Tolerance of a soy formula by infants and children.

For a minimum of one month (mean, 54 days), 287 infants and children less than 8 years of age were fed an isolated soy-protein formula. Prior to entry into the study, a cow's milk formula was being fed to 71%, a soy formula to 9%, and cow's milk or other formulas to 20%. Intolerance to cow's milk was reported in 35% of the patients, symptoms indicative of cow's milk intolerance in 23%, diarrhea or gastroenteritis in 18%, a family history of allergy in 13%, and insufficient weight gain, intolerance to other formulas, or constipation in 11%. The patients showed normal increases in weight and len

The effect of sleep on the dyskinetic movements of Parkinson's disease, Gilles de la Tourette syndrome, Huntington's disease, and torsion dystonia.

The effect of sleep on the involuntary movements or dyskinesias in Parkinson's disease, Huntington's disease, primary and secondary torsion dystonia, and Gilles de la Tourette syndrome was studied in a total of 52 patients and 10 normal subjects using video electroencephalographic telemetry. Movements typical of the wake pattern were seen occasionally during unequivocal sleep in all but two completed studies, and in each condition reappeared under similar circumstances. The movements were most likely to occur after awakenings or lightenings of sleep, or in stage one sleep. The movements were v

Differential effects of sleep stage on coronary hemodynamic function.

The sleep-wake cycle results in distinctive patterns of autonomic nervous system activity. The present study examined the effects of rapid eye movement and slow-wave sleep on coronary hemodynamic function in chronically instrumented dogs. Mean arterial blood pressure was measured via a catheter in the aorta, and coronary blood flow was determined with Doppler probes placed around the left circumflex and right coronary artery. Identification of sleep stages was accomplished by means of electrodes implanted via the frontal sinus to record electro-oculogram, electromyogram, and electroencephalogr

[What is new in nasopharyngeal video fiber optic endoscopy in the diagnosis of snoring and patients with obstructive apnea?].

Videoendoscopy of the oropharynx was performed in 57 patients with habitual snoring and obstructive sleep apnoea syndrome. Endoscopy was done in waking supine subjects relaxed and during voluntary snoring, Esmarch's and modified Müller's manoeuvre. Evaluation of results for the first time was semiquantitative. Statistical analysis showed significant correlations (p less than 0.004) between movements of the base of the tongue and the modified Müller's manoeuvre. For the first time it could be shown that the dynamic changes in the pharyngeal walls during voluntary snoring are identical with thos

[Training of patients with urinary incontinence].

The different types of urinary incontinence are defined and the possible educational treatment modalities are described. Stress incontinence may be treated by pelvic floor exercise and bio-feed-back. The object is training in muscle awareness with the purpose of teaching the patients to squeeze without activating the abdominal muscles. Significant improvement can be obtained in 22-63% of the patients. Urge incontinence can be treated by bladder drill and bio-feed-back. Voluntary control over the detrusor reflex is re-educated and the patients are instructed to adhere to a fixed voiding schedul

[Selective proximal vagotomy with and without pyloroplasty--results of a randomized clinical study of duodenal ulcer 5 and 8 years after surgery].

Selective proximal vagotomy (SPV) with pyloroplasty (n = 39) and without (n = 39) were compared five and eight years after surgery in a randomized clinical study. Pyloroplasty according to Finney had no effect on ulcer recurrence rates after both periods of time (7.9:7.9 or 10.8:8.3 per cent). Additional pyloroplasty, too, had no statistically significant effect on incidence and severity of post-vagotomy syndrome, dumping and diarrhoea, though some trend towards an increase was discernible in wake of pyloroplasty. Occurrence of the lactase deficit syndrome after SPV with pyloroplasty was incre

Narcolepsy in a pediatric population.

Narcolepsy, a sleep-wake disorder of unknown cause, has been reported as occurring in the pediatric population, but only two reports of cases in the literature have included polysomnographic data on children with narcolepsy. We compared the clinical and polysomnographic data on a series of eight patients 15 years of age or younger and an adult comparison group with narcolepsy. All patients presented with excessive daytime sleepiness, and no significant difference was found between groups for the incidence of cataplexy, hypnagogic hallucinations, and sleep paralysis. On polysomnographic evaluat

Pregnancy-induced hypertension and low birth weight in magnesium-deficient ewes.

