Subtleties of Lisfranc fracture-dislocations.

Tarsometatarsal (Lisfranc) fracture-dislocations can be very difficult to recognize radiographically. The evaluation of the Lisfranc joint can be simplified by meticulously studying the alignment of the metatarsal bases with their corresponding tarsal bones. The usefulness of this observation has been confirmed by: anatomic descriptions, biomechanical analysis, and clinical foot radiographs. Subtle abnormalities identified at the Lisfranc joint using this observation included: metatarsal subluxations identified only on a single projection, associated tarsal subluxations and dislocations, irred

Traumatic hip dislocation at a regional trauma centre in Nigeria.

BACKGROUND: Traumatic dislocation or fracture-dislocation of the hip is an orthopaedic emergency that is steadily increasing in incidence due to high-speed motor vehicular accidents. These injuries need to be recognized early and promptly treated to prevent morbidity and long-term complications. Some of the fundamental issues in the management of traumatic dislocations of the hip are the critical interval between injury and reduction, the type of reduction most suitable for various types of injury and the duration of immobilization that give the best results. This study was carried out at the

Improvements in vertebral body strength under teriparatide treatment assessed in vivo by finite element analysis: results from the EUROFORS study.

Monitoring of osteoporosis therapy based solely on DXA is insufficient to assess antifracture efficacy. Estimating bone strength as a variable closely linked to fracture risk is therefore of importance. Finite element (FE) analysis-based strength measures were used to monitor a teriparatide therapy and the associated effects on whole bone and local fracture risk. In 44 postmenopausal women with established osteoporosis participating in the EUROFORS study, FE models based on high-resolution CT (HRCT) of T(12) were evaluated after 0, 6, 12, and 24 mo of teriparatide treatment (20 microg/d). FE-b

Conservative management of type II and III odontoid fractures in the elderly at a regional spine centre: A prospective and retrospective cohort study.

BACKGROUND: The optimal management of odontoid fractures in the elderly population is unclear and management of this group of patients is complicated by multiple co-morbidities. This study aimed to determine the outcomes after conservative management strategies were applied in this patient group. METHODS: We carried out retrospective and prospective analyses of all patients with axial cervical spine injuries, at a single centre. We included patients aged over 60 years with type II and III odontoid fractures. Information was gathered on demographics, ASA grading-associated injuries and complic

Early Intraprosthetic Dislocation of Total Hip Arthroplasty with Double Mobility Implant: Case Report.

Total hip arthroplasty (THA) is a successful surgery in the treatment of hip pain, but there are potential complications, of which dislocation is one of the most common. Dislocation management is a challenging problem that requires a multimodal approach, and the use of dual mobility implants is an option. We present a patient with a history of femoral neck fracture who underwent THA with a double mobility implant. On the 18 th postoperative day, after a fall to the ground, she developed prosthesis dislocation and had a complication after closed reduction, a subsequent intraprosthetic dislocati

Indian salutation test in acute dorsal carpometacarpal joint dislocation of the ulnar four fingers.

Complete dislocation of the ulnar 4 carpometacarpal joints of the hand (excluding the thumb) is an uncommon injury. Anatomic reduction of the carpometacarpal joint (CMCJ) is essential to maintain function of the hand, and this can often be accomplished by prompt closed reduction and percutaneous Kirschner-wire fixation. The diagnosis is sometimes delayed or missed in the emergency department (ED), and a true lateral radiograph is mandatory in suspected cases. We present a case of dorsal unilateral dislocation of the ulnar 4 CMCJs without associated fracture. The injury was difficult to diagnos

Cosmetic results of supracondylar osteotomy for correction of cubitus varus.

From 1984 to 1995, 19 patients with cubitus varus resulting from supracondylar humeral fractures underwent lateral closing-wedge osteotomies at The Royal Alexandra Hospital for Children. As the indication for osteotomy in all cases was the cosmetically unacceptable cubitus varus, we reviewed the cosmetic results. Seventeen patients were available for review. Subjective reports from patients and parents, as well as objective clinical assessment by one of the authors, were used to assess these results. According to our grading system, 12 (76%) patients had excellent results. These patients and t

Surgical fixation methods for tibial plateau fractures.

