Alpha adrenoceptor regulation of coronary artery blood flow in normal and stenotic canine coronary arteries.

The response of systemic blood pressure, heart rate, lead II ECG and left circumflex (LCX) coronary artery blood flow to left cardiac sympathetic nerve stimulation was measured in pentobarbital-anesthetized, open chest, spinal transected and vagotomized dogs. After beta adrenoceptor blockade, left cardiac sympathetic nerve stimulation produced frequency dependent decreases in LCX blood flow. Selective alpha-2 adrenoceptor blockade with idazoxan produced a greater inhibition of this decrease in LCX blood flow than did selective alpha-1 adrenoceptor blockade with prazosin. In an additional popul

Diagnosis and treatment of aortic valve stenosis.

The present review has attempted to summarize the classic symptoms and signs of aortic valve stenosis, especially in an adult. It is emphasized that all the classic signs rarely are present and their absence may mislead an unwary clinician. The diagnostic help provided by noninvasive tests, including echocardiography and phonocardiography, has been emphasized. A need for cardiac catheterization and angiography in most patients prior to corrective surgery is stressed. The natural history of the disease without operative intervention is dim and a significant risk of sudden death exists. The curr

Effect of TAVI on Epicardial Functional Indices and Their Relationship to Coronary Microvascular Function.

BACKGROUND: The anatomic and physiological changes associated with severe aortic stenosis may influence the functional evaluation of coronary stenoses. In this study, we aimed to assess changes in functional resting and hyperemic indices before and immediately after transcatheter aortic valve implantation (TAVI) and their relationship with microvascular resistance. METHODS: In this pooled analysis of 3 prospective observational studies, fractional flow reserve (FFR), resting full-cycle ratio (RFR), and resting distal coronary pressure to aortic pressure ratio (Pd/Pa) were measured in patients

Validation of quantitative perfusion cardiovascular magnetic resonance employing deconvolution techniques with Tofts, modified-Tofts, and Fermi function models against 15O-water positron emission tomography.

BACKGROUND: Quantitative perfusion cardiovascular magnetic resonance (QP-CMR) allows the generation of pixel-wise myocardial blood flow (MBF) maps using model-based deconvolution with several models including Tofts, modified-Tofts, and Fermi function models. However, the accuracy of pixel-wise MBF mapping has not been fully investigated in humans. The aim of this study was to evaluate the accuracy of advanced QP-CMR using 15O-water positron emission tomography (PET) as a reference. METHODS: Thirty-nine patients (29 men, 68±11years) with known or suspected coronary artery disease underwent bot

Clinical, scintigraphic, and angiographic predictors of oxygen pulse abnormality in patients undergoing cardiopulmonary exercise testing.

BACKGROUND: The oxygen (O2 ) pulse curve obtained at cardiopulmonary exercise testing provides information on cardiorespiratory fitness and the presence of cardiovascular disease. O2 pulse abnormalities have been attributed to myocardial ischemia, among other conditions, but the predictors of abnormal O2 pulse curves are not completely known. HYPOTHESIS: Perfusion abnormalities detected by myocardial perfusion scintigraphy (MPS) may be associated with abnormal O2 pulse curves. METHODS: Forty patients with normal left ventricular function underwent cardiopulmonary exercise testing with radiot

Prevalence of Coronary Microvascular Dysfunction Among Patients With Chest Pain and Nonobstructive Coronary Artery Disease.

OBJECTIVES: This study assessed the prevalence of coronary microvascular abnormalities in patients presenting with chest pain and nonobstructive coronary artery disease (CAD). BACKGROUND: Coronary microvascular abnormalities mediate ischemia and can lead to an increased risk of cardiovascular events. METHODS: Using an intracoronary Doppler guidewire, endothelial-dependent microvascular function was examined by evaluating changes in coronary blood flow in response to acetylcholine, whereas endothelial-independent microvascular function was examined by evaluating changes in coronary flow veloc

Elevated circulating inflammatory biomarker levels in the SIRT1-NF-κB-sCD40L pathway in patients with acute myocardial infarction: a case-control study.