The fetal and maternal morbidity and mortality from the hypertensive disease states of pregnancy is a major problem. While much is known about the syndrome, the cause has been elusive. The ewe was chosen to test a hypothesis that depletion of magnesium may be involved. Twelve Finnish ewes were subjected to low magnesium diets with half given magnesium in the water. Tests included measurement of blood pressure in the waking state and by noninvasive technique. Magnesium levels were measured by atomic absorption spectrophotometry in the plasma and tissue of the ear tips. Findings included signifi

Forearm blood flow and metabolism during stylized and unstylized states of decreased activation.

We have measured forearm oxygen consumption and blood flow changes during two wakeful rest behaviors. We have observed acute reduction of forearm respiration (28%) during an acute stylized rest state (TM) and a nonsignificant small decline (11%) during unstylized ordinary eyes-closed rest. These changes were not associated with significant change of forearm blood flow or glycolytic metabolism. Hence, forearm oxygen consumption decline was due almost solely to decreased rate of oxygen extraction. Small variation of forearm blood flow implies that little of the previous findings of increased non

[EEG changes in the course of progressive cerebral disorders in children (author's transl)].

This survey presents EEG-data on the following disorders: Globoid-cell leucodystrophy (Krabbe), Metachromatic Leukodystrophy, GM2 Gangliosidosis I (Tay-Sachs), Neuronal Ceroidlipofuscinosis. During Krabbe's disease the background activity of the EEG slows down, multifocal and generalized hypersynchroneous activity (HSA) appears, in the final stage hypsarrythmia is found occasionally. In contrast HSA during Sulfatide lipidosis appears only in the final stages. The EEG changes in Tay Sachs disease are minimal in the beginning; later only high amplitude slowing and especially focal abnormalities

Conserved sleep disturbances in FOXP1 syndrome originate from developmental dysregulation of peptidergic signaling.

Sleep disturbances are among the most prevalent clinical features of FOXP1 syndrome, yet their nature and underlying mechanisms remain unclear. Here, we report that individuals with FOXP1 syndrome suffer from insomnia with sleep maintenance problems and early waking. Consistently, common variants in FOXP genes were associated with insomnia symptoms and short sleep. These sleep disturbances were recapitulated in Drosophila FoxP mutants, which exhibit severely fragmented and reduced sleep. FoxP loss also led to circadian arrhythmicity and impaired the plasticity of neuropeptide pigment dispersin

Off-label Use of Intravenous Thrombolysis in an Acute Ischemic Stroke Patient With Multiple Contraindications.

INTRODUCTION: Intravenous thrombolysis (IVT) is a powerful treatment for acute ischemic stroke, yet its use remains limited due to numerous absolute and relative contraindications. CASE REPORT: We present the case of a 54-year-old man who received intravenous thrombolysis (IVT) for an early recurrent wake-up ischemic stroke caused by a tandem left internal carotid artery and M2 middle cerebral artery occlusion, occurring <48 hours after the initial ischemic event, which had been misdiagnosed as a peripheral-type facial nerve palsy. Moreover, in the context of the initial work-up, the patient

Metabotropic Glutamatergic Signaling Adaptively Controls Trans-thalamic Communication.

Trans-thalamic circuits link cortical regions through higher-order sensory (HOs) thalamic nuclei to support cognition. These circuits are rich with metabotropic glutamate receptors (mGluRs), whose seconds-long dynamics conflict with the thalamus's traditional relay role, leaving their function unknown. Here, we present a model of group I mGluR-mediated suppression of K + leak currents in single neurons. Including mGluRs in a trans-thalamic network of spiking neurons revealed distinct modulatory roles for thalamocortical and corticothalamic mGluRs. In pyramidal neurons, thalamocortical mGluRs s

Ablative Fractional Carbon Dioxide Laser Surgery Improves Sleep Disorder in Pediatric Patients With Hypertrophic Scars.

