De novo cerebrospinal fluid-venous fistula following multilevel blood and fibrin patching for spontaneous intracranial hypotension.

Treated cerebrospinal fluid-venous fistulas (CVFs) can recur in up to 15% of cases, and they rarely occur at locations remote from the original lesion; the underlying mechanisms remain unclear. We report the case of a male sexagenarian with spontaneous intracranial hypotension (SIH), diffuse spinal nerve root diverticula, and no CVF identified on baseline lateral decubitus CT myelography (CTM). He underwent high-volume epidural and targeted transforaminal blood and fibrin patching with substantial but incomplete symptom relief, followed by transient rebound intracranial hypertension (RIH). Giant Vesicle Calculus Presenting With Azotemia and Hydroureteronephrosis: A Case Report.

The term "giant vesicle calculus" refers to the urinary bladder stone which is more than 4 cm in its largest diameter. Giant vesical calculus weighing more than 100 g is a rare entity. We report a case of a 63-year male with giant vesicle calculus (measuring 6 × 3 cm) presenting with azotemia and hydroureteronephrosis. Abdominal Ultrasonography confirmed the finding of the vesical calculus with evidence of moderate to gross bilateral hydroureteronephrosis along with diverticulum with thickened and echogenic debris in the urinary bladder lumen suggestive of cystitis. He was then advised for a c

Rare presentation of incarcerated ventral hernia containing transverse colon diverticulitis.

Ventral hernias are common abdominal wall defects; however, the presence of complicated diverticulitis within a hernia sac is exceedingly rare. We report the case of a 76-year-old female with multiple comorbidities who presented with acute left-sided abdominal pain and a tender, irreducible mass. Computed tomography imaging revealed a left lateral ventral hernia containing an inflamed diverticulum of the transverse colon. The patient underwent emergency hybrid laparoscopic surgery involving hernia sac resection, diverticulectomy, and primary abdominal wall repair. Her postoperative course was

Multiple simultaneous small intestinal diverticular penetrations.

Jejunal diverticulosis is a rare condition. Here, we report a rare case of multiple simultaneous small intestinal diverticular penetrations. An 82-year-old man was admitted with a chief complaint of slurred speech. The patient had fever and elevated C-reactive protein (CRP) levels. Incidentally, two abscesses with increased density in the surrounding fatty tissue were identified using computed tomography. An emergency surgery was performed. A series of diverticula were found on the mesenteric side, spanning the area of abscess formation. We performed a partial resection of the jejunum, includi

Acute Colonic Diverticulitis: CT Findings, Classifications, and a Proposal of a Structured Reporting Template.

Acute colonic diverticulitis (ACD) is the most common complication of diverticular disease and represents an abdominal emergency. It includes a variety of conditions, extending from localized diverticular inflammation to fecal peritonitis, hence the importance of an accurate diagnosis. Contrast-enhanced computed tomography (CE-CT) plays a pivotal role in the diagnosis due to its high sensitivity, specificity, accuracy, and interobserver agreement. In fact, CE-CT allows alternative diagnoses to be excluded, the inflamed diverticulum to be localized, and complications to be identified. Imaging f

Fasciocutaneous foreskin twin flaps in hypospadias repair: Reconstructing a functional penis with freestyle design.

BACKGROUND: Hypospadias is the most common congenital urological deformity in males; however, there is still no perfect solution or surgical method for one-stage operations to fix proximal hypospadias with ventral penile curvature (VPC). We developed a fasciocutaneous foreskin twin flap technique that can provide freestyle flap designs in hypospadias repair. METHODS: A retrospective study including 26 patients was performed between January 2017 and June 2020. The surgical procedures included VPC correction, skin flap design, transfer and reconstruction. The urethra and shaft were reconstructe

A new species of Cruzia (Ascaridida; Kathlanidae) parasitizing Salvator merianae (Squamata, Teiidae) from the Atlantic forest in Brazil.

Cruzia lauroi sp. nov. is described from Salvator merianae (Duméril & Bibron, 1839) (Squamata; Teiidae). The new species differs from all previously described species through several morphological characteristics: number of tooth like structures per row in the inner pharynx; and presence of unpaired papillae on the anterior border of the cloacal aperture. However, Cruzia lauroi sp. nov. is closest to C. tentaculata (Rudolphi, 1819), through having similar distribution of male caudal papillae, unpaired pre-cloacal papillae and females with an pre-equatorial vulva. Cruzia lauroi sp. nov. differs

Two simple modifications can potentially change the future of proximal hypospadias surgery. Our series and a review of the literature.

BACKGROUND/OBJECTIVE: To draw a hint towards two simple modifications that could potentially decrease the complication rate. PATIENTS AND METHODS: It was a single center, single operator and retrospective study. All patients with severe hypospadias operated according to koyanagi with or without modifications were presented. The surgical technique was described. Complications like fistula, stenosis, dehiscence and urethral diverticulum were studied. RESULTS AND LIMITATIONS: Nineteen patients were included and presented in a chronological manner. The first four patients were operated according

Caecum actinomycosis with acute abdomen: A case report.

