Interferon Crevicular Fluid Profile and Correlation with Periodontal Disease and Wound Healing: A Systemic Review of Recent Data.

The purpose of the present study was to see if there is a correlation between the effect of interferons in crevicular fluid and periodontitis, evaluating literature. Interferon gamma (IFN-γ) is an immunoregulatory cytokine that, when activated by its receptor, plays an important role in the activation of inflammatory processes, which are the basis of periodontal disease. Stem cells in the periodontal ligament, like stem cells from other tissues, have immunomodulatory capacity and are regulated by some cytokines such as interferon-γ (IFN-γ). The study searched MEDLINE databases from 2008

A simple modified bicarbonate regimen for urine alkalinization in moderate pediatric salicylate poisoning in the emergency department.

A 4-year-old Indian girl was seen in our emergency department for unintentional ingestion of topical medication oil with subsequent salicylate poisoning. Serum levels were 52 mg/dL at 12 hours after ingestion. She was started on urine alkalization therapy to enhance salicylate elimination. This was achieved by a bicarbonate bolus of 1 mEq/kg for an hour and a continuous bicarbonate-potassium-dextrose combination infusion. The infusion regimen was modified from adult recommendations to tailor for pediatric physiological requirements in a young child. This consisted of a combination solution of d

[Antibiotic resistance in Streptococcus pneumoniae strains isolated from sterile body sites].

Antibiotic resistance in Streptococcus pneumoniae has become an important issue in the last years. Penicillin resistance rates vary among countries and among different regions in countries. It is important to know penicillin resistance rates among isolates, in planning empirical antimicrobial therapy in pneumococcal infections. In this study, the antibiotic resistance rates of S. pneumoniae strains isolated from sterile body sites were investigated with both E-test and disc diffusion methods for penicillin, erythromycin, levofloxacin, and with only disc diffusion method for chloramphenicol, ce

Treatment of suspected heat illness.

1. Despite advances in the art and science of fluid balance, exertional heat illness -- even life-threatening heat stroke -- remains a threat for some athletes today. 2. Risk factors for heat illness include: being unacclimatized, unfit, or hypohydrated; certain illnesses or drugs; not drinking in long events; and a fast finishing pace. 3. Heat cramps typically occur in conditioned athletes who compete for hours in the sun. They can be prevented by increasing dietary salt and staying hydrated. 4. Early diagnosis of heat exhaustion can be vital. Early warning signs include: flushed face, hyperv

Detection of transforming growth factor beta 1 mRNA in cerebrospinal fluid cells of patients with meningitis by non-radioactive in situ hybridization.

Meningitis is a serious disease mostly caused by viral or bacterial infections. In complicated cases it may lead to brain damage and death. The infection and cell damage result in a cellular and immunological response. Following this, a high secretion of cytokines can be expected. Cytokines, especially tumour necrosis factor alpha (TNF-alpha) and interleukin-1 (IL-1), promote the inflammatory reactions in the subarachnoid space. Transforming growth factor beta 1 (TGF-beta 1) has antagonistic effects on TNF-alpha and IL-1-mediated processes. Therefore, it suppresses inflammatory reactions. To o

Exudative pleural effusions secondary to gastrointestinal diseases.

Several different diseases of the gastrointestinal tract may have an associated exudative pleural effusion. In the acutely ill patient with a pleural effusion, the possibility of esophageal perforation should always be considered. It is important to establish this diagnosis as soon as possible since the mortality rate increases markedly if drainage of the mediastinum is delayed for even 12 or 24 hours. The best screening test for esophageal rupture is the level of amylase in the pleural fluid. All patients with undiagnosed exudative pleural effusions should have the amylase level in their pleu

Development of Multidisciplinary Consensus-Informed Guidance for the Management of Paediatric Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis Among Clinicians From Australasian Tertiary Referral Hospitals.

BACKGROUND: Paediatric Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) is a rare, severe mucocutaneous reaction requiring coordinated multidisciplinary care. Existing guidelines provide evidence-based recommendations, but implementation across tertiary paediatric hospitals requires practical consensus on local resources. AIM: To develop and evaluate multidisciplinary consensus-informed practical clinical guidance for assessment and management of paediatric SJS/TEN among clinicians from Australasian tertiary referral hospitals. METHODS: A draft guidance document was developed af

Fulminant truncal necrotizing fasciitis of perianal origin with mediastinal extension: a case report.

BACKGROUND: Necrotizing fasciitis (NF) is a rare but life-threatening soft-tissue infection characterized by rapidly progressive necrosis of the fascia and subcutaneous tissue. Early manifestations are often nonspecific, and delayed diagnosis is associated with substantial mortality. Early recognition and immediate intervention are critical to improving outcomes. CASE PRESENTATION: A 34-year-old man presented with progressive left lower abdominal pain and persistent perianal discomfort 5 days after radical drainage of a perianal abscess combined with internal hemorrhoid ligation. Despite init

Balanced crystalloids versus 0.9% saline for fluid resuscitation in children with septic shock: an updated systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: 0.9% (normal) saline remains the most widely used bolus resuscitation fluid in paediatric septic shock, despite a supraphysiological chloride content that may impair renal perfusion and cause hyperchloraemic acidosis. Whether balanced crystalloids, which more closely approximate plasma, improve clinical outcomes in children remains uncertain. METHODS: Following PRISMA 2020 guidelines, PubMed/MEDLINE, Scopus, ScienceDirect and Web of Science were searched upto July 2026 for randomized trials of balanced or buffered crystalloids versus 0.9% saline in children (< 18 years) with sepsi

Clinical Characteristics, Diagnosis, and Treatment of Central Nervous System Candidiasis: A Systematic Review of 418 Cases.

