Multimodality management of large carotid-space paragangliomas: A single-center case series.

BACKGROUND: Large carotid-space paragangliomas (LCSPs) are uncommon, highly vascular tumors that present significant operative challenges because of their intimate relationship to the carotid circulation, lower cranial nerves, and skull base. The central hazard in surgical management is not tumor size alone but uncertainty regarding vascular anatomy, collateral circulation, and operative conditions before incision. We describe a structured multimodality strategy to reduce this uncertainty before critical dissection. CASE DESCRIPTION: A retrospective review was performed of eight consecutive p

Textbook outcome as a hospital benchmarking metric in adult cardiac surgery.

BACKGROUND: Textbook outcome, defined as survival to discharge without complications or prolonged hospitalization, has garnered increasing interest as a surgical quality metric. The present study used a national database to evaluate the utility of textbook outcomes for hospital benchmarking in adult cardiac surgery. METHODS: All elective admissions entailing coronary artery bypass grafting and/or valve operations were identified from the 2016 to 2022 Nationwide Readmissions Database. Textbook outcome was defined as survival to discharge without cardiac arrest, stroke, prolonged ventilation, r

Long-Term Outcomes of Mechanical Mitral Valve Replacement: A Comparison of Four Valve Types.

Background: The choice of mechanical prosthesis for mitral valve replacement (MVR) is critical, yet data comparing long-term outcomes across different valve types are still needed. This study aimed to compare the long-term clinical and echocardiographic outcomes of four distinct mechanical mitral valve prostheses. Methods: We retrospectively analyzed 431 patients who underwent mechanical MVR between 2009 and 2022 with one of four valve types: Carbomedics (n = 112), Bicarbon (n = 176), ATS (n = 89), or On-X (n = 54). A competing risk regression model was used to identify predictors of a composi

Zero-Contrast Versus Contrast-Guided Left Atrial Appendage Occlusion: A Comparative Analysis of Procedural and Long-Term Outcomes.

BACKGROUND: Contrast injection is standard in left atrial appendage occlusion (LAAO) procedures but may be forgone given advances in imaging and device technology. The contrast-free approach remains underexplored. OBJECTIVES: To compare the safety and efficacy of zero-contrast versus contrast-guided LAAO. METHODS: All consecutive patients who underwent LAAO at our center between September 2018 and October 2022 using Watchman FLX™ devices were enrolled in a prospectively maintained registry. A total of 621 patients were divided between zero-contrast (326) and contrast-guided (295) LAAO. Basel

Early versus delayed oral anticoagulation initiation after transcatheter aortic valve replacement in patients with atrial fibrillation: a retrospective cohort study.

The optimal timing of oral anticoagulation (OAC) initiation after transcatheter aortic valve replacement (TAVR) in patients with atrial fibrillation remains uncertain. This study compared early (within 2 days) versus delayed (5-7 days) OAC initiation on clinical outcomes in this population. In this retrospective cohort study using the TriNetX US Collaborative Network (data through 2024), we identified patients aged ≥ 18 years with atrial fibrillation or flutter undergoing TAVR via percutaneous femoral approach. Patients were categorized by OAC initiation timing post-TAVR. The index date was th

OPTIA-AF trial: A randomized study of rhythm-guided antithrombotic strategy after atrial fibrillation ablation in patients with prior drug-eluting stent implantation.

BACKGROUND: Recent randomized studies suggest that oral anticoagulation may be safely discontinued in carefully selected patients who maintain durable sinus rhythm (SR) after atrial fibrillation (AF) ablation. However, patients with previous drug-eluting stent (DES) implantation represent a distinct population in whom residual coronary ischemic risk persists and long-term antiplatelet therapy remains clinically relevant. The optimal antithrombotic strategy in this setting remains uncertain. OBJECTIVE: The Optimal Postablation Therapy for Ischemic and Arrhythmic Risk in Atrial Fibrillation (OP

Direct oral anticoagulants vs. warfarin for left ventricular thrombus.