BACKGROUND: Inflammation plays a key role in atherosclerosis development and progression. However, the role of novel inflammatory biomarker pathways, namely the SIRT1-NF-κB-sCD40L, in the etiopathogenesis of human atherosclerosis remains undefined. This study was designed to evaluate the changes and clinical implications of these inflammatory mediators in the plasma of patients with acute myocardial infarction (AMI). METHODS: The peripheral arterial blood of 88 participants (68 patients with AMI and 20 age-matched controls), was drawn prior to performing coronary angiography (CAG). The SIRT1,

The role of metabolic profiling and dietary intervention in the early diagnosis and prevention of atherosclerosis.

PURPOSE OF REVIEW: Atherosclerotic cardiovascular disease (ACVD) causes more than 17 million deaths annually, and traditional risk scores capture only part of individual risk. Metabolomic and lipidomic profiling has emerged as a route to better risk stratification and to mechanistic insight into how diet shapes atherogenesis. This narrative review summarizes the most replicated circulating biomarkers of ACVD and the dietary patterns that modulate them. RECENT FINDINGS: Eight metabolite classes - trimethylamine N-oxide (TMAO), phenylacetylglutamine (PAGln), short-chain fatty acids (SCFAs), lys

The dual role of the brain-derived neurotrophic factor as a regulator of hemostasis and thrombotic risk.

PURPOSE OF REVIEW: Brain-derived neurotrophic factor (BDNF), a neurotrophin associated with neuronal survival and plasticity, has emerged as a significant player in cardiovascular biology. This review examines the evidence for BDNF as a regulator of hemostasis and thrombosis, integrating experimental, clinical, and genetic data to clarify its role in platelet function and thrombotic disease. RECENT FINDINGS: Human platelets represent the largest peripheral reservoir of BDNF, releasing it upon activation by classical agonists. Once secreted, BDNF further amplifies platelet activation through a

Microcytic anemia in older adults: a comprehensive review.

PURPOSE OF REVIEW: Microcytic anemia (MA) is common in older adults (OA) with a significant impact on quality of life and survival. We explore current knowledge on the subject. RECENT FINDINGS: Nonspecific general signs (delirium, cognitive disorders, apathy, bradyphrenia, falls, …) may be at the forefront of MA in OA.Recent advances have clarified the roles of hepcidin, ferroportin and soluble transferrin receptors. Nevertheless, the assay of these new biomarkers is only recommended in cases of uncertainty. The most common mechanism of MA in OA is iron deficiency. It is often both absolute (d

Platelet-Derived PCSK9 Is Associated with LDL Metabolism and Modulates Atherothrombotic Mechanisms in Coronary Artery Disease.

Platelets play a significant role in atherothrombosis. Proprotein convertase subtilisin/kexin type 9 (PCSK9) is critically involved in the regulation of LDL metabolism and interacts with platelet function. The effect of PCSK9 in platelet function is poorly understood. The authors of this article sought to characterize platelets as a major source of PCSK9 and PCSK9's role in atherothrombosis. In a large cohort of patients with coronary artery disease (CAD), platelet count, platelet reactivity, and platelet-derived PCSK9 release were analyzed. The role of platelet PCSK9 on platelet and monocyte

Trimethylamine N-oxide and the reverse cholesterol transport in cardiovascular disease: a cross-sectional study.

The early atherosclerotic lesions develop by the accumulation of arterial foam cells derived mainly from cholesterol-loaded macrophages. Therefore, cholesterol and cholesteryl ester transfer protein (CETP) have been considered as causative in atherosclerosis. Moreover, recent studies indicate the role of trimethylamine N-oxide (TMAO) in development of cardiovascular disease (CVD). The current study aimed to investigate the association between TMAO and CETP polymorphisms (rs12720922 and rs247616), previously identified as a genetic determinant of circulating CETP, in a population of coronary ar

Klotho: a link between cardiovascular and non-cardiovascular mortality.