BACKGROUND: Central nervous system (CNS) candidiasis is a rare but life-threatening complication of invasive candidiasis. However, current knowledge derives mainly from isolated case reports, and no comprehensive synthesis has been previously performed. METHODS: We conducted a systematic review of published cases of CNS candidiasis. PubMed and Embase were searched without language or date restrictions. Eligible cases required microbiological or histopathological confirmation of Candida spp. and evidence of CNS involvement on cerebrospinal fluid (CSF) analysis or neuroimaging. Demographic, cli

Effectiveness of Psychotherapy for Post-Traumatic Stress Disorder in Subjects Suffering from Traumatic Brain Injuries After Motor Vehicle Accidents.

Background and Objectives: PTSD and traumatic brain injury (TBI) frequently co-occur in survivors of combat exposure, blasts, assaults, or motor vehicle accidents (MVAs), yet the impact of TBI on the psychotherapy outcomes for PTSD, especially in civilians, remains underexplored and frequently underestimated. Methods: This study focused on analysis of the effectiveness of psychotherapies (trauma-focused: prolonged exposure (PE); non-trauma-focused: self-efficacy-focused cognitive therapy (SEF-CT)) in individuals with PTSD, comparing those with and without TBIs. The data of 45 PTSD patients wit

Continuous Nicotine Monitors for Personal Nicotine Pharmacokinetics: A Receptor-Aware Research Agenda.

A minimally invasive "continuous nicotine monitor" (CNM) would resolve the dynamic nicotine concentration, [nicotine]t, faced by high-sensitivity nicotinic acetylcholine receptors (nAChRs) during and after nicotine intake by individual subjects. Motivations: "Know the potential enemy at an individual level." Smoking or vaping produces an initial "bolus" of nicotine in the blood and brain, lasting ~5 minutes with a peak concentration of ~100-200 nM. The bolus largely governs reinforcement, reward, and cognitive enhancement. A prolonged declining phase of [nicotine], with a half-time of 1-4 hour

Pancreatic Pseudocyst With Thoracic Extension: A Clinicoradiological Case Report.

Pancreatic pseudocysts are encapsulated, enzyme-rich peripancreatic fluid collections that typically develop following acute or chronic pancreatitis due to pancreatic ductal disruption. While commonly localized to the lesser sac, rare mediastinal extension may occur, presenting with nonspecific thoracic symptoms such as chest pain, dyspnea, or dysphagia. Cross-sectional imaging (CT/MRI) is essential for diagnosis. Management is individualized, ranging from conservative medical therapy to endoscopic, percutaneous, or surgical drainage based on symptom severity, complications, and anatomical con

Eosinophilic ascites and eosinophilic gastrointestinal diseases.

Background. Eosinophilic ascites (EA) is characterised by a high eosinophil count in the ascitic fluid and, although very rare, is mainly caused by eosinophilic gastrointestinal diseases (EGIDs), parasitic infections, and hypereosinophilic syndrome (HES). Symptoms caused by EGIDs are varied and depend on the gastrointestinal tract affected as well as the layer(s) involved; typically, EA implies a serosal involvement. Herein, we report our single Centre experience of three cases of EA in patients suffering from EGIDs together with a systematic review of all the other cases described in the lite

Non-Surgical Resolution of Phytobezoar-Induced Bowel Obstruction Using Sodium Bicarbonate and Catheter: A Report of 2 Cases.

BACKGROUND Phytobezoar-induced small bowel obstruction presents significant management challenges, particularly in patients who either decline surgery or have contraindications. These concretions, predominantly composed of persimmon tannins, account for 0.4-4.8% of intestinal obstructions in endemic regions. While surgical intervention remains standard for complete obstructions, non-surgical approaches are increasingly explored for high-risk patients. Current dissolution therapies, including the Coca-Cola protocol, often require large fluid volumes that can exacerbate the obstruction. This stu

Serendipitous discovery of intrathecal catheterization via the lumbosacral plexus in rats: A preliminary study.

BACKGROUND: Intrathecal catheterization is essential for experimental studies, including drug delivery, stem cell therapy, and gene therapy, but it can sometimes be quite costly. Since traditional techniques, such as acute needle puncture and catheterization with or without laminectomy have shown varying success rates and procedural challenges, there is room for improvement. NEW METHOD: This study presents an alternative approach to intrathecal catheterization via the lumbosacral plexus, which was serendipitously discovered during an attempt to cannulate the lymphatic system in male Lewis rat

Lymphoma combined with Tropheryma whipplei infection: A case report and literature review.

RATIONALE: Whipple disease is exceptionally rare in patients with lymphoma in China, particularly when presenting with pulmonary symptoms as the initial manifestation. Respiratory involvement is atypical for Whipple disease and may easily be overlooked, especially in patients receiving targeted drug therapy for lymphoma. In such cases, it can be challenging to distinguish between disease progression, drug-induced pneumonia, and infectious etiologies, as the clinical and radiological findings often overlap. PATIENT CONCERNS: A 39-year-old patient with lymphoma and persistent fever did not resp
