabolism after pathological stress. Here, we report increased mitochondrial calmodulin kinase II (CaMKII) activation and left ventricular dilation in mice one week after myocardial infarction (MI) surgery. By contrast, mice with genetic mitochondrial CaMKII inhibition are protected from left ventricular dilation and dysfunction after MI. Mice with myocardial and mitochondrial CaMKII overexpression (mtCaMKII) h

Hereditary transthyretin amyloidosis caused by the rare Phe33Leu mutation.

Hereditary transthyretin amyloidosis is a rare progressive systemic disease. We describe a physically active 46-year-old man who presented with dyspnoea on exertion. An echocardiogram showed increased left ventricular wall thickness and diastolic dysfunction, but normal systolic function. The QRS voltage on ECG was normal. The patient was diagnosed with hypertrophic cardiomyopathy, and several years passed before establishment of the accurate diagnosis of hereditary transthyretin amyloidosis caused by the rare mutation ATTR Phe33Leu, previously described in only five case reports. Three-dimensional entertainment as a novel cause of takotsubo cardiomyopathy.

Takotsubo cardiomyopathy (TC) is an uncommon entity. It is known to occur in the setting of extreme catecholamine release and results in left ventricular dysfunction without evidence of angiographically definable coronary artery disease. There have been no published reports of TC occurring with visual stimuli, specifically 3-dimensional (3D) entertainment. We present a 55-year-old woman who presented to her primary care physician's office with extreme palpitations, nausea, vomiting, and malaise <48 hours after watching a 3D action movie at her local theater. Her electrocardiogram demonstrated

Decrease in beta-receptor density explaining the development of pindolol- and prenalterol-tolerance in orthostatic hypotension.

A 58-year-old woman with diabetic autonomic dysfunction was well treated for orthostatic hypotension with pindolol. After eight months however, symptoms recurred in spite of continued treatment with pindolol. Addition of prenalterol, a beta-blocking agent with very high intrinsic sympathomimetic activity, had a marked clinical effect on her orthostatic hypotension. Before treatment with prenalterol, stroke volume and left ventricular enddiastolic volume markedly decreased during tilt, as demonstrated by radionuclide angiography. After short-term prenalterol treatment, the orthostatic decreases

Takotsubo Syndrome Following Inhalation of K4: A Case Report.

Takotsubo syndrome (TS), also known as stress cardiomyopathy, is a clinical condition characterized by transient left ventricular dysfunction in the absence of coronary artery disease findings. The most widely accepted mechanism suggests a trigger, either physical or emotional stress, which causes catecholamine release and results in myocardial stunning and contractility alterations. Clinically and analytically, it can mimic acute coronary syndromes. Synthetic cannabinoids, such as the inhaled drug K4, are more powerful than phytocannabinoids and act as strong agonists of cannabinoid receptors

Relation between systemic arteriosclerosis and choroidal blood flow in patients with acute coronary syndrome.

Blood flow parameters in the optic nerve head are associated with systemic atherosclerosis, early renal dysfunction, and diastolic cardiac dysfunction in patients with acute coronary syndrome (ACS). This study aimed to investigate the relationship between choroidal blood flow parameters evaluated using laser speckle flowgraphy (LSFG) and systemic atherosclerosis, cardiac function, and coronary artery disease severity in patients with acute coronary syndrome. We evaluated 44 patients admitted to the coronary care unit of Nihon University Itabashi Hospital for ACS between April 2019 and Septembe

The effects of extracellular vesicles derived from Krüppel-Like Factor 2 overexpressing endothelial cells on the regulation of cardiac inflammation in the dilated cardiomyopathy.

BACKGROUND: Dilated cardiomyopathy (DCM) is one of the common causes of heart failure. Myocardial injury triggers an inflammatory response and recruits immune cells into the heart. High expression of Krüppel-like factor 2 (KLF2) in endothelial cells (ECs) potentially exerts an anti-inflammatory effect. However, the role of extracellular vesicles (EVs) from KLF2-overexpressing ECs (KLF2-EVs) in DCM remains unclear. METHODS AND RESULTS: EVs were separated from the supernatant of KLF2-overexpressing ECs by gradient centrifugation. Mice were repeatedly administered low-dose doxorubicin (DOX) and

Effect of Mobile Internet Technology in Health Management of Heart Failure Patients Guiding Cardiac Rehabilitation.

Источник пометил публикацию как отозванную

In this paper, we are going to explore the effect of mobile Internet technology in health management for the heart failure patients with cardiac rehabilitation. To implement and evaluate its effects in real environment, a total of 60 heart failure patients were divided, preferably in random fashion, into two groups of 30 cases, i.e., control and observation groups. The former group has received the routine home cardiac rehabilitation after discharge, and the latter group, i.e., observation, has received home cardiac rehabilitation based on mobile medical technical guidance after discharge. Therapeutic Delivery of miR-148a Suppresses Ventricular Dilation in Heart Failure.