Klotho is a membrane-bound protein acting as an obligatory coreceptor for fibroblast growth factor 23 (FGF23) in the kidney and parathyroid glands. The extracellular portion of its molecule may be cleaved and released into the blood and produces multiple endocrine effects. Klotho exerts anti-inflammatory and antioxidative activities that may explain its ageing suppression effects evidenced in mice; it also modulates mineral metabolism and FGF23 activities and limits their negative impact on cardiovascular system. Clinical studies have found that circulating Klotho is associated with myocardial

Extracellular vesicles with ubiquitinated adenosine A2A receptor in plasma of patients with coronary artery disease.

Extracellular vesicles (EV) can transfer cellular molecules for specific intercellular communication with potential relevance in pathological conditions. We searched for the presence in plasma from coronary artery disease (CAD) patients of EV containing the adenosine A2A receptor (A2A R), a signalling receptor associated with myocardial ischaemia and whose expression is related to homocysteine (HCy) metabolism. Using protein organic solvent precipitation for plasma EV preparation and Western blotting for protein identification, we found that plasma from CAD patients contained various amounts o

Prevention of Stent Thrombosis in Rats by a Direct Oral Factor Xa Inhibitor Edoxaban.

BACKGROUND/AIMS: Stent thrombosis is a serious complication after percutaneous coronary intervention and femoropopliteal endovascular intervention. The aim of this study was to determine the antithrombotic effects of a direct factor Xa inhibitor edoxaban alone or in combination with antiplatelet agents in a rat model of stent thrombosis. METHODS: Stainless steel stents (4 stents per rat) were placed in an extracorporeal arteriovenous shunt. The shunt was inserted into the carotid artery and the jugular vein in each rat to circulate blood. Stent thrombosis was induced by exposing the stents to

ω-3 Fatty Acids, Impaired Fetal Growth, and Cardiovascular Risk: Nutrition as Precision Medicine.

Precision medicine refers to treatment or prevention strategies in a group of individuals identified by their phenotype or genotype. Dietary components or patterns may play an important role in precision medicine. There is emerging evidence to support a role for n-3 (ω-3) fatty acids in lowering blood pressure and reducing the extent of subclinical atherosclerosis in people born with impaired fetal growth, a group at increased risk of coronary artery disease partly due to an increased risk of hypertensive disorders. The evidence linking n-3 fatty acid intake with less atherosclerosis and lower

Apocynin Prevents Abnormal Megakaryopoiesis and Platelet Activation Induced by Chronic Stress.

Environmental chronic stress (ECS) has been identified as a trigger of acute coronary syndromes (ACS). Changes in redox balance, enhanced reactive oxygen species (ROS) production, and platelet hyperreactivity were detected in both ECS and ACS. However, the mechanisms by which ECS predisposes to thrombosis are not fully understood. Here, we investigated the impact of ECS on platelet activation and megakaryopoiesis in mice and the effect of Apocynin in this experimental setting. ECS induced by 4 days of forced swimming stress (FSS) treatment predisposed to arterial thrombosis and increased oxida

Decreased Helios Expression in Regulatory T Cells in Acute Coronary Syndrome.

Regulatory T cells (Tregs) play an essential role in acute coronary syndrome (ACS). However, there is debate about which Treg subsets are truly critical to ACS. Helios, a transcription factor, was recently reported to be a bona fide marker for natural Tregs or activated Tregs with a suppression function, but little is known about its role in ACS. We therefore examined Helios+ Tregs in patients with ACS, patients with stable angina, and control subjects. 73 patients with ACS, 30 patients with stable angina, and 48 control subjects were enrolled. The frequencies and estimated absolute numbers of
