Clinical decision support systems for polypharmacy optimization in older patients: a narrative review.

PURPOSE: Multimorbidity and polypharmacy are increasingly prevalent in the older population and are associated with a higher risk of potentially inappropriate medications (PIM), drug-drug interactions (DDI), and adverse drug reactions (ADR). Although medication review (MR) and deprescribing are effective strategies, their manual implementation can be complex, time-consuming, and prone to clinical variability. Clinical Decision Support Systems (CDSS) offer advanced digital solutions to optimize polypharmacy by analyzing multidimensional clinical data and generating personalized recommendations.

Pharmacist-Led Assessment of Y-Site Compatibility in Pediatric Total Parenteral Nutrition: A Prospective Study of Clinical Practice.

BACKGROUND: Hospital pharmacists are essential for the safe use of total parenteral nutrition (TPN), a complex high-alert medication. Errors may occur during TPN administration; for example, Y-site incompatibilities may cause embolism or thrombosis. Pediatric patients are especially vulnerable because of complex formulations and limited venous access. The stability of both TPN and any drugs administered concurrently depends on physicochemical properties. The American Society for Parenteral and Enteral Nutrition emphasizes the pharmacist's role in preventing such incompatibilities. OBJECTIVE:

Gaps, Barriers, and Facilitators to Implementing Comprehensive Medication Management in Cardiovascular-Kidney-Metabolic Care: A Qualitative Study.

BACKGROUND: Obesity, diabetes, and cardiovascular disease often co-exist in people with chronic kidney disease, placing them at high risk for adverse health outcomes. Availability of highly effective medications that span cardiovascular-kidney-metabolic (CKM) conditions, but limited uptake in real world settings, calls for a new transformative practice paradigm. We sought to identify key gaps, barriers, and facilitators to implementing a standardized CKM-focused comprehensive medication management (CMM) intervention, including needs/solutions among various care team members involved in CKM car

Infection rates of trans-perineal versus trans-rectal prostate biopsy: A Middle Eastern tertiary center experience-Time for a change?

INTRODUCTION: Prostate cancer is the most diagnosed cancer in men, with transrectal ultrasound-guided (TRUS) biopsy being the most frequently used method of tissue diagnosis in the past. With the introduction of the new modality of trans-perineal (TP) ultrasound-guided prostate biopsy, urologists and radiologists adopted this new technique for the decreased rate of complications such as urinary tract infections, bacteremia, sepsis and the avoidance of antimicrobial prophylaxis. We aim to compare the infectious rates of TRUS versus TP biopsy from our Middle Eastern tertiary care center. METHOD

Coronary Artery Calcification on Non-Cardiac Gated CT Thorax Scans: A Single Tertiary Centre Retrospective Observational Study.

BACKGROUND: While the 2024 ESC Guidelines provide guidance on utilising incidental CAC findings from non-gated CT scans to enhance risk stratification and guide treatment decisions, there remain gaps in detailed protocols for managing such incidental findings, particularly in inpatient settings. An incidental finding of CAC in a patient without known atherosclerosis provides an opportunity to assess cardiac risk, promote risk factor optimisation and evaluate need for further cardiac work up. The aim of this study was to assess the prevalence of incidental coronary artery calcification on non-c

Predicting Acute Cerebrovascular Events in Stroke Alerts Using Large-Language Models and Structured Data.

BACKGROUND: Acute stroke alerts are often activated for non-cerebrovascular conditions, leading to false positives that strain clinical resources and promote diagnostic uncertainty. We sought to develop machine learning (ML) models integrating large-language models (LLMs), structured electronic health record data, and clinical time series data to predict the presence of acute cerebrovascular disease (ACD) at stroke alert activation. METHODS: We derived a series of ML models using retrospective data from stroke alerts activated at Mount Sinai Health System between 2011 and 2021. We extracted s

Pre-conception clinical risk factors differ between spontaneous and indicated preterm birth in a densely phenotyped EHR cohort.

BACKGROUND: Preterm birth (PTB) is the leading cause of infant mortality. Risk for PTB is influenced by multiple biological pathways, many of which are poorly understood. Some PTBs result from medically indicated labor following complications from hypertension and/or diabetes, while many others are spontaneous with unknown causes. Previously, investigation of potential risk factors has been limited by a lack of data on maternal medical history and the difficulty of classifying PTBs as indicated or spontaneous. Here, we leverage electronic health record (EHR) data (patient health information in