Hypertrophic scarring from burns can lead to sleep disturbances in children and significantly impact their quality of life and that of their caregivers. This study investigated the sleep quality of 9 pediatric patients with hypertrophic scars who underwent ablative fractional carbon dioxide laser (CO2-AFL) surgery. Electrocardiography was recorded before the first CO2-AFL treatment and after the final treatment. Cardiopulmonary coupling software was used to automatically generate sleep reports, including total time in bed, total sleep time, sleep efficiency, sleep latency time, deep sleep time

Orexin pathway modulation in narcolepsy: from Autoimmune mechanisms to nanotechnology enabled therapeutic delivery.

Narcolepsy is a long-term brain disorder caused by disrupted orexin signaling. Orexin neurons sit in the hypothalamus and control the sleep-wake cycle; damage to these neurons leads to narcolepsy. Early evidence pointed to an autoimmune attack on orexin neurons as the main cause. Current treatments - stimulants, antidepressants, sodium oxybate, and scheduled naps - ease symptoms but do not stop the disease, and they often cause side effects such as anxiety, insomnia, and heart problems. Most of these drugs also cannot cross the blood-brain barrier (BBB) to act directly on the hypothalamus. Thi

Slow wave generation and propagation in a model of brain lesions.

Slow waves (SWs), the hallmark of non-rapid eye movement (NREM) sleep, reflect the periodic occurrence of transient silent periods in cortical neurons (Down states). During NREM, SWs and Down states physiologically disrupt large-scale network interactions. Since early EEG studies, SWs have also been observed in awake patients after brain injury. Emerging evidence indicates that these intrusions of sleep-like activity interfere with ongoing network activity and contribute to motor and cognitive deficits; yet, the mechanisms governing the generation and spread of post-lesional SWs remain unclear

Hormonal contraceptives and women's sleep health: A systematic review.

Sex hormones and hormonal contraceptives influence the regulation of sleep-wake behavior. However, there are very few large-scale studies to date that have comprehensively evaluated how hormonal contraceptives influence women's sleep health. The purpose of this systematic review was to synthesize the existing research on hormonal contraceptive use and sleep in women ages 18-50. The systematic review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) statement checklist. Nineteen studies were included in this review. Data were extracted and evalu

The role of circadian rhythms in the pathogenesis of myopia.

Myopia is a complex ocular disorder arising from the interaction of genetic predisposition and environmental cues that regulate eye growth. Increasing evidence indicates that the circadian timing system plays a critical role in ocular development and refractive homeostasis. The retina, choroid, and sclera possess intrinsic molecular clocks that generate rhythmic oscillations in gene expression, neurotransmitter release, and tissue physiology. Disruption of these ocular circadian rhythms has been implicated in abnormal axial elongation through pathways involving dopamine and melatonin signaling

Targeted Sedation And Circadian Interventions In Mechanically Ventilated ICU Patients: A Narrative Review.

Circadian rhythms regulate sleep-wake cycles, hormone secretion, immune functions, and cardiovascular activity. However, they are frequently disrupted in mechanically ventilated ICU patients due to critical illness, environmental factors, and sedative exposure. This narrative review aims to examine the interaction between sedation practices, environmental interventions, and circadian disruption. A structured, non-systematic search of MEDLINE/PubMed, Web of Science, and Scopus (200-2025) was performed to identify mechanistic and clinical studies. Evidence indicates that deep and prolonged sedat

Current insights into bacterial secondary infection following influenza A virus infection.

Influenza A virus (IAV) continues to pose a substantial challenge to global health, not merely through primary viral pneumonia but largely due to lethal secondary bacterial complications. Pathogens such as Streptococcus pneumoniae, Staphylococcus aureus, and Haemophilus influenzae capitalize on the physiological "storm" induced by IAV, leading to significantly exacerbated morbidity. This review provides a comprehensive synthesis of the multifaceted mechanisms that dismantle host antibacterial defenses. Beyond the classical understanding of respiratory epithelial damage and the compensatory upr