AIMS: Left ventricular (LV) thrombus carries a high risk of death and systemic embolism. While warfarin has been the standard treatment, evidence comparing direct oral anticoagulants (DOACs) with warfarin in this setting remains limited. This study aimed to compare real-world, risk-adjusted outcomes of DOAC vs. warfarin use in patients with LV thrombus. METHODS AND RESULTS: We conducted a retrospective cohort analysis using the TriNetX research network database. Adults (≥18 years) with echocardiographically confirmed LV thrombus from 2016 to 2022 were included. Patients with atrial fibrillati

Lost in translation: Gaps in multidimensional assessment undermine the role of clot histology in thrombectomy-An expert roadmap and meta-analytic review.

BACKGROUND AND PURPOSE: Red blood cell (RBC) content, fibrin/platelet (F/P) composition, and clot length are critical determinants of mechanical thrombectomy (MT) outcomes in acute ischemic stroke. However, clinical evidence remains fragmented due to methodological heterogeneity. We systematically evaluated the association between these histologic features, structural heterogeneity, and angiographic/procedural outcomes. MATERIALS AND METHODS: Following PRISMA guidelines (PROSPERO: CRD20251110596), we searched PubMed and MEDLINE for studies published between January 2013 and July 2024. Inclusi

Bilateral Prostate Artery Embolization for Massive Benign Prostatic Hyperplasia in a Frail Elderly Patient With Recurrent Urinary Retention.

Benign prostatic hyperplasia (BPH) is a highly prevalent condition among elderly men and a major cause of lower urinary tract symptoms (LUTS), urinary retention, recurrent urinary tract infections (UTIs), hematuria, and bladder outlet obstruction (BOO). Management of severe BPH in elderly patients with multiple comorbidities remains challenging, particularly in those who are poor surgical candidates. Prostate artery embolization (PAE) has emerged as a minimally invasive alternative for symptomatic relief in patients with moderate-to-severe LUTS secondary to BPH. We present the case of an 87-ye

Thromboembolic risk in subcutaneous versus intravenous immunoglobulin therapy: a systematic review and narrative meta-synthesis.

Intravenous immunoglobulin (IVIg) therapy is associated with thromboembolic complications, including deep vein thrombosis (DVT), pulmonary embolism (PE), ischaemic stroke, and myocardial infarction (MI). Subcutaneous immunoglobulin (SCIg) achieves lower peak serum IgG concentrations and may confer a reduced thromboembolic risk. We systematically reviewed the comparative thromboembolic safety of SCIg versus IVIg across neurological and immunological indications. We searched PubMed, Embase, Cochrane Library, Web of Science, Scopus, and ClinicalTrials.gov from inception to December 2024. Studies

Increased Cardiovascular Risk With Lorlatinib in Patients With ALK-Mutated Lung Cancer: A Real-World Comparative Study.

BACKGROUND: The discovery of driver mutations has transformed advanced non-small cell lung cancer treatment, with ALK (anaplastic lymphoma kinase)-rearranged patients achieving longest overall survival. However, the risk of cancer therapy-related cardiovascular diseases (CTRCVDs) is underrecognized, particularly comparing second-generation ALK-tyrosine kinase inhibitors alectinib and brigatinib to the third-generation tyrosine kinase inhibitor lorlatinib, which offers the longest progression-free survival. We aimed to compare CTRCVDs risks among patients with lung cancer receiving ALK-tyrosine

Does the venous cannulation method affect gaseous embolic load to the patient during extracorporeal circulation?

IntroductionCardiopulmonary bypass (CPB) is essential for cardiac surgery but poses risks, including gaseous micro emboli (GME). While the incidence of stroke-a common clinical consequence of embolism-ranges from 1-5% in cardiac surgery, prevalence of GME during CPB remain poorly understood.ObjectivesTo quantitatively compare GME incidence in the arterial line between cavoatrial and bicaval cannulation during open-heart surgery. Secondary exploratory objectives include evaluating the impact of venous reservoir volume on GME, and the correlation between GME in the venous and arterial lines.Meth