BACKGROUND: Tibial plateau fractures, which are intra-articular injuries of the knee joint, are often difficult to treat and have a high complication rate, including early-onset osteoarthritis. The most common treatment for complex tibial plateau fractures is surgical fixation. Additionally, orthopaedic surgeons often use bone defect fillers to address bone defects caused by the injury. Currently, there is no consensus on the best method of fixation and on whether bone defect fillers are necessary. OBJECTIVES: To assess the benefits and harms of different surgical interventions and bone defec

Closing the gap: a novel technique for humeral shaft nonunions using cup and cone reamers.

INTRODUCTION: Nonunion following closed treatment of humeral shaft fracture is estimated to be 5.5%. Many surgical techniques have been described to address humeral shaft nonunion including: open reduction, internal fixation (ORIF) with compression plating and bone graft, dual plating, cortical strut allograft and autograft, and adding biologic augmentation (BMP). The current standard of care includes ORIF with compression plating and bone grafting, but even this technique has an approximated 10% failure rate. We describe a novel surgical technique using cup and cone reamers, which were origin

Influence of Grain-Scale Heterogeneity on Hydraulic Fracturing: A Study Based on a Hydro-Mechanical Phase-Field Model.

Heterogeneity at the grain scale strongly influences hydraulic fracturing in crystalline rock; however, systematic studies quantifying its impacts on the evolution of injection pressure and crack propagation remain limited. To address this gap, we employ a hydro-mechanical phase-field model incorporating Voronoi-based microstructures to systematically quantify the effects of grain-scale heterogeneity on hydraulic fracturing. Two numerical experimental programs are designed to examine the effects of (i) mean grain size and (ii) mineral distribution under different axial stresses. The simulation

Metaphyseal band fractures in pediatric patients treated with denosumab.

OBJECTIVE: Denosumab, a potent antiresorptive agent targeting receptor activator of nuclear factor kappa-B ligand (RANKL), is increasingly used off-label in RANKL-mediated pediatric bone disorders, but concerns remain regarding rebound hypercalcemia and altered bone modeling and remodeling. This study describes clinical, biochemical, and radiological features of metaphyseal sclerotic band fractures in children on denosumab and assesses risk factors with focus on timing and occurrence of rebound hypercalcemia. DESIGN AND METHODS: We conducted a retrospective multicenter study. Clinical, bioche

Management of pediatric ankle fractures: comparison of biodegradable PLGA implants with traditional metal screws.

INTRODUCTION: The relevance of biodegradable implants has gained more importance in modern clinical practice. The study aimed to evaluate the effects and outcomes of ankle fracture treatment with absorbable implants compared to metal screws. These implants are made from poly l-lactic-co-glycolic acid (PLGA), however, there are several other materials available on the market. METHODS: In a retrospective review, a total of 128 patients were under observation, with distal tibial fracture types ranging from Salter-Harris II-IV. In the absorbable group, patients were treated with the implants (n =

Beyond Mortality: Severely Frail Femur Fracture Patients Can Regain Independence after Surgery.

Objectives: Little is known about the post-operative functional outcomes of severely frail femur fracture patients, with previous studies focusing on complications and mortality. This study investigated patient- or proxy-reported outcomes after femur fracture surgery in older adult patients with severe frailty. Methods: This was a retrospective cross-sectional study of older adult (>70 years) patients with severe frailty (defined by a Comprehensive Geriatric Assessment-based Frailty Index (FI-CGA) ≥ 0.40), who underwent femur fracture surgery at a Level 1 Trauma Center. Patients or their proxy

Phantom chain simulations for the fracture of star polymer networks with various strand densities.

Despite many attempts, the relationship between the fracture and structure of polymer networks is yet to be clarified. For this problem, a recent study on phantom chain simulations [Y. Masubuchi et al., Macromolecules, 2023, 56, 9359-9367.] has demonstrated that the fracture characteristics obtained for polymer networks with various node functionalities and conversion ratios lie on master curves if they are plotted against cycle rank, which is the number of closed loops in the network per network node. In this study, we extended the simulation to the effect of prepolymer concentration c on the

Early correction of malunited supracondylar humerus fractures in young children.

INTRODUCTION: Supracondylar fracture malunion usually results in cubitus varus deformity. The long-standing unsightly deformity causes psychological stress to the child and his/her parents in addition to biomechanical disturbances and functional disorders in the affected elbow. The optimal timing of corrective osteotomy is not well-established in the literature. The present study aimed to report the results of early correction of cubitus varus deformity using a simple technique and to focus on the timing of early correction after supracondylar fracture malunion. PATIENTS AND METHODS: Thirty c
