Bicipital swelling in adult-onset Still's disease.

A patient with typical features of adult-onset Still's disease is described in whom bicipital swellings were a feature. Arthrography of shoulders and elbow joints failed to demonstrate communication with the cyst, suggesting the swelling was due to inflammation in a bursa overlying biceps muscle.

Closed Intramedullary Pinning of Displaced Radial Neck Fracture (Metaizeau Technique).

BACKGROUND: Radial neck fractures account for 1% of all pediatric fractures and 5% to 10% of pediatric elbow fractures. The mechanism of injury is typically a fall with the elbow in hyperextension and the forearm in supination. A valgus force compresses the radial head against the capitellum, causing a radial neck fracture. Displaced radial neck fractures are difficult to treat and account for a disproportionate number of bad outcomes, including malunion, nonunion, and osteonecrosis. The preferred treatment is closed reduction and fixation, as open reduction is associated with an inordinately …

Electrodiagnosis in common mononeuropathies and plexopathies.

Electrodiagnosis has a prominent role in the diagnosis of common entrapment neuropathies (carpal tunnel syndrome and ulnar neuropathy at the elbow) and plexopathies (idiopathic brachial plexopathy and diabetic lumbar radiculoplexopathy). The relevant anatomy and pathology of these disorders is reviewed in the context of electrodiagnosis and prognosis.

Evaluation of Point-of-Care Ultrasound in Paediatric Haemophilic Arthropathy: A Prospective Comparative Study With Comprehensive Ultrasound and MRI.

AIM: To evaluate the diagnostic accuracy of point-of-care (POC) ultrasound (US) in detecting joint changes in haemophilic arthropathy compared to comprehensive US, physical examination and MRI. METHODS: Single-centre, prospective study examining 22 ankles, eight knees and nine elbows of 28 subjects (median/range, 16/6-27 years) with inherited bleeding disorders. Same-day physical and imaging joint assessment was performed using Haemophilia Joint Health Score (HJHS) 2.1, POC-US, comprehensive US and MRI. Two readers reviewed the imaging studies using standardized scoring systems. The inter-rea…

Haemophilia early arthropathy detection with ultrasound and haemophilia joint health score in the moderate haemophilia (MoHem) study.

INTRODUCTION: Detection of early arthropathy is crucial for the management of haemophilia, but data on moderate haemophilia are limited. The … sessed 693 joints in 118 patients. HEAD-US scores (medians [interquartile ranges]) were as follows: elbows 0 points (0-0), knees 0 (0-0) and ankles 0 (0-1). Respectively, by HJHS: elbows 0 (0-1), knees 0 (0-1) and ankles 0 (0-1). Cartilage (14%) and bone (13%) were most commonly affected by HEAD-US. Frequent HJHS findings were crepitus on motion in knee …

Magnetic resonance imaging and joint outcomes in boys with severe hemophilia A treated with tailored primary prophylaxis in Canada.

BACKGROUND/OBJECTIVES: Tailored primary prophylaxis (TPP) is a reduced-intensity treatment program for hemophiliacs with the goal of preventing arthropathy. Our primary aim was to evaluate the joint outcomes of treated subjects using magnetic resonance imaging (MRI) and physical examination as outcome measures. METHODS: Ankles, elbows and knees (index joints) of 24 subjects (median [range] age at start of therapy, 1.6 [1-2.5] years) with severe hemophilia A enrolled in the Canadian Hemophilia Primary Prophylaxis Study (CHPS) were examined by MRI at a median age of 8.8 years (range 6.2-11.5 ye…

[Closed reduction and percutaneous pinning with three Kirschner wires in children with type III displaced supracondylar fractures of the humerus].

OBJECTIVES: We evaluated the results of closed reduction and percutaneous pinning in the treatment of displaced supracondylar fractures of the humerus in children. METHODS: Sixty-one children (41 boys, 20 girls; mean age 7.6 years; range 2 to 13 years) were treated for type III displaced supracondylar fractures of the humerus within the first six hours after trauma. Initial neurologic examinations were normal. Treatment included closed reduction followed by percutaneous pinning with two lateral and one medial K-wires, the last being placed during decreased (80 to 90 degrees) elbow flexion. Th…

Distal humerus fracture in patients over 70 years of age: results of open reduction and internal fixation.

BACKGROUND: Due to the complexity of distal humerusfractures and often poor bone quality in elderly patients, these entities remain a challenge. However, because of a high rate of complications related to total elbow prostheses, reconstruction of distal humerus fractures should still be considered a therapeutic option, also in the elderly patient. The purpose of the present study was to investigate the clinical outcomes after open reduction and internal fixation and to evaluate whether the results justify reconstruction even in elderly patients. We hypothesized that despite advanced age, reaso…

Corticosteroid injections for lateral epicondylitis: a systematic overview.

BACKGROUND: Lateral epicondylitis (tennis elbow) is a common complaint, for which corticosteroid injections are a frequently applied therapy. However, there were no up-to-date reviews available that systematically addressed the effectiveness and adverse effects, including questions concerning optimal timing of injections and composition of the injection fluid. AIM: The aim of the study was to assess the effectiveness of corticosteroid injections in the treatment of lateral epicondylitis (tennis elbow) by systematic review of the available randomized clinical trials. DATA SOURCES: The data so…

Krukenberg's operation. Indications and limitations.

The Krukenberg operation--creating a sensory forceps--is indicated principally in bilateral below elbow amputation with blindness. Other indications are proposed, in particular in children. The limitations to this procedure are essentially aesthetic problems and should not be minimized. The operative technique is described in details.

Conservative Management of Lateral Epicondylalgia: A Review.

A common overuse injury to the elbow is called tennis elbow or lateral epicondylitis (LE). LE is a condition that causes substantial discomfort and dysfunction in the upper extremity. Thermal and electrical agents are examples of conservative techniques. It is a significant public health concern since it frequently occurs due to occupation. It also happens in recurrent upper extremity motions including desktop use, weight training, forceful forearm movements, and repetitive vibratory movements, which are the most common activities performed by an engineer. LE leads to lateral epicondylar pain,…

Case report: Treatment of rare type of open supracondylar humerus fracture.

INTRODUCTION AND IMPORTANCE: Supracondylar humerus fractures are common in children but open type is very rare (almost %1 of all supracondylar humerus fractures) and the treatment strategy for these fractures is yet to be standardized. CASE PRESENTATION: 11-year-old boy was brought to our hospital with pain in his left arm with high energy trauma history of motor accident. On the first examination the elbow was found swollen and 3 cm wide wound observed on the flexor aspect of the elbow and the median nerve was seen stretched on the distal humerus. We shared the 6-month results of our patient …

Downclimbing and the evolution of ape forelimb morphologies.

The forelimbs of hominoid primates (apes) are decidedly more flexible than those of monkeys, especially at the shoulder, elbow and wrist joints. It is tempting to link the greater mobility of these joints to the functional demands of vertical climbing and below-branch suspension, but field-based kinematic studies have found few differences between chimpanzees and monkeys when comparing forelimb excursion angles during vertical ascent (upclimbing). There is, however, a strong theoretical argument for focusing instead on vertical descent (downclimbing), which motivated us to quantify the effects…
