The effect of multimodal temperature management on the incidence of perioperative hypothermia in patients undergoing general anesthesia: a randomized controlled trial.

BACKGROUND: Inadvertent Perioperative Hypothermia (IPH) remains a common clinical problem. This study aimed to evaluate the efficacy of a multimodal temperature management strategy versus passive insulation in reducing the incidence of IPH among patients undergoing general anesthesia. METHODS: This was a single-center, randomized, assessor-blinded randomized trial conducted at our hospital from November to December 2025. This study enrolled 206 patients scheduled for 4-8 h surgeries. They were randomly assigned to either a multimodal group (preoperative warming, forced-air warming blanket, fl

Uterine Leiomyosarcoma Incidentally Diagnosed After Sigmoid Colon Perforation: A Case Report and Review of the Literature Highlighting Individualized Surgical and Oncologic Decision-Making.

Introduction: Uterine leiomyosarcoma (uLMS) is a rare, highly aggressive malignancy arising from the smooth muscle tissue of the uterine wall. It typically presents with abnormal vaginal bleeding, pelvic pain, or a pelvic mass. In rare instances, symptoms may result from local invasion or metastasis. We report a case of uLMS initially diagnosed following colonic perforation due to direct tumor invasion, accompanied by a comprehensive narrative review of the literature on the incidental identification of uterine sarcomas during emergency general surgery to further emphasize the diagnostic chall…

Considering the full care pathway in regional variation in paediatric otitis media treatment in the Netherlands: an observational study.

OBJECTIVE: Ventilation tube insertion for paediatric otitis media (POM), including acute otitis media (AOM) and otitis media with effusion (OME), has been signalled in the past for potential unwarranted treatment variation. Quality improvement initiatives, like Audit & Feedback (A&F), often ignore the care pathway when identifying such variation, possibly overestimating variation at a specific care step. To gain more insight into the effect of prior care steps, this study examined (1) the degree of regional variation in each step of the care pathway (general practitioner (GP) contacts, referra

Optimizing the Recovery of Pediatric Tonsillectomy: Application of Opioid-Free Anesthesia and Analgesia.

PURPOSE: Opioids are considered the gold standard for controlling postoperative acute pain. However, side effects such as nausea, vomiting, respiratory depression, and pain sensitization have gained significant attention. The risks are higher in children with enlarged tonsils and sleep apnea, making effective pain management while minimizing analgesia-related side effects a pressing concern. DESIGN: This was a randomized (1:1), single-blind, parallel-group superiority trial. METHODS: A prospective single-blind study was conducted with 44 children aged 3 to 7 years who planned to undergo tons

Effect of driving pressure-guided individualized positive end-expiratory pressure (PEEP) ventilation strategy on postoperative atelectasis in patients undergoing laparoscopic surgery as assessed by ultrasonography: study protocol for a prospective randomized controlled trial.

BACKGROUND: Ventilator-induced lung injury caused by mechanical ventilation under general anesthesia as well as CO2 pneumoperitoneum and special positions for laparoscopy may increase the risk of postoperative pulmonary complications (PPCs). Lung protective ventilation under general anesthesia is advised by the guidelines to lower the risk of PPCs in surgical patients. However, there is considerable controversy about the optimal level of positive end-expiratory pressure (PEEP) and how to set it. Driving pressure reflects the overall respiratory stress and high driving pressure is an independen

Knowledge, Perceptions, and Expectations of Dental Anesthesia Among Patients Attending the College of Dentistry Clinic in Hail, Saudi Arabia: A Cross-Sectional Study.

INTRODUCTION: Anesthesia is unique because it is not a direct means of treatment; rather, it allows other physicians to do things that may treat, diagnose, or cure an ailment, which would otherwise be painful or complicated. In the preoperative period, there are a lot of goals of preparation, but one of the most important is to eliminate anxiety. The current study aims to assess the patients' knowledge regarding the anesthetist's role and their knowledge about dental anesthesia. METHODS: It is a sectional questionnaire prepared at the Faculty of Dentistry, Hail University, Hail, Saudi Arabia.

Impedance planimetry and panometry (EndoFLIP™) can replace manometry in preoperative anti-reflux surgery assessment.

BACKGROUND: Endoluminal functional impedance planimetry and panometry assesses secondary peristalsis in response to volumetric distention under sedation. We hypothesize that impedance planimetry and panometry can replace high-resolution manometry in the preoperative assessment prior to anti-reflux surgery. METHODS: Single institution prospective data were collected from patients undergoing anti-reflux surgery between 2021 and 2023. A 16-cm functional luminal imaging probe (FLIP) assessed planimetry and panometry prior to surgery under general anesthesia at the start of each case. Panometry wa

The Management of Postoperative Cognitive Dysfunction in Cirrhotic Patients: An Overview of the Literature.

Background and objectives: Postoperative cognitive dysfunction (POCD) represents a decreased cognitive performance in patients undergoing general anesthesia for major surgery. Since liver cirrhosis is associated with high mortality and morbidity rates, cirrhotic patients also assemble many risk factors for POCD. Therefore, preserving cognition after major surgery is a priority, especially in this group of patients. The purpose of this review is to summarize the current knowledge regarding the effectiveness of perioperative therapeutic strategies in terms of cognitive dysfunction reduction. Dat

Prediction of Acute Kidney Injury after Extracorporeal Cardiac Surgery (CSA-AKI) by Machine Learning Algorithms.

BACKGROUND: Acute renal failure after extracorporeal cardiac surgery under general anesthesia is high and unpredictable, but machine learning algorithms could change this. A feasible approach is to use machine learning models to construct models to predict acute kidney injury after extracorporeal cardiac surgery (CSA-AKI) and screen for the best predictive model. METHOD: From January 2014 to December 2021, 2187 patients undergoing extracorporeal cardiac surgery at the third hospital of Hebei Medical University and the first medical centre of Chinese PLA General Hospital were collected in this

Outcomes of Intramuscular Gluteal Augmentation With Implants Using Tumescent Local Anesthesia.

BACKGROUND: Tumescent local anesthesia (TLA) describes the practice of injecting a very dilute solution of local anesthetic combined with epinephrine and sodium bicarbonate into the tissue until it becomes firm and tense to obtain local anesthesia and vasoconstriction. The use of TLA in augmentation intramuscular gluteoplasty has never been described for implants positioning. Advantages of the TLA technique include a reduction in blood loss through epinephrine-induced vasoconstriction and hydrostatic compression from the tumescent effect. We describe TLA technique for primary intramuscular glu