Abdominal actinomycosis, one of the causes of ileocaecal disorders, is usually considered when other more common clinical conditions have been excluded. Actinomycosis is a rare infectious bacterial disorder caused by the Actinomyces species. We present the case of a 38-year male Saudi soldier who presented with pain in the right iliac fossa since 4 days prior to presentation. This stabbing pain started gradually. Based on clinical examination and abdominal ultrasound findings, an appendectomy was performed. Histological examination revealed appendicular actinomycosis with lymphoid hyperplasia,

Pearls and pitfalls in imaging of blunt traumatic thoracic aortic injury: a pictorial review.

Blunt thoracic aortic injury (TAI) occurs most frequently as a sequelae of high impact deceleration such as high-velocity road traffic accidents and falls from height. The burden of mortality and morbidity is high, however advances in pre-hospital care, diagnostic imaging and endovascular therapies have improved outcomes in this group of patients. Emergent treatment depends on accurate, early diagnosis by the radiologist. It is therefore of paramount importance that radiologists are familiar with both the direct (intimal flap, pseudoaneurysm, aortic contour irregularity and contrast extravasat

A rare cause of acute abdomen: Isolated necrosis of the cecum.

OBJECTIVE: We aimed to present a 76-year-old female patient with cecal necrosis, which is a rare cause of acute abdomen in elderly women and a variant of ischemic colitis. MATERIALS AND METHODS: The patient was admitted to hospital with abdominal pain, anorexia and nausea. Physical examination, laboratory parameters and abdominal computed tomography revealed acute abdomen. We operated the patient with below umbilical midline incision. Although the pain localized to the right lower quadrant made us consider acute appendicitis as first diagnosis, other pathologies were also kept in mind for dif

Perinatal Case of Fatal Simpson-Golabi-Behmel Syndrome with Hyperplasia of Seminiferous Tubules.

BACKGROUND Simpson-Golabi-Behmel syndrome (SGBS) is a rare X-linked recessive syndrome characterized by fetal overgrowth. CASE REPORT We present a case of a male infant with SGBS. Abnormal prenatal ultrasound (including congenital diaphragmatic hernia) prompted microarray testing of amniotic fluid cells, which showed deletion on chromosome Xq26.2 affecting the glypican-3 gene consistent with SGBS type I. The infant died six hours after birth and at autopsy showed features of SGBS, including macrosomia, organomegaly, diaphragmatic hernia with consequent pulmonary hypoplasia, cleft palate, large

Urinary tract anomalies in patients with anorectal malformation.

Urinary tract anomalies are common associated anomalies in anorectal malformation (ARM) and major contributory factor for high morbidity and mortality in ARM. In this study, urinary tract anomalies were evaluated in 72 patients of ARM (43 high varieties & 29 low varieties) that were admitted in Dhaka Medical College & Hospital (DMCH) during the period of January 2007 to April 2008. In all cases, evaluation was done by ultrasonogram (USG) and voiding cysto-urethrography (VCUG). Intravenous urography (IVU) was done in selected cases. Urinary tract anomalies were found in 20(27.85%) cases of ARM

Hydrocephalus with spontaneous regression in a 14-year-old girl.

We report on a 14-year-old girl with hydrocephalus that underwent spontaneous regression without any specific treatment, such as ventriculoperitoneal shunt surgery. A 14-year-old girl was referred to our hospital with severe headache, dizziness, nausea, and vomiting. Computed tomography and FLAIR-MRI findings on admission showed markedly dilated lateral, third and fourth ventricles with periventricular hyperintensity and downward displacement of the tonsils induced by dilatation of the fourth ventricle. We diagnosed hydrocephalus of unknown etiology. Although no specific treatment for hydrocep

Duration of follow-up to diagnose hypospadias urethroplasty complications.

OBJECTIVE: We report the time when hypospadias urethroplasty complications (UC) were diagnosed postoperatively. METHODS: The time UC occurred after primary distal and proximal TIP and TIP reoperations was obtained from prospectively maintained databases in consecutive patients. UC included fistulas, glans dehiscences, meatal stenoses, neourethral strictures and diverticula. RESULTS: Of 125 UC, 64% were diagnosed at the first postoperative visit and 81% were encountered within the first year after repair. Median time for diagnosis was 6 months (1.5-95) for fistulas, meatal stenoses/urethra st

Single-incision laparoscopic surgery for small bowel resection with an intraumbilical arcuate incision in children.

Single-incision laparoscopic surgery (SILS) has been performed on children for various procedures. However, few reports are available about SILS for small bowel resection, particularly involving conventional instruments in the pediatric population. Herein, we report four cases of small bowel resection with single umbilical incision and a three-trocar approach. From October 2010 to September 2011, we performed small bowel resection with SILS on four cases, including a boy with an intestinal duplication cyst and three children with Meckel's diverticulum. An intraumbilical arcuate incision was ma
