Leishmaniasis due to Leishmania infantum (syn. L. chagasi) infection is a zoonotic disease present mainly in Mediterranean basin, central Asia and Brazil. Besides a limited number of human cases of clinical visceral leishmaniasis, a great number of infections remains asymptomatic. In this review, the prevalence of asymptomatic carriers of L. infantum was evaluated worldwide using parasitological methods or indirect testing such as a skin test or serology. The consequences of the presence of asymptomatic carriers on parasite transmission by blood donation or the development of clinical visceral…

Plasmodium knowlesi morphologically resembles P. malariae; PCR assays are able to differentiate between the 2 species correctly. However, PCR is not available in many hospitals in P. knowlesi endemic areas, particularly in Southeast Asia. In places where PCR is not available, anti-malarial drugs for P. malariae or other non-P. falciparum or P. falciparum species are effective against P. knowlesi. Even with a wrong diagnosis of another malaria species by light microscopy instead of P. knowlesi, the antimalarial drugs given are still effective for treating P. knowlesi infection.

We present a case of disseminated histoplasmosis with acquired immunodeficiency syndrome (AIDS). A 38-year-old woman presented with fever, cough, dysphagia, significant weight loss and a loss of appetite. She also suffered from anaemia and cervical lymphadenopathy. A lymph node biopsy and a bone marrow aspirate showed intracellular yeast forms of Histoplasma capsulatum. She improved with parenteral amphotericin B and oral itraconazole. Even though histoplasmosis is not endemic in South East Asia and India, it should be kept in mind when evaluating AIDS patients in view of the current scenario …

Dengue infection is a major cause of morbidity in tropical and subtropical regions, bringing nearly 40% of the world population at risk and causing more than 20,000 deaths per year. But there is neither a vaccine for dengue disease nor antiviral drugs to treat the infection. In recent years, dengue infection has been particularly prevalent in India, Southeast Asia, Brazil, and Guangdong Province, China. In this article, we present a brief summary of the biological characteristics of dengue virus and associated flaviviruses, and outline the progress on studies of vaccines and drugs based on pot…

Alpha-thalassaemia is one of the most common human genetic disorders. Couples in which both partners carry alpha(0)-thalassaemia traits have a 25% risk of having a fetus affected by homozygous alpha-thalassaemia or haemoglobin Bart's disease, with severe fetal anaemia in utero, hydrops fetalis, stillbirth or early neonatal death, as well as causing various maternal morbidities. This disorder is common in southeast Asia and southern China, and the expanding populations of southeast Asian immigrants in the US, Canada, UK and Europe mean that this disorder is no longer rare in these countries.

Epidemics of tick-borne encephalitis involving thousands of humans occur annually in the forested regions of Europe and Asia. Despite the importance of this disease, the underlying basis for the development of encephalitis remains undefined. Here, we prove the key role of CD8(+) T-cells in the immunopathology of tick-borne encephalitis, as demonstrated by prolonged survival of SCID or CD8(-/-) mice, following infection, when compared with immunocompetent mice or mice with adoptively transferred CD8(+) T-cells. The results imply that tick-borne encephalitis is an immunopathological disease and …

While enforceable National Environmental Quality Standards for pollutants exist in Pakistan, ambient air quality guidelines have never been established. However, there is no reliable epidemiological/toxicological monitoring system in place in Pakistan to provide a basis for establishing guidelines for various air pollutants. This paper reviews the World Health Organization air quality guidelines as well as standards applied in countries of the Eastern Mediterranean and South-East Asia Regions. Based on these, and available data on the existing air pollution situation in Pakistan, national air …

Eosinophilic meningitis is a well-known disease in areas where the rat lung worm Angiostrongylus cantonensis is endemic, i.e. in Southeast Asia and in the Caribbean. We report a case of eosinophilic meningitis due to infection with Angiostrongylus cantonensis in a traveler returning from Cuba. The diagnosis was based upon symptoms of chronic meningitis associated with hyperesthesia and presence of eosinophilia in the cerebrospinal fluid. With increasing mobility rare infections like eosinophilic meningitis due to A. cantonensis may be encountered in the western hemisphere. A Giemsa stain of th…

Pliny the Elder describes in his 'Natural History' XXIX 86 a species of spider dangerous for human beings and looking like a hornet without wings. This description corresponds to Solipugae of the genus Karschia, living in arid areas of Central Asia. These animals are not venomous but can cause harm by their powerful bite and in some cases by a following inflammation of the wound. On the contrary, an animal called "wasp-like", mentioned by Nikander of Kolophon ('Theriaca', v. 738-746) and often regarded to be the same creature, seems to be a typical venomous spider which causes an acute intoxic…