Heart failure is preceded by ventricular remodeling, changes in left ventricular mass, and myocardial volume after alterations in loading conditions. Concentric hypertrophy arises after pressure overload, involves wall thickening, and forms a substrate for diastolic dysfunction. Eccentric hypertrophy develops in volume overload conditions and leads wall thinning, chamber dilation, and reduced ejection fraction. The molecular events underlying these distinct forms of cardiac remodeling are poorly understood. Here, we demonstrate that miR-148a expression changes dynamically in distinct subtypes

Recent advances in cardiovascular risk reduction: implications of ONTARGET.

Renin-angiotensin-aldosterone system (RAAS) overactivity is associated with increased cardiovascular risk, a finding that may be explained by the key role of the RAAS in stimulating vascular and cardiac remodeling. Inhibition of RAAS activity with the use of angiotensin-converting enzyme (ACE) inhibitors or angiotensin II receptor blockers (ARBs) has been shown to reduce cardiovascular mortality in patients with heart failure. ACE inhibitors have also been shown to reduce the incidence of stroke, myocardial infarction (MI), and heart failure in high-risk patients without heart failure. These f

Angiotensin-converting enzyme gene polymorphism is associated with vulnerability to alcoholic cardiomyopathy.

BACKGROUND: Chronic alcohol abuse has a dose-dependent toxic effect on the myocardium, leading to alcoholic cardiomyopathy. The fact that only a minority of persons with chronic alcoholism have this condition suggests the possibility of a genetic vulnerability. In this context, polymorphism of the angiotensin-converting enzyme (ACE) gene has been implicated in cardiac dysfunction. OBJECTIVE: To compare the ACE genotypes of alcoholic persons who have cardiomyopathy with those of comparable alcohol abusers who have normal cardiac function. DESIGN: Case-control study over a 2-year period. SETT

The role of nitric oxide in ischaemia/reperfusion injury of isolated hearts from severely atherosclerotic mice.

Nitric oxide (NO) may play an essential role for maintenance of cardiac function and perfusion, while endothelial dysfunction of atherosclerotic vessels may aggravate ischaemia/reperfusion injury. This paper investigates the role of nitric oxide in ischaemia/reperfusion injury in hearts with coronary atherosclerosis. Hearts of apolipoprotein E/LDL receptor double knockout (ApoE/LDLr KO) mice fed an atherogenic diet for 7-9 months were isolated and Langendorff-perfused with 40 minutes of global ischaemia and 60 minutes reperfusion, and funtion and infarction compared with hearts of C57BL/6 cont

Circulating Biomarkers are Associated with Impaired Right Ventricular Circumferential and Longitudinal Strain Measured by Cardiac Magnetic Resonance Imaging in Children and Adolescents with Repaired Tetralogy of Fallot.

Prior studies have shown that circulating biomarkers are associated with increased myocardial fibrosis and worse outcomes in patients with surgically repaired tetralogy of Fallot (rTOF). However, associations of circulating biomarkers with right and left ventricular myocardial deformation using strain by cardiac magnetic resonance (CMR) imaging remain understudied. We aimed to investigate whether these associations in rTOF youth undergoing CMRs inform ventricular remodeling. Eight circulating biomarkers were measured in rTOF patients at the time of CMR. Right and left ventricular (RV, LV) long

Bromocriptine-induced coronary spasm caused acute coronary syndrome, which triggered its own clinical twin--Takotsubo syndrome.

Bromocriptine-induced coronary spasm (BICS) causing myocardial infarction has been reported. Association between BICS and Takotsubo syndrome (TS) has not been described. We report on a 37-year-old woman presenting with a clinical picture of acute coronary syndrome 1 day after initiation of treatment with bromocriptine for ablactation 3 weeks after a full-term spontaneous vaginal delivery. Coronary angiography showed diffuse narrowing of a large diagonal branch. Left ventriculography showed widespread hypokinesia extending beyond the diagonal branch supply region. There was a slight elevation o

Biventricular Systolic Function and Myocardial Deformation in Liver Cirrhosis: A Systematic Review and Meta-Analysis of Speckle-Tracking Echocardiography and Cardiac Magnetic Resonance Feature Tracking Studies.

Background: Liver cirrhosis is frequently associated with cardiovascular abnormalities collectively referred to as cirrhotic cardiomyopathy, characterized by impaired cardiac reserve and subclinical myocardial dysfunction despite preserved conventional systolic function. Advanced myocardial deformation imaging techniques, including two-dimensional speckle-tracking echocardiography (2D-STE) and cardiac magnetic resonance feature tracking (CMR-FT), may allow earlier detection of subtle ventricular impairment. We performed a systematic review and meta-analysis to comprehensively evaluate conventi

Renal dysfunction is an independent risk factor of chronic cardiac failure (CCF) and death due to this disease. CCF patients are elderly patients with diabetes mellitus, arterial hypertension and long-term chronic cardiac insufficiency. CCF patients do not often have left ventricular systolic dysfunction, renal affection is not associated with low ejection syndrome. Renal affection in CCF is primarily caused by activation of the system rennin-angiotensin, inflammation, disturbed bioavailability of nitric oxide, hyperactivation of the sympathetic nervous system.
