Effect of cerebellar stimulation on postural control and associated resting-state functional alterations in chronic ankle instability.

BACKGROUND: Cerebellar maladaptive plasticity underlies postural deficits in chronic ankle instability (CAI), and we tested whether cerebellar-targeted transcranial direct current stimulation (tDCS) can provide a rapid, mechanism-specific postural benefit. RESEARCH QUESTION: Would a single cerebellar tDCS session yield immediate improvements in postural control along with associated alterations in cerebellar activity? METHODS: In this randomized, sham-controlled laboratory study, 22 participants with CAI received active or sham cerebellar tDCS (20 min). Pre- and post-intervention assessments…

Active knee-ankle-foot orthosis with personalized knee trajectory compared with locked orthosis and multi-joint exoskeleton.

Assistive walking devices after spinal cord injury (SCI) should balance functional gait assistance with practical usability. This study presents a user-priority-guided pathway that links a needs-assessment survey to minimal actuation in a knee-ankle-foot orthosis (active KAFO) and objective-driven personalization of knee motion. A retrofit prototype with a single powered knee joint was implemented to preserve the orthosis-like form factor while enabling personalized, impedance-controlled knee motion. Personalization was formulated in OpenSim as a kinematic optimization, where a low-dimensional…

Diagnostic efficacy of pulse oximetry versus Ankle-Brachial index for peripheral vascular disease in type 2 diabetes.

The Peripheral Vascular Disease (PVD) is a debilitating macrovascular complication associated with Type 2 Diabetes Mellitus (T2DM) with significant mortality. This remains undiagnosed in the asymptomatic cases due to diabetic neuropathy. Therefore, it is of interest to evaluate the diagnostic accuracy of the pulse oximetry against the gold-standard Ankle-Brachial Index (ABI). Data shows a high prevalence of PVD with ABI, whereas the pulse oximetry validated moderate sensitivity but low specificity, limiting its utility as a standalone diagnostic tool. While ABI remains the superior standard fo…

Comparison of Fatty Acid Binding Protein 3 and Ankle Brachial Index for Predicting Peripheral Artery Disease Outcomes.

Background: Peripheral artery disease (PAD) impacts more than 200 million individuals globally. Despite its prevalence, management remains suboptimal, partly due to the lack of reliable blood-based biomarkers. The ankle-brachial index (ABI), the current gold-standard test for PAD, is limited by inter-operator variability, misinterpretation, and reduced accuracy in patients with diabetes. Fatty acid binding protein 3 (FABP3) has emerged as a potential biomarker for PAD; however, its prognostic performance relative to ABI remains unclear. This study compared FABP3 and ABI for predicting PAD outc…

Lateralized ankle positioning deficits at initial contact distinguish affected limbs in Hoehn-Yahr stage 1 Parkinson's disease.

OBJECTIVE: Gait impairment emerges early in Parkinson's disease (PD), yet quantitative kinematic characterization at Hoehn-Yahr (H-Y) stage 1 remains limited. This study examined lateralized gait deficits to identify early motor control biomarkers. METHODS: This cross-sectional study enrolled 30H-Y stage 1 participants with PD and 20 age-/sex-/BMI-matched healthy controls (controls). Three-dimensional gait analysis during the medication OFF state assessed spatiotemporal parameters, joint kinematics (hip/knee/ankle angles), and Gait Deviation Index (GDI). Within-PD limb comparisons (affected v…

Case Report: Gait training using a wearable ankle-exoskeleton in a pediatric patient with Guillain-Barré syndrome.

INTRODUCTION: Guillain-Barré syndrome (GBS) is an acute inflammatory demyelinating polyradiculoneuropathy that can lead to severe gait impairment and functional limitations in pediatric patients. Although conventional physiotherapy (CT) is the standard of care, recovery is often prolonged and characterized by persistent deficits in strength and motor control. Wearable ankle-exoskeletons have emerged as a potential adjunctive tool for task-specific gait training; however, their application in pediatric GBS remains largely unexplored. The aim of this single case report was to describe the clinic…

Treatment of superficial wound infection following open reduction and internal fixation of ankle fractures: A scoping review.

INTRODUCTION: Ankle fractures account for approximately 10% of fractures among adults and frequently require surgical intervention, with open reduction and internal fixation being the gold standard. This intervention is associated with a high complication rate. Superficial infections can easily progress due to limited soft-tissue coverage. It is therefore vital to treat early infection. This study has aimed to map the treatments of superficial infection, following open reduction and internal fixation of ankle fractures. METHODS: This scoping review was conducted in accordance with the Joanna …

Distinct effects of three knee-preserving surgeries on hip-knee-ankle alignment in patients with knee osteoarthritis.

BACKGROUND: Unicompartmental knee arthroplasty (UKA), high tibial osteotomy (HTO), and distal femoral osteotomy (DFO) are common knee-preserving surgeries for knee osteoarthritis (KOA), yet systematic comparisons of their effects on lower limb biomechanical parameters remain limited. OBJECTIVE: To retrospectively evaluate and compare the impacts of UKA, HTO, and DFO on hip-knee-ankle alignment in KOA patients, providing evidence for personalized surgical strategy formulation. We hypothesized that these procedures would exert differential effects on specific alignment parameters. METHODS: A t…

Artificial intelligence-guided assessment of the hip-knee-ankle angle shows excellent correlation with experienced human raters.

PURPOSE: The hip-knee-ankle angle is a crucial parameter in orthopaedic surgery for lower limb assessment. However, traditional methods for measuring the hip-knee-ankle angle on standing long-leg anteroposterior radiographs are time-consuming, require significant expertise and lack reproducibility. Given the emergence of artificial intelligence as a promising tool to automate these measurements, this study aimed to assess the accuracy of Gleamer BoneMetrics for hip-knee-ankle angle measurement and its correlation with assessments by experienced orthopaedic surgeons. METHODS: A total of 75 pat…

Lateral ankle ligament reconstruction using free split peroneal brevis tendon and fibula bone pegs: A case report.

INTRODUCTION: Ankle sprain is a frequent soft-tissue injury; however, chronic pain sometimes persists, requiring surgery. Tenodesis repair using free tendon graft provides satisfactory outcomes in cases where no viable ligament structure is available. However, rupture of the graft tendon can occur postoperatively, causing recurrent ankle instability. We report a case of lateral ligament reconstruction using fibula bone pegs to enhance the graft tendon fixation strength for a chronic calcaneofibular ligament injury. PRESENTATION OF CASE: A 44-year-old male presented with right ankle pain that …