Melioidosis is a disease caused by the facultative intracellular pathogen Burkholderia pseudomallei iand is associated with a high mortality rate. Melioidosis is endemic in the tropics of southeast Asia and northern Australia and is of worldwide concern, particularly as it is a potential agent of bioterrorism or biological warfare. Also of concern is the lack of a fully effective antibiotic regime, as cases of bacteremia have unacceptably high mortality rates and relapse of melioidosis is common. This review focuses on the approaches that have been undertaken towards the development of an effe…

INTRODUCTION: Sandhoff disease is a GM2 gangliosidosis that may present within the first 6 months of life with developmental regression. This is the first report of a pedigree from Southeast Asia.
CLINICAL PICTURE: All the affected siblings presented in the first year of life with developmental regression, spasticity, seizures and loss of vision. The diagnosis was confirmed by an enzymatic deficiency in both beta-hexosaminidase A and B.
CONCLUSION: As the disorder is autosomal recessive, and no curative therapy is currently available, genetic counselling is necessary to prevent the burden of…

Adult T-cell leukaemia/lymphoma (ATLL) is a unique disease with distinct manifestations, a characteristic phenotype and a likely retroviral aetiology. It is unusual in Southeast Asia, though more commonly seen in some countries like Japan. We report a case of this disease in a 71-year-old man in Singapore who presented with papular erythematous eruptions for 6 years and subsequently developed generalised lymphadenopathy. He was diagnosed to have ATLL and, despite an initial response, became resistant to combination chemotherapy. We discuss the aetiology, characteristics and management of this …

The neurocysticercosis is an infestation of the central nervous system by larvae of Taenia solium. The disease is endemic in a few countries of Latin America, Asia, and Africa and is becoming increasingly prevalent in the United States and Europe. The immigration of individuals from endemic areas and the tourism in these areas require a good knowledge of this disease. A retrospective study evaluating the neurocysticercosis lesions with MR and CT is proposed. Parenchymal, subarachnoid and intraventricular cysts are evaluated. We were able to recognise four CT and MR stages. MR was useful in det…

Dengue haemorrhagic fever/dengue shock syndrome (DHF/DSS) is a major cause of hospitalisation and mortality among children in South East Asia. We now report, for the first time, the occurrence of DHF/DSS in Trinidadian children. The presence of vomiting, abdominal pain and hepatomegaly in the setting of a dengue epidemic should alert clinicians to the possibility of DHF/DSS. Timely diagnosis and aggressive supportive treatment are essential for a successful outcome. Source reduction, vector control and community participation are also necessary to avert the South East Asian scenario from emerg…

In the 1950s dengue fever left its home in South-East Asia. The result, as we see it 40 years later, is terrifying. From being an almost unknown infection, dengue fever is today's most common arboviral disease with up to 100 million cases annually. In addition, the clinical picture has changed from a benign influenza-like disorder (classic dengue fever) to a serious disease with bleeding and hypovolemic shock (dengue hemorrhagic fever). In this article the authors give an overview of the present epidemiology, prognosis and possible preventive measures against dengue fever.

Echinococcal disease is endemic in many areas of Asia, Europe, South America, the Near East, Australia, and New Zealand. Hepatic and pulmonary cysts are the most common presentation. Isolated renal hydatid cyst is an uncommon presentation of echinococcal disease, since renal involvement occurs in only 2-3% of cases. We present the case of a young man with renal colic, in whom the diagnosis of renal hydatid disease was established cytologically. Urine cytology, performed because of persistent hematuria, revealed scolices of Echinococcus granulosus and prompted further imaging investigation, and…

Penicillium marneffei is endemic in Southeast Asia. Although Thailand is a country in this region, penicillosis marneffei was rare. It was found occasionally in immunocompromised patients. With the increasing incidence of HIV seropositivity penicillosis marneffei emerged as one of the major problems in HIV infected patients residing in Thailand. The common presenting signs were fever, anemia, hepatomegaly, lymphadenopathy. productive cough and a common skin manifestation as molluscum contagiosum-like lesions. The diseases should be considered in Thais and immunocompromised travelers with a his…

We report the first case of cryptococcosis due to Cryptococcus neoformans var. gattii in France. This Cambodian, non-AIDS patient had been living in France for 23 years when he developed cryptococcosis due to C. neoformans serotype B in 1985. It is possible that he was infected back in Asia or several months before, in Zaire, where he lived between 1981 and 1984. To our knowledge, this is the second case providing clinical evidence for reactivation of latent infection with C. neoformans. The mechanism is probably similar to that for C. neoformans var. neoformans.
